Pregnancy Complications Research
Pregnancy is an exquisitely controlled, extremely complex process in which numerous biological safeguards guarantee a successful outcome. The reason for these safeguards is evident when we consider the implications of their failure. Premature birth is a devastating pregnancy complication, accounting for the majority of infant deaths and bad neurological outcomes. Preeclampsia is a pregnancy problem that affects mothers and babies. It increases perinatal mortality five-fold. Since preeclampsia is pregnancy specific, the treatment is early delivery, and this accounts for 15% of all preterm births. Whereas in the USA and developed countries the risk is primarily to the infant, in developing countries mortality from preeclampsia accounts for 50,000 maternal deaths yearly.
Magee-Womens Hospital has a long history of involvement in pregnancy research. This history antedates Magee-Womens Research Institute (MWRI). Starting with Dr. Zeigler's invention of a cord clamp over 60 years ago; continuing with the participation of Magee in the original studies (20 years ago) indicating that artificial surfactant administered to preterm babies prevented neonatal lung disease; and including the participation of Magee as a charter member of the NICHD Maternal Fetal Medicine Network for clinical trials 15 years ago, Magee has been at the forefront of clinical research in pregnancy problems. With the development of the MWRI ten years ago, this clinical research expanded and is now accompanied by mechanistic studies.

A retrospective study has been conducted in order to analyse pregnancy history and identify risk factors during the pregnancy which can lead to complications both during the pregnancy and during the delivery. The analysis of the pregnancy history of women at risk for age (women aged less than 18 and more than 35) has been conducted in order to identify possible complication during pregnancy and delivery and to analyse the outcomes.
6.1 Methods
A revision of the total records of the outcomes occurred in 1996, both from the Delivery Register and from the Maternal and Child Health cards has been done. A nurse, our local assistant, has conducted the whole revision and the selection of all cases of complications as: Miscarriages/Abortion

Neonatal deaths
Premature births
In addition, all the antenatal records belonging to women 'at risk' for age (aged less than 18 and more than 35 years) have been collected and analysed to investigate the outcomes. DES-Related Pregnancy Complications Lower conception rates overall are reported for DES daughters. See Infertility Of those who do conceive, spontaneous miscarriages, including tragic, mid-trimester losses, ectopic pregnancies, and preterm births are greatly increased. The risk is greatest for women with demonstrated structural abnormalities such as hypoplasia, a T-shaped uterus or cervical deformities. There is a five-fold increase in ectopic (tubal) pregnancies for DES daughters. This is a pregnancy that implants in the tube as opposed to the uterus. All DES daughters should be considered at risk for an ectopic pregnancy. Early confirmation of pregnancy and vigilance in treatment is essential, since an undiagnosed tubal pregnancy can rupture and be life-threatening. While as many as half of all DES daughters may have no problem getting pregnant and delivering healthy children, you should be aware of the risks associated with your exposure and see a doctor skilled in handling them. Preparation, good medical care and paying attention to your own body go a long way towards a successful pregnancy. Infant intrauterine growth restriction (IUGR), a complication of pregnancy, results in the delivery of hypoglycemic growth-retarded infants. Abstract:
Objectives: Prenatal care is commonly understood to have a beneficial impact on birthweight. This study describes socioeconomic differences in utilization of prenatal medical care and birthweight in a population with universal medical insurance.
Methods: Measures of prenatal care utilization, incidence of pregnancy complications, and birthweight were obtained from physician reimbursement claims and hospital separation abstracts for 12,646 pregnant women. Maternal socioeconomic status was derived from small-area census data.
Results: Infants born to women in the poorest income quintile had lower birthweights than infants born to wealthier women. Much of the difference was associated with a higher prevalence of complications, smoking, unmarried status, and inadequate prenatal care among low-income women. The difference in birthweight between adequate and less than adequate care groups was small, and the benefit associated with prenatal care was no greater among women with pregnancy complications.
Conclusions: The lower utilization of prenatal care by poorer women accounted for a small proportion of the difference in birthweight. Socioeconomic differences in birthweight are primarily attributable to factors not directly influenced by early prenatal medical care.

Dear Dr. Henney:
As a pediatrician and former Chair of the National Academy of Sciences' Committee on Pesticides in the Diets of Infants and Children, I am writing to express my grave concern over the Food and Drug Administration's final regulation defining the types of statements that can be made concerning the effects of dietary supplements on the structure or function of the human body. I am particularly concerned by your decision in this regulation to classify various complications of pregnancy such as morning sickness and edema of pregnancy as "non-diseases," a decision that appears to have been made in order to exclude these conditions from the scope of regulation, but that runs counter to medical understanding and practice. This decision has the potential to put pregnant women and their offspring at serious risk of unwitting exposure to chemical teratogens in inadequately labeled health products and patent medicines.
A major finding of the NAS Committee on Pesticides in the Diets of Infants and Children that I chaired is that fetuses, infants, and children are fundamentally different from adults in their vulnerability to a wide range of xenobiotic compounds. We encapsulated this concept in the phrase "Children are not little adults." It was on the basis of our recognition of fetuses', infants', and children's unique patterns of exposure and unique susceptibility to chemical pesticides that we recommended that legislative and regulatory procedures for establishing standards, "tolerances," for pesticide residues in diet in the United States be fundamentally revamped so as to take cognizance of the special risks and vulnerabilities of children. This recommendation was accepted by the Congress and formed the basis for the Food Quality Protection Act of 1996.

Researchers in Glasgow, Scotland, suggest that recent evidence justifies monitoring women who have had difficult pregnancies - for they are at risk of health problems later on in life. For instance, those who develop diabetes during pregnancy are at increased risk of mid-life diabetes. Similarly, those who have a premature or low weight baby may also be at increased risk, as are those who develop pre-eclampsia. The researchers suggest it would be worthwhile screening such women for risk factors, like high cholesterol and high blood pressure. The pregnancy information could be logged in the family doctor's database and used to guide mid-life screening - from the late 30s onwards. It seems that there may be underlying genetic factors influencing both difficult pregnancies and diabetes and heart disease. Further research will help determine whether lifestyle factors also play a role.     If untreated pre-eclampsia can develop into muscle irritability, twitching, epileptic fits and ultimately may result in a stroke. It is one of the major causes of maternal mortality and we take it very seriously. If we suspect you may be developing pre-eclampsia, you will be admitted to hospital. Pre-eclampsia settles after delivery and if you are in danger, your baby will be delivered early. Pre-eclampsia is the principal reason that blood pressure is monitored so closely in the ante-natal period. Arbitrarily, the cut off for normal blood pressure is set at 140/90 mmHg with high blood pressure defined as being above this. However, your doctor or midwife will be taking into consideration your normal level of blood pressure at previous visits. This might vary a little from visit to visit and a difference of 10-20 mmHg may not be important but a rise of 30/20 mmHg above your booking blood pressure is probably significant. We also check your urine for protein at each visit and that is why it is important to provide us with a fresh specimen at each attendance.

    With advances in medical care, women with diabetes are almost as likely as women without diabetes to have an uncomplicated pregnancy and a healthy baby, as long as blood glucose levels are well controlled beginning prior to pregnancy. However, women with poorly controlled diabetes, especially pre-existing diabetes, are at increased risk of certain pregnancy-related complications. These include:
    Miscarriage Pregnancy-related high blood pressure Polyhydramnios (an excess of amniotic fluid, which can contribute to pre-term labor) Pre-term delivery, and stillbirth
