Back to Home Page Males after age 50 are prone to the development of BPH (benign prostatic hypertrophy). As the prostate gland surrounds the male urethra, enlargement of this gland puts pressure on the urethra resulting in the symptoms of decreased force and increased frequency of urination. Men often awaken multiple times at night to urinate. Other symptoms may include sensation of pressure as well as dribbling after urination. Prostatic hypertrophy or BPH is a common condition occurring in most males with age, beginning usually at 50+. In medical school we were instructed that about 50% of males over age 50 have some degree of BPH, and at age 80, 80% of males have already developed this condition. In reality, after talking to several men ages 50 to 80, the comments I received is: “No, it is more like 100% of all males …” Symptoms may include a frequent, urgent need to urinate, particularly at night with frequent night time awakenings, difficulty or hesitancy in urination, inability to empty the bladder, and a weak urinary stream or dribbling. The issue of affecting libido is somewhat similar. The prostate gland is like a switching valve. It switches between allowing urine to pass through to that part of the urethra on the distal side of the prostate, or allowing semen to pass through. The male sperm duct passes through the prostate gland, just like the male urethra; but also attached are the male seminal vesicles. These are one or two sacs that add fluid to the sperm to form semen. The prostate gland adds the secretions from the seminal vesicles, to the sperm, and then directs the semen flow out the male urethra. Restricted passages through the prostate gland make this a difficult task, and thus, male libido can and usually is affected to some degree.

In Brief: In men with BPH, oral administration of Serenoa repens 160mg twice daily for 1 to 3 months was generally superior to placebo in improving subjective and objective symptoms of BPH. The drug appears to have similar efficacy to finasteride as evidenced by data from over 1000 men. Few significant differences were demonstrated between Serenoa repens and a -adrenoceptor blockers in small comparative studies. In Brief: Benign prostatic hypertrophy is one of the common complaints of older men who visit their primary care physicians. Nonoperative therapy, such as drug therapy or lifestyle modification, is aimed primarily at reducing bothersome lower urinary tract symptoms and preventing serious morbidity such as urinary retention and renal function impairment. As more men reach advanced age, it becomes crucial for the primary care physician to be aware of the epidemiology, pathophysiology, natural history, clinical presentation, and therapeutic modalities available for the treatment of clinically significant cases. B enign prostatic hypertrophy (BPH) is the nonmalignant enlargement of the prostate gland from an increase in cellular growth. BPH is the most common benign neoplasm among men, and its incidence increases steadily with age. Clinical BPH is well defined and quite prevalent: Up to 20% of men between 40 and 64and 40% of men older than 65are afflicted. Histologic BPH is found in 50% of men older than 60 and up to 88% of men older than 80.

    BENIGN PROSTATIC HYPERTROPHY Gabe Mirkin, M.D. If you are a man who finds it difficult to start your stream, check with your doctor to see if you have an infection or a benign or cancerous enlargement of your prostate. Your doctor usually examines your prostate for potentially cancerous nodules, orders a PSA blood test and checks your urine for infection. If your doctor finds a nodule, he usually orders an ultrasound test and then cuts out small pieces of the nodules which are then analyzed for cancer. If he finds no cancer, he must check you for infection or benign prostate enlargement. It is so unusual to find bacteria in a man's urine, that it requires further tests to check your entire urinary system. PSA or Prostate Specific Antigen is made by prostate cells and released into the bloodstream. The larger your prostate, the more cells you have to release PSA into your bloodstream. Therefore, PSA is a measure of the size of the prostate gland, rather than just prostate cancer. A sudden rise in PSA means that the gland is enlarging rapidly which can be a sign of cancer, but it also can mean infection or benign enlargement. If you have an elevated PSA, burning on urination, feeling that you have to urinate all the time and terrible discomfort when your bladder is full, the odds are overwhelming that you have an infection, that can be very difficult to cure.

When a man has to stand for long time before he can start urinating, he is usually diagnosed as having Benign Prostatic Hypertrophy, which means that his prostate gland is so large that it squeezes his urinary tube and blocks the flow of urine from his bladder to the outside. Doctors don't have the foggiest idea what causes BPH, but they continue to cut out or burn prostates, even though the surgery often causes men to become incontinent, impotent, ejaculate backwards into the bladder, and have prostate infections. This study shows that the prostate enlarges much faster in men with high blood insulin, high blood cholesterol, lower blood levels of the good HDL cholesterol, diabetes, high blood pressure, or obesity. All these conditions increase your risk for a heart attack. If future studies support this one, you can protect your prostate by eating a plant-based diet, restricting refined carbohydrates such as bakery products, pastas, sugar-added foods, and fruit juices, and checking to see if you have diabetes.     Benign Prostatic Hypertrophy
    What is it? Benign prostatic hypertrophy is also called "BPH." It is when the prostate gland slowly grows until it is larger than normal, or "enlarged." BPH is a very common problem in aging men. It is more common in men over 45 to 50 years of age. The prostate is a male sex gland that makes fluid called semen. It sits below the bladder and wraps around the urethra like a donut. The urethra is a tube that carries urine from the bladder into the penis. The urine is then passed out of the body. As the prostate grows, it can squeeze the urethra making it harder to pass urine. Causes: It is not known what causes BPH. It may be just part of getting older. Some care givers believe it is caused by the continual and cumulative exposure to testosterone (male sex hormone) as men age. Signs and Symptoms: Most men with BPH feel like they need to pass urine often. Some men may pass little or no urine. You may dribble or leak urine. You may pass urine while you sleep. Or you may need to wake up at night to urinate . You may have to push to start your urine stream. You may feel that you need to pass more urine even if you just did. You may also think your urine stream is weak. Your urine may look pink or red. This may mean there is blood in it. Wellness Recommendations: If you have prostatic enlargement you should avoid stimulants or decongestants (often found in over-the-counter cold and allergy medications) as these can contract the muscle fibers in the prostate and make the symptoms much worse.

    Background
    Finasteride decreases the conversion of testosterone to dihydrotestosterone and can reduce the volume of the prostate in men with prostatic hypertrophy. This study was designed to look at the efficacy of finasteride in improving symptoms and reducing the incidence of urinary retention and prostate surgery in men with symptomatic, enlarged prostate glands.
    Methods
    Patients Patients were enrolled between 1990 and 1992. Inclusion criteria: Men with a serum PSA between 4.0 and 9.9 were required to have a negative biopsy for cancer. Exclusion criteria: Use of alpha-blockers, anti-androgens, chronic prostatitis, prostate or bladder cancer or surgery.
