Proteus syndrome The term "Proteus Syndrome" is applied to an extremely variable condition involving atypical growth of the skin, skull and bones associated with a variety of other symptoms. It has been suggested that Joseph Merrick (known as the Elephant Man) had suffered from this condition rather than neurofibromatosis, as was initially suggested This condition was first identified by Michael Cohen, Jr., in 1979 and in 1983, a German pediatrician, Has Rudolf Wiedemann, named it Proteus Syndrome, after the Greek God Proteus "the polymorphous" because of the variable manifestations in the four unrelated boys first identified with the syndrome. The condition is extremely rare but there are over 50 reported cases worldwide. Cases have been isolated occurrences in families, suggesting that the condition is not genetic. Subscription Images in Clinical Practice Indian Pediatrics 2001; 38: 677-678 Proteus Syndrome A term newborn was found to have a large lympho-hemangioma on the left side involving the chest and abdomen ( Fig.1 ). Multiple small nevi were seen over the hemangioma. This was associated with hyper-trophy on the left side. The baby’s left sole was bigger than the right sole ( Fig. 2 ). Width of left and right sole at level of head of metatarsals was 4.5 cm and 4.2 cm, respectively. The length of foot on the left and right was 9.3 cm and 8.2 cm, respectively. Thickening, excessive wrinkling and furrow-ing of the skin was also seen on the left sole. Congenital dislocation of hip was present on the left side. Bilateral inguinal hernia was present. Ultrasonography of the head, chest and abdomen were normal. Skull X -rays and a skeletal survey did not reveal any abnormality. The initial platelet count was 70,000/mm3 but subsequent counts were normal. Proteus syndrome is a bizare clinical condition characterized by overgrowth of various body parts, hemihypertrophy, multiple cutaneous nevi, unusual skeletal malforma-tions, soft tissue tumors and visceral anomalies. It is a congenital hamartomatous disorder and hence is quite similar to other disorders of overgrowth. However, the salient clinical features of massive overgrowth, hamartomata, thickening of palms and soles and hemihypertrophy are usually present to enable a confident diagnosis.

Proteus Syndrome Common Signs of Proteus Syndrome * Partial enlargement of the hands and/or feet, usually asymmetric. * Pigmented nevi(darkened, discolored skin) throughout the body. The skin is often raised and rough. * Subcutaneous tumors (beneath the skin): usually on the skin surface, resembles lymph gland tumors (lymphangiomas), and occasionally, fat cell tumors (lipomas). They can develop and grow rapidly, but can also remain unchanged or even show some signs of regression. * Skull anomalies: macrocephaly (large head) or asymmetry of the skull due to hemi hypertrophy may be present. * Plantar hyperplasia: deep lines and overgrowth of soft tissue on the soles of feet . Treatment/Prevention Proteus Syndrome is an extremely variable condition. The severity of the syndrome ranges from mild cases to severe. There is, as yet, no way to detect who will have a mild case and who will develop serious complications. While evidence of this syndrome has been cited in medical literature as early as 1907, recognition has been difficult due to the variability of the manifestations and the fact that only 100cases of this conditon has ever been reported. As of right now there is no possible cure for this rare disease, although surgery and medical care will require a multidisciplinary approach covering orthopaedic, dermatological, genetic, surgical, dental, ophthalmological, radiological and psychological management. 3rd Annual Alex Hoag Run for Sunshine In Memory of Alex Hoag A year after his birth in 1990, Alex Hoag was diagnosed with Proteus Syndrome, a rare overgrowth disorder that produces exaggerated overgrowth in skin, fat, and bones. Against great odds, he survived 10 major surgeries and a body that kept him in a permanent and oftentimes uncomfortable sitting position. His family and friends marveled at his knowledge of famous jazz artists, his excitement over the often overlooked beauties of life, and his never-ending laughter. As a 9 year old boy in 3 rd grade at Vera Scott Elementary in Colorado Springs he enjoyed the beat of a good song, playing tether ball with his friends, the loud noise of a Harley Davidson, his family, and he especially enjoyed sitting in the sunshine and dancing! He fought his battles with courage and faith and a wonderful sense of humor. Alex unexpectedly died of complications due to Proteus syndrome on Sept. 20, 1999. His spirit remains a gift to us. Alex loved to sit in the sunshine. We will run in memory of Alex. The Alex Hoag Run for Sunshine has been founded to help all people who suffer with Proteus Syndrome. Transcripts Neurofibromatosis and Proteus Syndrome, a condition also known as "The Elephant Man's Disease," are often confused as the same disorder. While both are genetic disorders,their diagnoses and manifestations are separate. Neurofibromatosis (NF) is defined as a disorder causing the growth of tumors on the nerves anywhere in the body at anytime. Half of the cases of NF result from spontaneous genetic mutation, while half are inherited. Proteus Syndrome affects the body through abnormal, asym-metrical growths and diverse abnormalities involving the skin, face, eyes, ears, lungs, muscles, and nerves. There is no evidence that Proteus Syndrome can be transmitted genetically. The story of Joseph Merrick, the man who was called "The Elephant Man," is recounted in the play and the movie by the same name. These dramas are based on an essay by Sir Frederick Treves, the medical doctor who 'discovered' Merrick in a carnival sideshow and cared for him until his death in 1890. The record of their association offers a window into the formations of medical history and has been used to provoke significant discussion about medical ethics, patient's rights and human dignity. Although Dr. Treves was a contemporary of Dr. Frederick von Recklinghausen, the German scientist who originated the term "Neurofibromatosis," he never referred to Joseph Merrick's condition as Neurofibromatosis. But, in 1971, based on the medical knowledge available to him at the time, Ashley Montagu, a medical anthropologist and professor at Princeton University, suggested the link in his book, 'The Elephant Man'. The association of the two conditions has since worked its way into popular conversation.
