Clinical Profile:
A four-month-old girl presented for investigation of repeated attacks of seizures, extensive facial abnormalities and protruding right eyeball. She was the first child of non-consanguineous parents. The baby did not cry immediately after the birth. However, the five minute APGAR score was 9/10. Physical examination revealed right hemifacial and calvarial enlargement, soft tissue swelling over the right side of face and neck, macrocephaly, right facial palsy, right proptosis, hypertrophy of the right upper limb, hyperpigmented skin lesions on the face, trunk and back and protuberant abdomen. Head control was still not achieved. Ophthalmologic examination revealed bilateral optic atrophy. Fig.1 :

Etiology of Sporadic Multiple Congenital Anomaly Syndromes

Summary:
This study will examine rare congenital disorders that involve malformations and abnormal growth. It will focus on patients with Proteus syndrome, whose physical features are characterized by overgrowth, benign tumors of fatty tissue or blood vessels, asymmetric arms or legs, and large feet with very thick soles. The study will explore the genetic and biochemical cause and course of the disease, the changes in symptoms over time, and the effects of the disease on patients. Patients with Proteus syndrome and their parents may be eligible for this study. Parents will be studied, when possible, for comparison of molecular findings. Study candidates will have a medical history and physical examination, including X-rays and possibly other imaging tests, such as computerized tomography (CT), magnetic resonance imaging (MRI) and ultrasound. Other tests and examinations may be done if needed. Those enrolled in the study will have will be interviewed or complete questionnaires, or both, about how their disease affects them. (Parents will be asked about their feelings about having a child with a rare disorder.) Patients will provide a small blood sample for research and may be asked to undergo biopsies from a normal area of skin and from a tumor.

Received: 9 September 1998 Accepted: 28 December 1998 Abstract The Proteus syndrome is a rare hamartoneoplastic syndrome that may affect the brain, skull, and extracranial head and neck. We present a case with severe, characteristic findings. Brain abnormalities are not common in Proteus syndrome; when present, hemimegalencephaly and migrational disorders are typically seen, commonly with an associated seizure disorder. Maxillary and mandibular dysmorphism may occur, including unilateral condylar hyperplasia. Subcutaneous fatty, fibrous, lymphangiomatous masses commonly seen in this syndrome may involve the neck and face, leading to disfigurement and potential airway compromise.
