Idiopathic Intracranial Hypertension
(Pseudotumor Cerebri)
Idiopathic Intracranial Hypertension is a condition of high pressure in the fluid around the brain. It is also known as pseudotumor cerebri because there are some of the signs and symptoms of a brain tumor without a brain tumor being present (pseudo meaning false). The space around the brain is filled with a water-like fluid. (Fig 1.) If there is too much of this fluid present, (for example, if not enough being absorbed) (Fig 2), the pressure around the brain rises. This is because the space containing the fluid cannot expand. It is this high pressure that produces the symptoms of idiopathic intracranial hypertension (idiopathic means unknown cause; intracranial means inside the head; hypertension means the fluid is under high pressure). Your doctor thinks you may have pseudotumor cerebri. This is a condition in which high pressure inside your head can cause problems with vision and headache. In the days before CT and MRI scans, doctors who noted swelling of the optic disc (the beginning of the optic nerve in the back of the eye) were concerned about the possibility of a tumor within the head. Patients with optic disc swelling but no evidence of a tumor were said to have "Pseudotumor." Anatomy

Pseudotumor cerebri (PTC) or Idiopathic Intracranial Hypertension (IIH) is a disease where there is increased intracranial pressure that mimics a brain tumor. This condition, which affects mainly women of childbearing age, is not widely known and little information is currently available about it. This web site will attempt to increase the availability of information about this disease. This site will be changed and updated as new information becomes available.

    Pseudotumor Cerebri

Question Posted By: msb on Thursday, June 28, 2001
How can I be experiencing so many visual problems without having swelling of the optic nerves? Can PTC be caused by the narrowed carotid artery from the dissection?
Answer Posted By: CCF-Neuro-M.D.-RPS on Thursday, June 28, 2001
Dear MSB: Sorry that your having trouble with your vision. Usually a carotid dissection should not cause PTC. Since CSF is absorbed by the venous not the arterial system, it doesn't make too much sense that it has a connection to the PTC. We most commonly see PTC in overweight females and the usual is to start diamox and encourage weight loss. There are medications such as vitamin A and medications such as tetracyclines that can induce PTC. Yes, one can have visual problems without frank papilledema but it is rare and usually it is just a matter of time before it develops. Since the diuretics are not working, it may be important to begin another medication such as a steriod burst. As you are probably aware, the big worry is loss of vision with PTC. Sometimes, it even comes to having to shunt a patient, draining CSF fluid from the brain. I do hope that you can get relief.

Central nervous system Alternative names Return to top Idiopathic intracranial hypertension; Benign intracranial hypertension Definition Return to top A benign process affecting the brain which appears to be, but is not a tumor. It is characterized by increased intracranial pressure and normal brain ventricle size. There is no evidence of tumor, infection, blocked drainage of the fluid surrounding the brain or any other cause. Causes, incidence, and risk factors Return to top The major symptoms of pseudotumor are increased pressure within the skull ( increased intracranial pressure ICP ). The cause for the condition itself is unknown, and the diagnosis is made when other health conditions are ruled out. The mechanism causing the elevated ICP is not well understood. Possible causes of pseudotumor cerebri include a defect in Cerebral Spinal Fluid (CSF) absorption, increased cerebral swelling ( edema ) or increased cerebral blood volumes.
