FIN-20520 Turku Finland Tel.: +358 2 251 9255 Fax: +358 2 251 9254 E-mail: kaisa.granfors@utu.fi Project Description: The goal of the project is to obtain information about host-microbe interactions in vivo and in vitro in order to understand the mechanisms determining the development of reactive arthritis in some HLA-B27 positive persons, eg. after Salmonella and Yersinia infections. There is evidence that elimination of causative bacteria is not effective in HLA-B27 positive persons developing reactive arthritis, but these bacteria/bacterial antigens persist in the patients for long times, even for years. Elimination of these bacteria is impaired also in vitro, in HLA-B27-transfected monocytic cells and in monocytes/macrophages from HLA-B27 positive persons with reactive arthritis in the past. Both in vivo and in vitro , activation of HLA-B27 positive cells is different compared to HLA-B27 negative cells when infected with causative bacteria or stimulated with bacterial antigens. Knowledge about the exact mechanisms leading to impaired elimination of causative bacteria can help to find effective treatments and prevention of these diseases in HLA-B27 positive persons.

What is reactive arthritis?
Reactive arthritis, also known as Reiter's syndrome, is a type of arthritis that occurs as a reaction to an infection somewhere in the body. Most infections that cause the disease originate in the genitourinary tract (the bladder, urethra, penis, or vagina) and are spread through sexual intercourse. Other infections that can cause reactive arthritis include gastrointestinal infections due to eating contaminated food or handling contaminated substances.
Who is affected by reactive arthritis?
Reactive arthritis is characterized by inflamed joints and affects mostly young men, between the ages of 20 and 40. Although researchers are not sure why some people develop reactive arthritis in response to certain infections, a genetic factor (presence of the HLA-B27 gene) seems to increase the risk.

Reactive Arthritis
Reactive Arthritis is one of more than one hundred different types of arthritis and mostly affects young men 16-35 years old. It is one of the most common forms of arthritis affecting young men, but it may also affect young women and children. It is called Reactive Arthritis because an infection in one part of the body causes a reaction in another part of the body without the infection being found there. Reactive Arthritis causes inflammation (redness, swelling, pain) in different parts of the body at the same time or in different parts of the body at different times. Arthritis is the most important symptom in a chain of events which is the attack of reactive arthritis. In a person at risk the attack is usually triggered by an infection. (A blood test may identify a protein on your white blood cells that indicates that you are a person at risk. This protein is called HLA B27. Up to 25% of patients with Reactive Arthritis do not have HLA B27). Often it is a bowel infection causing loose, frequent bowel motions (diarrhoea) or it may be an infection transmitted during sexual intercourse. Occasionally there does not seem to be a triggering infection. In sexually acquired reactive arthritis the first symptom noticed is usually a pain when passing urine or discharge of pus from the inside of the penis (urethritis). In women a discharge from the neck of the womb (cervix) may not be noticed. More...
