Chronic Pelvic Pain Syndrome (CPPS) is a noninfectious inflammation of the lower urinary tract in men associated with symptoms of pain and urinary frequency and urgency. A research study of an investigational drug for the treatment of CPPS is underway and male volunteers are needed to participate. If you have some or all of the above symptoms and are 18-50 years old, you may qualify to participate and receive study medication and study related medical care at no cost to you.     About Reiter's syndrome
    What is it?
    Reiter's syndrome is a form of arthritis that produces pain, swelling, redness, and heat in the joints. It is one of a family of arthritic disorders affecting the spine. Reiter's commonly involves the joints of the spine and the sacroiliac joints, areas where the spine attaches to the pelvis. In 1916 Hans Reiter, a German military physician, described the disease in a World War I soldier who had recovered from a bout of diarrhea. Dr. Reiter described three characteristic features of the disease: inflammation of the joints , urinary tract, and eyes. More recently, doctors have recognized a fourth major feature: ulcerations of the skin and mouth.
    Incidence and risk factors
    Reiter's syndrome primarily affects young white men between the ages of 20 and 40, although it can occur in older people or children. African American men rarely get the disease, and women get it only a fifth as often as men.

A patient information handout on reactive arthritis, written by the authors of this article, is provided on page 507. Reactive arthritis, also called Reiter's syndrome, is the most common type of inflammatory polyarthritis in young men. It is sometimes the first manifestation of human immunodeficiency virus infection. An HLA-B27 genotype is a predisposing factor in over two thirds of patients with reactive arthritis. The syndrome most frequently follows genitourinary infection with Chlamydia trachomatis , but other organisms have also been implicated. Treatment with doxycycline or its analogs sometimes shortens the course or aborts the onset of the arthritis. Reactive arthritis may also follow enteric infections with some strains of Salmonella or Shigella, but use of antibiotics in these patients has not been shown to be effective. Reactive arthritis should always be considered in young men who present with polyarthritis. Symptoms may persist for long periods and may, in some cases, cause long-term disability. Initial treatment consists of high doses of potent nonsteroidal anti-inflammatory drugs. Patients with large-joint involvement may also benefit from intra-articular corticosteroid injection. Here is an overview of the five major medical problems that some Melungeon descendants inherit. MEDITERRANEAN DISEASES WHICH MAY AFFECT MELUNGEON DESCENDANTS SYNDROME is a relapsing, multi-system inflammatory disease in which there are oral/genital ulcers. There may be inflammation of the eyes, joints, blood vessels, central nervous system and gastrointestinal tract involvement. Attacks last about a week to a month and reoccur spontaneously. Onset is usually between 20-30 years of age with symptoms ocurring up to several years after the onset. Twice as many men as women are affected. There is a genetic predisposition with autoimmune mechanism and viral infection which may all play a part. Related disorders are Reiters Syndrome, Stevens Johnson Syndrome, and Ulcerative Colitis.

Dear Dr. Holodniy: Thank you for your response in advance. I'm now 24 weeks past exposure and negative at the 19wk mark by home access. I will take a final test this month. I had symptoms that could have been related to acute seroconversion at the 10 week mark. I had the typical GI symptoms, sore throat and sore glands. At this point I am having arthritic symptoms pain in arms, hands and pain in the shoulder area which feels like a burning muscular pain(both shoulders). I also had a urethral syndrome after my exposure and took cipro and zithromax for treatment. Do you think these symptoms could be related to HIV or possible some other condition such as reiters syndrome? Any suggestions as to appropriate follow-up at this point would be Greatly Appreciated.
    Given the negative HIV tests to date, this is not related to HIV infection. Given the urethral symptoms and other symptoms you describe, it could be Reiters syndrome, or related to some other sexually transmitted infections. You may want to ask your doctor about doing some other blood tests to rule these things out. MH
