    There are many birth defects that involve the genitals and urinary tract. These defects can affect the kidneys (organs that filter wastes from the blood and form urine), ureters (tubes leading from the kidneys to the bladder), bladder (sac that holds urine), urethra (the tube that drains urine out of the body from the bladder), and the male and female genitals. For boys, the genitals include the penis, prostate gland and testes. For girls, the genitals include the vagina, uterus, fallopian tubes and ovaries.
    Abnormalities of the genitals and urinary tract are among the most common birth defects, affecting as many as 1 in 10 babies. Some of these abnormalities are minor problems that may cause no symptoms (such as having two ureters leading from one kidney to the bladder), and go undiagnosed unless the child has an X-ray, ultrasound examination or surgery for a related or unrelated problem. Other abnormalities can cause problems such as urinary tract infections, blockages, pain, and kidney damage or failure.

    Etiology
    Renal agenesis is usually an isolated sporadic abnormality but, in a few cases, it may be secondary to a chromosomal abnormality or part of a genetic syndrome (such as Fraser syndrome), or a developmental defect (such as VACTERL association). In non-syndromic cases, the risk of recurrence is approximately 3%. However, in about 15% of cases, one of the parents has unilateral renal agenesis and in these families the risk of recurrence is increased.
    Diagnosis
    Prognosis

    Oligohydramnios means reduced amniotic fluid and anhydramnios means absence of amniotic fluid.
    Prevalence
    Oligohydramnios in the second trimester is found in about 1 per 500 pregnancies.
    Etiology
    Oligohydramnios in the second trimester is usually the result of preterm premature rupture of the membranes, uteroplacental insufficiency and urinary tract malformations (bilateral renal agenesis, multicystic or polycystic kidneys, or urethral obstruction).

    Clinical History
    Female, 12 year old.
    She complained of an abdominal pain in concomitance with the menstrual cycle; the symptomatology remitted at the end of every cycle.
    She occasionally had dysuria. A lower abdominal palpable and painful mass was found at physical examination.
    A compression by an extrinsecal mass on the right wall of the upper vagina was observed at the vaginalscopy.
    She underwent US examination which showed an oval, liquid mass, with low-level echoes in the pelvis and the emptiness of the right renal fossa.
    MRI examination was then performed.

  • Axial FSE T2W image (TR= 2500, TE=110, NEX=3, ETL= 11 )
  • Sagittal FSE T2W image (TR= 2500, TE= 110, NEX=3, ETL= 11)
    Coronal T1W image
    shows the characteristically hyperintense hematometrocolpos (*), the agenesis of the right kidney and the vicar hypertrophy of the controlateral kidney (K).
    Axial FSE T2W image
    confirms the presence of a right hematometrocolpos (*) and shows a thin, linear, high signal structure on the left, which indicates the presence of another uterus with its endometrium (arrow). Two separate cervices, joining together at the level of the vagina, were found on the caudal scans.

    Description: Potters Syndrome is the term used to describe the total absence or malformation of infant kidneys in combination with other congenital abnormalities, including Potter's facies, pulmonary hypoplasia, oligohydramnios, failure of foetal urinary production and skeletomuscular anomalies such as clubbing of the hands and feet and contractures. Oligohydramnios (deficiency of the amount of amniotic fluid) is the cause of failure of lung development and of limb compression in the foetus. Males are more commonly affected than females. The affected children are usually stillborn or die shortly after birth. Authors disagree whether inheritance is autosomal dominant recessive. Bibliography:

    Ultrasound examination shows anhydramnios. Scans of the renal fossae ( arrows ) fail to demonstrate the kidneys and bladder.
    Diagnosis
    Absent amniotic fluid due to renal agenesis
    Discussion
    Profound oligohydramnios may result from fetal urinary tract anomalies that include bilateral renal agenesis, posterior urethral valves, and bilateral multicystic dysplastic kidneys. Nonrenal etiologies include intrauterine growth retardation, post-term pregnancy, fetal demise, and ruptured membranes. Due to the lack of amniotic fluid, the lungs are hypoplastic and neonates who survive delivery die of respiratory failure. Severe and prolonged oligohydramnios leads to a constellation of findings that include Potter's facies (flattened nose, recessed chin, and low-set ears) and positional abnormalities (clubbed feet, dislocated hips, and contractures). Bilateral renal agenesis is a result of absence or failure of development of the ureteric bud and occurs in approximately 1 in 4,000 births. The inability to produce urine results in severe oligohydromnios and nonvisualization of the bladder. Absence of the kidney may be difficult to diagnose due to severe oligohydramnios. A normal bladder excludes the diagnosis. Bilateral renal agenesis is incompatible with life.

    Fraser Syndrome is a rare autosomal recessive disorder characterized by partial webbing of the fingers and toes (partial syndactyly ), renal abnormalities, genital malformations and/or, in some cases, complete fusion of the eyelids ( cryptophthalmos ). A preterm child with Fraser syndrome presenting with unusual abnormalities is reported.
