WINSTON-SALEM, N.C. - In the past 10 years, physicians have been seeing an increase in the number of infants diagnosed with vitamin D deficiency rickets, a disease once considered to be virtually nonexistent, according to an article in the August edition of the Journal of Pediatrics. African-Americans are more susceptible to the disease because dark skin inhibits the absorption of sunlight, which is needed to make Vitamin D. "We have seen a 4.4 increase in the number of African-American babies with the disease and a three-fold increase in all babies," said Robert P. Schwartz, M.D., pediatric endocrinologist and an investigator in the study. Rickets is the softening and weakening of the bones due to the body's inability to absorb calcium, usually because of a vitamin D deficiency. A healthy person gets vitamin D from two sources: food and sunlight.

Vitamin D is a fat-soluble vitamin that may be absorbed from the intestines or may be produced by the skin when the skin is exposed to ultraviolet light (particularly sunlight). It is converted to its active form by the body in 2 steps, occurring first in the liver and completed in the kidneys. In its active form, Vitamin D acts as a hormone to regulate calcium absorption from the intestine and to regulate levels of calcium and phosphate in the bones. Active Vitamin D is assisted by the actions of other body hormones.
    Because Vitamin D is fat soluble, conditions that reduce digestion or absorption of fats will decrease the ability of Vitamin D to be absorbed from the intestines. Sunlight is important to skin production of Vitamin D, and environmental conditions where sunlight exposure is limited may reduce this source of Vitamin D.

    Child with rickets
    Vitamins are groups of substances which are essential for the body to work properly. Vitamins cannot usually be made by the body, but are found in certain foods. Vitamin D is fat soluble, and is unusual because the body can make it.
    Vitamin D is found in butter, egg yolk, oily fish like mackerel, and red and green vegetables. Vitamin D is made by the body from the action of sunlight on the skin.
    Calcium is found in milk, cheese, green vegetables and hard water. Nowadays calcium is put into bread and yoghurt.
    Victorian children wore lots of layers of clothes and often lived in very polluted areas. This meant that their bodies were not able to make Vitamin D using the sunlight. It was mainly poor children with a bad diet who had rickets, and because they were weak these children were also more likely to catch diseases like tuberculosis . Rickets is very uncommon in Great Britain in the 21st century.

    It is a disease of weak bones caused by low amount of vitamin D in the body. In the Northwest, rickets is usually due to too little sunlight on a baby's skin.
    WHO IS AT RISK?
    All babies who receive only breastmilk for 6 months or more, all breastfed babies of color, premature babies, and infants weaned to vegan diets.
    WHAT ARE THE SIGNS OF RICKETS?

    HOW CAN YOU PREVENT RICKETS IN YOUR BABY?

It is a condition commonly seen in children adopted abroad. There can be several causes of rickets but in adopted children the cause is commonly due to nutritional deficiencies. Poor diets lack calcium and vitamin D, both of which are necessary for proper bone growth. S ymptoms of rickets can vary from no obvious symptoms to the more obvious: Teeth can be affected with delayed eruption of baby teeth along with enamel defects, cavities and some affects of the permanent teeth
    Muscle tone can be decreased and muscles poorly developed contributing to the delayed motor development.
    The head can be box shaped with frontal bossing. The fontanel or soft spot can have delayed closure up to several years.

    Q: Rickets... Is it still possible for people to suffer rickets? David Dr Rob Hicks responds Yes, this is still possible. When there's a deficiency of vitamin D the bones become weak and soft. This means they may develop an abnormal shape and are certainly at risk of breaking easily. In children vitamin D deficiency is called 'rickets', and in adults it's known as 'osteomalacia'. Bones need calcium to make them strong. However, to absorb calcium from the diet (dairy foods, fortified bread and cereals, for example) into the body vitamin D is needed too. Some of the vitamin D that we need to do this comes from our diet. Few foods actually contain vitamin D but those such as fish, eggs, and milk are quite a good source. The body also manufactures vitamin D in the skin when it is exposed to sunlight. The effects of vitamin D deficiency on the bones mean that children may not grow to their full potential and they may have knock-knees or bowed legs. Adults may suffer pain in the bones and find that they have difficulty climbing stairs or getting up from a squatting position.

    Not only Karl May's eyesight was affected in his early childhood. His general health suffered as well: "... [I was] for full four years in bad health " . After treatment in Dresden he "returned, also for the rest recovered , home." May quite clearly states that his general health suffered apart from the eyesight. The only other remark about his childhood condition Karl May made in the following passage : "I was born as a sick, weak child, who even at the age of six years crawled on the ground, without being able to stand or even run ... I was blind three times ... ". This of course is a literary interpretation that nevertheless was based on Karl May's personal experience. There are no more clues left by Karl May, except perhaps in a book found in his library: "Der Saugling, Seine Ernaehrung in gesunden und Kranken Tagen", (The Toddler and its Nourishment in Health and Sickness) published in Hamburg in 1888 . It is quite understandable that Karl May might have tried to learn about his childhood illness from a contemporary text. We know that he did attempt to find out more about his mental state he was in when young from a textbook of psychiatry, also discovered in his library . Some leads to the childhood sickness of Karl May have been left by people who observed him. We also have photographs of Karl May that we can study. When May visited Komotau (the town of Chomutov presently in the Czech Republic) on the 13 July 1897, there is a record of his appearance at Hotel Scherber : " [The married couple entered the hall] ... She was an impressive womanly figure, as one would imagine a Wagner's Valkyrie, however the look of Karl May disappointed us. We imagined a hero's stature, instead of that in came a hardly middle sized man, with somewhat bow legs (from to much horse riding, as Fischer explained) and with upstanding, mostly uncombed hair, a gold pince-nez on his nose..."
