IMMUNIZATION INFORMATION - RUBELLA (GERMAN MEASLES)
OVERVIEW The Disease
Rubella is also called German measles or 3-day measles. When children get it, it is usually a mild disease. Rubella also strikes adults, and outbreaks can occur among teenagers and young adults who have not been immunized. Rubella usually occurs in the winter and spring and spreads very easily. People catch it through contact with other people who are infected. It is spread through coughing, sneezing, or talking. Rubella Immunization
Rubella vaccine can be given by itself, but it is usually given together with measles and mumps vaccines in a shot called MMR. You can read about MMR vaccine on page . State Immunization Requirements
All 50 States have laws requiring school children to be immunized against rubella. Forty-nine states have laws requiring children entering day care to be immunized against rubella. MMR Vaccine

What is Rubella?
Rubella is a viral disease characterized by slight fever, rash and swollen glands. Most cases are mild. Who gets rubella?
In unvaccinated populations, rubella is primarily a childhood disease. Where children are well immunized, adolescent and adult infections become more evident. Rubella occurs more frequently in winter and spring. How is rubella spread?
Rubella is spread by direct contact with nasal or throat secretions of infected individuals. What are the symptoms of rubella?
Rubella is a mild illness which may present few or no symptoms. Symptoms may include a rash, slight fever, joint aches, headache, discomfort, runny nose and reddened eyes. The Iymph nodes just behind the ears and at the back of the neck may swell causing some soreness and/or pain. The rash, which may be itchy, first appears on the face and progresses from head to foot, lasting about three days. As many as half of all rubella cases occur without a rash. How soon do symptoms appear?

Rubella virus is spread by contact with discharges from nose or throat The virus is usually shed in the discharges from about 7 days before the rash until at least 4 days after the rash begins. Rubella virus is passed to the next person by direct contact with the secretions. Babies with rubella acquired at or before birth can continue to shed virus in nose and throat secretions and in urine for 1 year or more. Anyone can get rubella except those who have had rubella or rubella vaccine. Symptoms to look for, Low fever, Rash (lasts 3 days or less), Joint aches, Swollen glands, especially behind the ears and the back of the neck, Symptoms start within 16-18 days after exposure, with a range of 14-23 days. Up to half of all people who get rubella don't have any symptoms.
Unborn babies are at high risk of rubella complications, Most children and adults fully recover from rubella with few complications. However, rubella infection in a pregnant woman during the first 3 months of pregnancy can result in miscarriage, fetal death, or a baby with deafness, eye, heart, liver or skin problems, or mental retardation. By Janine Roberts - Government scientist Dr Elizabeth Miller recommended the Measles and Rubella campaign (MR) in November 1994 when 8 million English children were jabbed campaign as helping "the interruption of rubella transmission in the United Kingdom. The Chief Medical Officer on 27th September 1994 advised doctors and nurses not to pass on the manufacturer's warning that it was dangerous to be pregnant near the time of vaccination to the 1 million teenage girls being vaccinated. He stated: "We do not believe there is justification for counseling all girls to avoid pregnancy for one month following immunisation'. He also recommended that girls not be asked when was their last period and that information on the risk of vaccination to pregnancy be only to be made available to girls that asked SmithKline Beecham, gives the following warning in bold print on the leaflet it supplied last year with the vaccine to the Department of Health: "Never give to pregnant women, or women of child-bearing age not fully aware of the need to avoid pregnancy for one month after vaccination." This is the only warning so stressed by the company. A spokesman for the company said that they were aware the Department had deleted the warning, but "we are keeping the warning in place. We prefer in such cases to deal directly with the physician and make a best judgment on a case by case basis." Dawn Corrigan is the mother of a 1 year old son, George, who came down with ITP last December. She reports: "On Christmas Day, 10 days after he had the MMR, I found his nappy full of blood. His nose then started bleeding. The roof of his mouth was dark mauve. His skin was covered in spots. It was really frightening. He was 2 weeks in hospital and still is not fully recovered."

Rubella (German measles) is a mild childhood illness that poses a serious threat to the fetus (unborn baby), if the mother contracts the illness during pregnancy. More than 20,000 babies were born with defects during an outbreak of rubella in 1964-65. The same outbreak also resulted in at least 10,000 miscarriages and stillbirths.
Fortunately, major outbreaks of rubella no longer occur in the United States. Since 1969, when a vaccine for rubella became available, children have been routinely vaccinated, helping to prevent the spread of the illness to susceptible pregnant women. Most women of childbearing age are immune to rubella because they either were vaccinated or had the illness during childhood. Because of widespread use of the vaccine, birth defects caused by rubella have become rare.
However, since small outbreaks of rubella continue to occur, the potential for susceptible pregnant women to become infected continues to exist. As many as 2 in 10 women of childbearing age are susceptible to rubella. Women can protect their future children from the effects of rubella by getting tested for immunity prior to pregnancy and being vaccinated if they are not immune.

Rubella What you need to know:
Rubella (German measles) is a mild childhood illness that can cause serious birth defects in an unborn baby. About 25 percent of babies whose mothers get rubella in the first trimester of pregnancy are born with one or more birth defects (congenital rubella syndrome) including eye defects, hearing loss, heart defects, mental retardation and, less frequently, movement disorders.
With the widespread use of the rubella vaccine, major outbreaks of rubella no longer occur in the United States. Still, small outbreaks do occur. As many as 20 percent of childbearing women are susceptible to rubella infection.
What you can do:
There is a simple blood test that can determine if you are immune to rubella. The March of Dimes recommends that all women be tested for immunity to rubella before becoming pregnant. If you are not immune, consider being vaccinated at least 28 days before trying to get pregnant. Pregnant women are routinely tested for rubella immunity at an early prenatal visit. If a pregnant woman finds out she is not immune, she should not be vaccinated during pregnancy. All she can do is try to avoid exposure to anyone with the illness. She can also get vaccinated soon after delivery so she is immune during any future pregnancies.

DESCRIPTION: Maternal rubella (German measles) in the first trimester of pregnancy is generally responsible for a triad of defects in the fetus: heart defects, hearing problems and eye problems; mental retardation also often accompanies these defects. Ocular involvement is typically cataracts (bilateral in 75% of the cases) but also may include uveal colobomas, searching nystagmus, microphthalmus, strabismus, retinopathy, and infantile glaucoma. TREATMENT: Cataract surgery is usually delayed until at least age 2, since the live rubella virus remains in ocular tissues for many months after birth. Unfortunately, this preferred delay also results in a poor prognosis for visual functioning following cataract removal. Monitoring of ocular status (for complications) is recommended. Educational adjustments will vary, according to functional vision. Optical aids and/or illumination levels should be according to individual needs. Glare may be a factor. IMPLICATIONS: Some physicians are recommending therapeutic abortion in cases of maternal rubella, since the probability of serious congenital anomalies is so high.

Many are unclear about the facts concerning the vaccine and want to have accurate, scientific information. These pages aim to provide that information. Background MMR contains three separate vaccines in one injection. MMR vaccine protects children against measles, mumps and rubella (German measles). It is given to children at 13 months of age and again before they go to school. The second dose protects anybody who did not respond to the first dose. Since 1988 when MMR was introduced in the UK the number of children catching these diseases has fallen to an all-time low. Children who have MMR vaccine do sometimes experience side effects. These are most usually mild forms of the diseases the vaccine protects against. Very rarely, children can have severe allergic reactions straight after any immunisation (about 1 in 100,000 immunisations for MMR). If the child is treated quickly, he or she will recover fully. People giving immunisations are trained to deal with allergic reactions.
