Runner's Knee
(Chondromalacia Patella)
Runners Knee refers to pain "primarily" on the inside of the knee, although pain on both sides or beneath the kneecap can also occur. It is more commonly seen in runners as opposed to walkers due to high level of activity, particularly mileage. The key element in this injury is due to overpronation causing a repeated stress of the patella bone moving abnormally from side to side over the groove of the thigh bone (femur), gradually softening the cartilage under the kneecap. The result is the surface of the joint can become rough. Pain may be just a prelude to further destruction of the surface (traumatic arthritis). This injury can be confused with "Patella tendonitis" which is usually pain beneath the patella bone. In either case it is the "tracking" of either the bone or tendon which needs to be addressed.
Other causes of this injury can be:
  • Weak quads Muscle imbalance Direct or repeated trauma An untreated ligament injury Running on a banked surface (similar to ITB Syndrome History of trauma

  • Decrease activity Rest if pain and swelling Ice (for 15-20 minutes) Aspirin or Ibuprofen Physiotherapy Silicone Dynamic Orthotics to correct musculo-skeletal alignment, by correcting over- pronation.

    Date: Tuesday, 27 August 2002, at 12:33 p.m. In Response To: UH-Oh (Shawn Gray) Shawn, You can actually get an idea of the condition of your ACL by performing the drawer test on yourself. Only your OS can determine for real, but this will give you an idea. Set in a chair with your feet on the ground, relax both legs. Simply push out on your tib/fib just under your knee on your good leg and see how little it moves forward. Then do the same thing on your gimp leg. You have to really relax both legs to do this and it's not as accurate when your legs are bent at 90 degrees. I showed this to my OS and he thought that it was neat that I could do it myself. He also explained that the movement I got was not near what he got when my leg was at the proper angle. Of course you can't to that yourself. When your leg is bent the pattella tendon keeps the tibia held back. But it's a great party trick! For me the good leg (rebuilt, 99) move about 3-5 millimeters and the newly torn one moves out 10-13 mm. 13mm is a half an inch.

    "Runners knee":
    This injury usually builds up over several weeks of running. The major subjective complaints are pain at the front of the knee (anterior knee pain) during running, stair walking and in particular during prolonged sitting with the knee at a 90 degree ankle. There is often pain with sideway movement of the kneecap and occasionally swelling under the kneecap. There are often stiffness of the fibres of the lateral vastus muscle (the muscle at the outside of the knee) as well as biomechanical faults, such as increased ankle pronation, changes of the angle of the knee, increased swayback and general poor muscle control of the lower limbs. The treatment consists of pain relieving techniques release of the tight tissue and retraining of muscles. Foot orthotics may also be used.

    Chondromalacia
    What Is Chondromalacia?
    Chondromalacia (pronounced KON-DRO-MAH-LAY-SHE-AH), also called chondromalacia patellae, refers to softening of the articular cartilage of the kneecap. The disorder occurs most often in young adults and may be caused by trauma, overuse, parts out of alignment, or muscle weakness. Instead of gliding smoothly across the lower end of the thigh bone, the kneecap rubs against it, thereby roughening the cartilage underneath the kneecap. The damage may range from a slight abnormality of the surface of the cartilage to a surface that has been worn away completely to the bone. Traumatic chondromalacia occurs when a blow to the knee cap tears off either a small piece of articular cartilage or a large fragment containing a piece of bone (osteochondral fracture).

    The anterior cruciate ligament of the knee is one of four principal knee ligaments. However, it seems to be the most important ligament in terms of a sense of well-being and stability of the knee. When torn, it is now routinely rebuilt utilizing a portion of the patellar tendon or hamstrings tendons as a donor source. I utilize an arthroscopically aided approach in my cruciate reconstructions, and find that with proper postoperative rehabilitation, the prognosis is quite good for returning to competitive sports.

    Prevention of Runners Foot and Knee Problems
    By Jeff Gundel, MD As a runner, your feet and legs take a lot of abuse due to the repetitive stresses. I will review some of the more common injuries affecting runners and suggest some ways to prevent them. If you have already sustained one of these injuries, I will review treatment and rehabilitation methods. In conclusion, most overuse injuries affecting the runner can be prevented by wearing a well-fitted shoe, stretching routinely, and gradually increasing distance and hills. Attempting to do too much too soon leads to problems. Despite this advice, injuries do occur and most can be treated easily with rest, ice, anti-inflammatories, stretching, and gradual return to running. Appropriate and timely treatment will return the runner to many enjoyable miles.
