The main symptoms of runner's knee (chondromalacia) are aching around the knee cap. This ache may be worse on climbing stairs or cycling. Runners may experience knee pain after running a certain distance when their knee seizes up and forces them to stop. Cause
Chondromalacia is caused by disproportionate development of some muscles around the knee. These muscles then pull the knee cap in the wrong direction. The back of the knee cap then starts rubbing against other parts of the knee joint and ache develops. Some patients have a predisposition to knee pain because the knee cap is small or anatomically misplaced. Treatment
This condition often gets better on its own in time, but can persist for years. You can speed up the recovery process if you follow this advice. Modify your exercise to e.g. cycling in low gears, gentle jogging, swimming etc. Do the following; Knee exercises:
  • Sitting on a bed, keep your leg flat and bend your toes towards your head as much as possible, keep this position for ten seconds and release. Repeat this ten times.

    “Runner’s Knee”
    At time trials and races you will often hear fellow competitors talking about the somewhat nebulous concept of “runner’s knee”. This term is loosely and often incorrectly used. It should be used in referring to pain around the patella (knee cap) that arises from incorrect movement of the patella over the top of the knee in runners. It is not another term for ITB friction syndrome and has many other labels in sports medicine such as peri-patella pain syndrome, patello-femoral pain, anterior knee pain and patella malalignment syndrome. Simplistically speaking the patella should glide smoothly in an underlying groove as the knee bends and straightens during the running action. Any factor that causes the patella to be out of alignment may cause pain around the knee cap particularly when the knee is bent. These factors may arise from the structure or position of the knee cap itself, from the muscles just above the knee, from the runner’s gait (i.e. the way the feet and legs move during running) or the alignment of the upper legs, amongst other influences. Runner’s knee has a number of distinguishing features. It often affects both knees, occurs once mileage is increased, may occur during other activities such as sitting for prolonged periods or walking up and down stairs and may be associated with crepitus or a scraping sensation behind the knee cap when the knee is straightened against resistance. The reasons for pain are not entirely understood but may be caused by a build up of pressure within the knee joint when the knee is bent, wear-and-tear of the cartilage lining the back of the patella (you may have heard it referred to as “chondromalacia”) and abnormal tension on tendons around the knee causing inflammation.

    Knee pain
    Knee pain is often caused by an increase, or simply a change, in pressure under the kneecap (patella) bone. This may happen even without a specific injury, although it may happen after you change your activity level. The patella is a "V" shaped bone that sits in a groove (trochlea) at the end of the thighbone (femur). The kneecap (patella) bone is part of the tendon of the quadriceps muscle (thigh). Normally, the patella stays within the groove in the thighbone as we flex and extend our knees, because of a balance among muscle strength of the leg, ligament-like restraints called (retinaculum), and flexibility of our joint.
    Weak quadriceps (front of thigh) muscles, after knee swelling, injury, or simply because of lack of use, can result in poor tracking of the kneecap. Poor tracking also occurs in people with malalignment of the leg. This poor tracking can cause pressure under the kneecap to be unevenly distributed on the cartilage of the kneecap. In addition, the ligament-like tissue (retinaculum) on the inner part of the knee often stretches, while the retinaculum on the outer part of the knee shortens simply as a result of our knee anatomy and daily use.
    Pain is often worse when you bend your knee or when the quadriceps muscles are active, because each action places more force on the patella. This kneecap pain is not only associated with weakened, unbalanced muscles, but may also result from tight hamstring (back of the thigh) and/or calf muscles, as well as flat feet.

Go figure, but there seems to be a fixation with the idea that we are literally running our knees into the ground with every step we take. Turns out that knee injuries are not inevitable and, though common, are easily corrected if properly managed. Knee pain need not deter you from your ideal running program.
    Runner's Knee
    (Chondromalacia of the patella) Description: Pain around and sometimes behind the kneecap. One of the most common injuries among runners, runner's knee most often strikes as runners approach forty miles per week for the first time. Even after taking a few days off, the pain seems to come right back, sometimes even intensifying, after the first few miles of the next run. The pain often feels worst when running downhill or walking down stairs, and the knee is often stiff and sore after sitting down for long periods. You might hear a crunching or clicking sound when you bend or extend your knee.

    The picture to the right shows that when the foot rolls in (pronates) the lower leg muscles become strained. The muscles that act to lift the arch become strained when the foot collapses or rolls in. This is common in distance runners, or those doing aerobics or step classes. Muscles become inflammed as do tendons ("tendonitis") and the covering of the bone as well ("periostitis"). Proper orthotics can help greatly. Also consider ice and stretching exercises for possible tight calf muscles which would make the front muscles work harder. Strengthening exercises may be required for the front of the lower legs in those runners where the calf muscles overpower the front muscles. Well cushioned shoes reduce jarring during the working day. Runner's knee. This diagram shows a neutral or proper foot position and knee alignment in the left example, but in the right example it shows a pronated foot and internal rotation of the lower leg, causing abnormal kneecapm tracking. Knee pain due to pronated feet. When the foot pronates, the lower leg rotates inward and this affects the knee. Generalized knee pain under the knee cap is common in runners and is known as "runners' knee" ("chondromalacia"), or more accurately known as "patello femoral syndrome" (See next illustration).     Technique drills for runners are usually performed using three activities, marching (walking), skipping and running. Each activity helps to develop important components of proper and economical running technique. There are several reasons for using the activities:
    • The exercises serve as an excellent warm-up tool for both training and competitive situations. After all, running drills imitate specific characteristics of technically sound running form, including upright posture of the trunk and spinal column, proper carriage of the arms during the running stride, proper knee drive and leg action and the co-ordinated dynamic balance associated with shifting weight from one leg to the other The drills help to develop the important proprioceptive and kinaesthetic (body awareness) abilities that a runner must have to deal with changes in terrain while running, and also to ''change gears" while training or racing. The drills are performed slowly at first while developing a "feel" for proper technique, and proper technique is then gradually carried over to drills and runs carried out at faster speeds The exercises also help to strengthen specific muscle groups needed for powerful running, especially the muscles of the feet, calves, shins. thighs and hips. The ankle, knee and hip joints undergo considerable flexion and extension during the running stride, and each of these joints is exercised through a similar or greater range of motion during the various drills

  • To reduce the inflammation
    This is done through rest, ice and anti-imflammatory medication (always check with a doctor or pharmacist before taking any drugs - you should not take ibruprofen if you have asthma). A heat retainer can also be helpful.
  • To stretch out the Illiotibial band. Place the leg you want to stretch behind the other one. Put your weight onto your back leg, hold onto something and lean away as shown. Hold for 30 seconeds, repeat 5 times and do this at least three times a day. It is a lot of stretching but it is worth it in the end if you want to be rid of this injury.
