    Reviewed by Dr. Clement J. Cheng Back pain is one of the most frustrating chronic diseases. Degenerative disk disease is a back disorder that occurs as part of the aging process. It is usually caused by osteoarthritis of the spine, which irritates the vertebrae of the spine with excessive use. When the vertebrae become inflamed due to overuse, they work to heal themselves and leave calcium deposits and bone spurs as side effects. The pain and stiffness that result from this imperfect healing process are known as degenerative disk disease.
    Detailed Information
    Degenerative disk disease is a general classification including herniated or ruptured disk, prolapsed intervertebral disk, and others.
    The disks in the spine are like small sponges that rest between each vertebrae and act as shock absorbers. When arthritis compresses a disk, it tends to bulge out of its alignment; sometimes, it will rupture. When it does, the rupture will usually occur posteriorly. The displaced disk segment may then press on the spinal nerve as it emerges from the spinal cord, or on the cord itself.

    Definition: A condition in which part or all of the soft, gelatinous central portion of an intervertebral disk (the nucleus pulposus) is forced through a weakened part of the disk, resulting in back pain and leg pain caused by nerve root irritation.

    Causes, incidence, and risk factors: The bones of the spinal column, or vertebrae, run down the back connecting the skull to the pelvis. These bones protect nerves as they exit the brain and travel down the back and then to the entire body. The spinal column is divided into several segments the cervical spine (the neck), the thoracic spine (the part of the back behind the chest), the lumbar spine (lower back), and sacral spine (the part connected to the pelvis that does not move). The spinal vertebrae are separated by cartilage disks filled with a gelatinous substance, that provide cushioning to the spinal column. These disks may herniate (move out of place) or rupture from trauma or strain , especially if degenerative changes have occurred in the disk.

      Separating any two vertebral bodies is a soft elastic material called a disk. The disk is composed of two parts, a soft center called the nucleus and a tough outer band called the annulus. Now she has become pregnant, and we need to know if this pregnancy will make her back pain worse. We need to know soon so that we can terminate the pregnancy if this back problem and the pregnancy together will make her disabled. Answer: Let me review for you and my other readers how a ruptured disk (also known as a slipped disk, herniated disk, or herniated nucleus pulposus) can cause back pain. As I suspect you know, the nerves of the spinal cord are surrounded and protected by a series of interlocking bones called the vertebrae. Each of these vertebra, in addition to having an opening for the passage of the spinal nerves, has disks that both support and cushion it in relation to the vertebrae above and below. The disk is constructed of a tough outer layer and a softer gelatinous inner filling called the nucleus pulposus. This is similar to the construction of a jelly-filled doughnut. Mechanical stress on a disk can cause a tear in the disk's outer layers. As a result, the nucleus pulposus bulges through the tear in a manner similar to what happens when you bite into a doughnut and the jelly squirts out a gash on the other side. Because of the mechanical forces on a disk, the bulge always occurs toward the spinal canal. Thus the ruptured gelatinous material (and swollen adjacent tissue) press on the nerves at that level of the spine. This often causes back pain as well as pain in the areas served by the nerves. For instance, a herniated disk in the low back typically causes pain not only in the back but also in the buttocks and legs.

    OVERVIEW:
    The spinal column is composed of a series of 24 bones called vertebrae that are separated from each other by a disk. These disks help absorb the wear and tear. The disks are about the size of four silver dollars stacked on one another. Their structure is like a jelly donut, with tough surrounding ligaments and a jelly center called a nucleus. This nucleus will flatten out and bulge, depending on the stress applied to the back. If the force is too much, the nucleus can rupture and break through the surrounding ligament. When this happens, it will pinch a nerve either in the low back or neck.
    DIAGNOSIS:
    TREATMENT:

    Under no circumstances should more than three to four doses of narcotics be consumed in a 24-hour period, because of potential injury to the liver or kidneys. If physical therapy and medications are inadequate for control of pain, injections can be given to ease the discomfort. Epidural steroid injections apply powerful anti-inflammatory drugs to the involved nerve root easing pain and swelling secondary to disc irritation. The injections are usually given as a series of two to four over a period of weeks. Relief can begin within 7 hours of the first injection. Patients experiencing severe pain despite medications, physical therapy and injections may decide to have surgery. Surgery is advised only if you have a ruptured disk confirmed with the CT scan, MRI or myelogram, and have severe pain following the nerve into the foot or hand.

    Back pain is one of the mast common ailments in the United States. It will affect aver 50 percent of the population at some point in their lives. The reason is because man evolved from a four-legged creature. When he began walking upright the spine remained adapted for walking on all four limbs. This put significant stress on the low back and accounts for the widespread problem in our society today. The total cost of low back pain, including direct medical care, disability payments, and lost productivity, is in the tens of billions of dollars. In addition to walking upright, the complex structure of the back makes it vulnerable to injury.
    It is composed of a combination of nerves, muscles, ligaments, joints and bones. The base of this structure is the 24 vertebrae bones stacked on top of one another like bricks. They go from the very bottom of our spine to the underside of our head. The bones in the low back are called the lumbar vertebrae. In between every vertebra is a shock absorber or disk. The disk is the size of about three silver dollars stacked on one another. They provide cushioning during physical activity. Stringing the vertebrae and disks together is a network of ligaments.
    Overlying the ligaments are the muscles of the back that allow us to move. All these structures are pain sensitive, which is why back problems can be so difficult to treat. With so many structures capable of causing pain, it is sometimes difficult to determine where the symptoms are coming from. A ruptured disk can put pressure on the nerve. Arthritis can affect the joints. Inflammation, called fibrositis, can irritate the muscles and ligaments. The bones can suffer a compression fracture from osteoporosis or traumatic accidents. In addition, diseases unrelated to the back can cause back pain. These include a variety of different cancers, problems with the major blood vessels of the abdomen, pelvic disorders in women, and even gallbladder disease can refer pain into the back underneath the shoulder blade.

    The spinal column is made up of a row of small bones, the vertebrae, that run from just beneath the skull through the tail. The spinal cord runs through the middle of these bones for the length of the animal. This arrangement allows the spine of the neck, chest and back to be flexible while maintaining a rigid spinal column. The vertebrae are maintained in the proper position by the muscles and ligaments that run on each side of the spine, and bony projections of the vertebral bodies called facets. Material between the vertebral bones cushions them from each other; these are the intervertebral disks. The intervertebral disks are made up of a soft central nucleus surrounded by a tough outer capsule. Intervertebral disk disease develops when a degenerative change in the disk causes rupture of the internal portion of the disk through the tough outer capsule. Because the spinal cord and disks are surrounded by bone and muscle, pressure from the ruptured disk cannot be exerted outward and so compression of the spinal cord occurs. This compression causes the animal pain, and the development of neurological signs of disease such as knuckling of the feet, ataxia (wobbly gait), decreased pain sensation, and paralysis. Intervertebral disk disease is not known to be hereditary, but is significantly more common in long-backed breeds with short legs, such as the Dachshund, Shih Tzu, Pekingese, and Corgi. Obese animals are predisposed to intervertebral disk disease.

Very few people with ruptured disks require surgery.
