Editorial: Recent Developments in Atypical Antipsychotic Medications During the 1990s there have been some dramatic advances in the treatment of schizophrenia, and the publication of the Schizophrenia Patient Outcomes Research Team (PORT) study ( Schizophrenia Bulletin (Vol 24, No. 1, 1998)) is an opportunity to point out where we believe the "state of the science" is heading. Just as the serotonin re-uptake inhibitor (SRI) class of antidepressants has largely replaced the older and more problematic tricyclic antidepressants (based in large part on safety and side-effect considerations), a shift in the treatment of schizophrenia is now taking place. Some quite recent studies (since the PORT data were collected) indicate that newly available treatments for schizophrenia may offer substantial clinical advantages over the older standard antipsychotic (or "neuroleptic") medications.
Because these medications have been available for only a short time, there is relatively little scientific literature on the subject, but the previous issue of Schizophrenia Bulletin (Vol. 23, No. 4, 1997) contains several relevant articles. Sheitman et al. (1997) note that

This illness affects at least 1% of the population worldwide, regardless of race, economic condition, or geographic location. Families who have one member of the family ill with schizophrenia have a greater chance of developing this illness than families who have no relative with this illness. The scientists at the National Institute of Mental Health believe that environmental factors combined with genetic predisposition lead to the development of schizophrenia. The NIMH genetic studies focus on gathering families' genetic and environmental information to better understand what places one family member at risk and conversely, what protects other family members. To this end, family members are asked to participate in a number of procedures that help scientists measure various family characteristics to see if they run in families. Genotyping, or DNA analysis, is an important part of this process. However, families should understand that this is a very complex analysis and NIMH does not plan to provide family members with individual genetic test results. The scientists believe that future treatments and preventive methods will rely heavily on the findings of this developing study. It is for this reason that we ask families to participate. SEROQUEL is indicated for the treatment of schizophrenia. The most common adverse events associated with the use of SEROQUEL are dizziness (10%), postural hypotension (7%), dry mouth (7%), and dyspepsia (6%). The majority of adverse events are mild or moderate. The incidence of somnolence in clinical trials was 18% versus 11% for placebo. As with all antipsychotic medications, prescribing should be consistent with the need to minimize the risk of tardive dyskinesia, seizures, and orthostatic hypotension. Only dyspepsia, weight gain, and abdominal pain were reported at a significantly higher incidence with increasing doses of SEROQUEL. Patients should be periodically reassessed to determine the need for maintenance treatment.

I was the only person from my junior school to go to this comprehensive. This was the first stage in my isolation. While at the comprehensive I was never myself and acted the clown in class. On top of this, when I was 11 or 12, my dad found out that my mother was seeing another man who she had met on holiday in Spain. They did not divorce but the atmosphere at home became more and more strained over time. When I was 15 I went on a school trip to France and ended up going out with a girl called Karen. She was 13 and yet far more experienced than me. We had a great time but it did not last. On coming back to school I felt uncomfortable about going out with someone younger than myself.. A week after the French trip I went on a geography field trip in Wales. Almost by accident, I ended up going out with Catherine who I had liked for some time. On returning from the trip I ran into Karen who quite rightly gave me a hard time and made me feel really bad about myself. I carried on going with Catherine for a few weeks but things did not work out and I decided to call things off. I phoned up Karen and told her about it. I hoped we could get together but she did not want to know.
