Seborrheic dermatitis is a common inflammatory disease of the skin characterized by scaly lesions usually on the scalp, hairline, and face. Description Seborrheic dermatitis appears as red, inflamed skin covered by greasy or dry scales that may be white, yellowish, or gray. It can effect the scalp, eyebrows, forehead, face, folds around the nose and ears, the chest, armpits (axilla), and groin. Dandruff and cradle cap are mild forms of seborrheic dermatitis, and appear as fine white scales without inflammation. Causes and symptoms The cause of seborrheic dermatitis is unclear, though it is has been linked to genetic or environmental factors. Pityrosporum ovale , a species of yeast normally found in hair follicles, has been proposed as one possible causative factor. A high fat diet and alcohol ingestion are thought to play some role. Other possible risk factors include:
  • stress and fatigue
  • weather extremes (e. g. hot, humid weather or cold, dry weather)
  • oily skin
  • infrequent shampoos
  • obesity
  • Parkinson's disease
  • AIDS
  • use of drying lotions that contain alcohol
  • other skin disorders (for example acne rosacea , or psoriasis)
Mild forms of the disorder may be asymptomatic. Symptoms also disappear and reappear, and vary in intensity over time. When scaling is present, it may be accompanied by

Definition
Dermatitis is a general term used to describe inflammation of the skin. Description Most types of dermatitis are characterized by an itchy pink or red rash. Contact dermatitis is an allergic reaction to something that irritates the skin and is manifested by one or more lines of red, swollen, blistered skin that may itch or seep. It usually appears within 48 hours after touching or brushing against a substance to which the skin is sensitive. The condition is more common in adults than in children. Contact dermatitis can occur on any part of the body, but it usually affects the hands, feet, and groin. Contact dermatitis usually does not spread from one person to another, nor does it spread beyond the area exposed to the irritant unless affected skin comes into contact with another part of the body. However, in the case of some irritants, such as poison ivy, contact dermatitis can be passed to another person or to another part of the body. Stasis dermatitis is characterized by scaly, greasy looking skin on the lower legs and around the ankles. Stasis dermatitis is most apt to affect the inner side of the calf.

Seborrheic Dermatitis Seborrheic dermatitis is a relatively common skin condition that usually is easily recognized and differentiated from other eczematous conditions. Its characteristics are:
  • Reddened, somewhat swollen patches of skin; and, Yellowish-brown scales and crusts on affected areas of the skin, frequently on the scalp (a common cause of dandruff), and around the nose and mouth.
Seborrheic dermatitis is common in infancy and can occur again after puberty and in adulthood. Its cause is unknown. Unlike acne, which also has increased sebum production, seborrheic dermatitis is not a disease of the hair follicles and attendant sebaceous glands. Itching: Causes and Treatment
Itching is a sensation common to a number of skin disorders, including psoriasis. It is a common sensation not specifically associated with any particular skin disorder, and may sometimes seem entirely random—for example, the spontaneous “itchy spot” on the nose, cheek, etc. It is a frequent manifestation of systemic diseases such as chronic renal disease, biliary obstruction, cirrhosis, some types of thyroid disease, Hodgkin’s Lymphoma, and some cancers.
Itching is so common one would think it is well understood. To the contrary, itching is not well understood at all. There isn’t even a good definition of itching. The working definition of itching from a standard dermatology textbook is, “a sensation that causes a desire to scratch.”
The medical term for itching is pruritus. By using medical terminology we can get a step away from the popular conception of itching as a minor or even funny phenomenon. When pruritus is severe, it is anything but humorous. As anyone with severe pruritus can attest, the sensation closest to severe pruritus is severe pain. Pain and pruritus do share some neurophysiologic pathways.
Pruritus seems to occur in two phases:
  • An itch is identified with a local site A subsequent more diffuse area of “itchy skin” that can be stimulated to intense pruritus by a light touch—for example, by fabric brushing against the skin.

Seborrhoeic dermatitis is a common, harmless, scaling rash affecting the face, scalp and other areas. Dandruff (also called pityriasis capitis ) is seborrhoeic dermatitis of the scalp. Seborrhoeic dermatitis may also occur on the eyebrows, eye lid edges, ears, the skin near the nose and skin-folds of the armpits and groin. Sometimes seborrhoeic dermatitis produces round, scaling patches on the middle of the chest or on the back.

Seborrhoeic dermatitis on nasolabial fold Cause Seborrhoeic dermatitis is believed to be related to a proliferation of a normal skin inhabitant, a yeast called Pityrosporum ovale (also known as Malassezia ). Patients with seborrhoeic dermatitis appear to have a reduced resistance to the yeast. It is not contagious or related to diet, but it may be aggravated by illness, psychological stress, fatigue, change of season and reduced general health. While seborrheic is the common US spelling, in Europe an "o" may be added to make "seborrhoeic" dermatitis. Seborrheic eczema, seborrhea oleosa, and pityriasis capitis are other names for the same condition. Diagnosis of this condition is generally straightforward, but the differential diagnosis includes a variety of conditions, such as psoriasis vulgaris, atopic dermatitis, tinea capitis and candidiasis, as well as other, more rare conditions. The exact cause of seborrheic dermatitis is not known but there is probably a genetic component involved. The prevalence of seborrheic dermatitis changes in different locations and communities around the world. The most susceptible ethnic group are Caucasians and people of Celtic descent in particular. People with Parkinson's disease are also very susceptible to seborrheic dermatitis. People with Parkinson's disease produce very high levels of of sebum. This can be partly improved by patients taking L-dopa supplements.

Miss S S, aged 3 ½ years, resident of Baroda, came to my clinic in Bombay with her grandfather on July 8, 1987 with the following history: Since 1-½ years, she has had an eruption on the scalp with crust formation and scaling. On combing the hair, these fall out with white flakes in large quantities leaving behind raw areas. She has a poor growth of hair, which is, thin and light brown in color. (See photo 1). Her skin is usually dry, especially in winter. She also has a problem of bed wetting, whenever she sleeps (night as well as day). Sometimes they notice that she scratches the anal region. On further interrogation: Appetite: good Thirst: good Perspiration: normal Desires: ice , ice cream , cold drinks , bananas, milk Aversion: sweets Birth: FTND Milestones: Teething/ Walking- normal, Talking-late Mind: Inclined to be stubborn. Mixes well with all. Fears dark. No significant past or family history or other symptoms were available, as the patient’s grandfather had not observed anything else.

First encounters are memorable. I can remember feeling this slightly itchy, flaky rash developing around the edge of my nose during the first year of medical school, a highly stressful time. This was my first encounter with seborrheic dermatitis and it has remained with me off and on ever since, acting as a barometer of my stress level. It is a common skin problem of young professionals. Seborrhea is a disorder of inflammation and scaling that occurs in oily skin, like behind the ears, in the groove around the nose, on the eyebrows, sideburns and the scalp. The scalp component is considered a variant of common dandruff (see companion piece under D). Cause
Current thinking is that the skin is reacting to an overgrowth of Pityrosporum yeast in oil-bearing skin. There is an inherited component and there are some features in common with psoriasis. An intriguing fact is that any emotional or physical stress can cause seborrhea to worsen. There is no more dramatic example than when an individual develops the AIDS syndrome. In these cases, the rash shows a florid redness and scaling covering not only the face but the entire chest.

A patient information handout on seborrheic dermatitis, written by the authors of this article, is provided on page 2713. Seborrheic dermatitis is a chronic inflammatory disorder affecting areas of the head and trunk where sebaceous glands are most prominent. Lipophilic yeasts of the Malassezia genus, as well as genetic, environmental and general health factors, contribute to this disorder. Scalp seborrhea varies from mild dandruff to dense, diffuse, adherent scale. Facial and trunk seborrhea is characterized by powdery or greasy scale in skin folds and along hair margins. Treatment options include application of selenium sulfide, pyrithione zinc or ketoconazole-containing shampoos, topical ketoconazole cream or terbinafine solution, topical sodium sulfacetamide and topical corticosteroids. (Am Fam Physician 2000;61:2703-10,2713-4.) T he etiology of seborrheic dermatitis remains unknown, although many factors, including hormonal, have been implicated. This chronic inflammatory skin disorder is generally confined to areas of the head and trunk where sebaceous glands are most prominent. When seborrheic dermatitis occurs in the neonatal period, it usually disappears by six to 12 months of age, suggesting that it may be a response to maternal hormone stimulation.

An integrated approach to treatment is usually very effective... Diagnosis A Dermititis rash is quite itchy and can make the affected parts red and somewhat swollen. The cause is quite clear: Sebaceous glands, excrete fatty toxins through the skin. There are a lot of these fat-soluble toxins in the body, they become overactive. They produce more sebum which irritates the subcutaneous layer of the skin. This is the layer underneath the top layer of skin which is in fact dead skin cells. This leads to dermatitis or to acne. The tip of the nose has a particularly large collection of sebaceous glands and this is why some people get white heads there - they are simply the sebum being discharged from the glands. In seborrheic dermatitis, the production of the toxins is so intense that the skin becomes 'angry' and this results in a red nose.

Seborrheic Dermatitis
Seborrheic dermatitis is a common skin condition affecting millions of Americans. It shows up as flaking skin, or reddish patches. Unlike people with dry skin, the skin in people with seborrheic dermatitis is usually oily. It can be unsightly, itchy and, since it is often on the face, may cause embarrassment. It is not contagious. Scales form on reddened skin.
In adults seborrheic dermatitis usually affects the scalp, eyebrows, ear canals, sides of the nose, and behind the ears. It sometimes affects the armpits, chest and in the groin area. Most people with it complain of dandruff , especially on the back and sides of the scalp (bad dandruff bad dandruff is usually seborrheic dermatitis).
Stress, fatigue, weather extremes, oily skin and infrequent shampooing or skin cleaning make it worse. Severe medical illnesses, including AIDS, Parkinson's disease, head injury, and stroke are associated with seborrheic dermatitis. These are thought to make the normal skin care that prevents seborrheic dermatitis harder to keep up with. The vast majority of people with seborrheic dermatitis have no associated conditions.
Seborrheic dermatitis may start in infancy as cradle cap. It affects the scalp as thick, crusty, yellow scales. Children usually outgrow it by age 3 and do not normally get seborrheic dermatitis. Cradle cap is not contagious, it is not caused by poor hygiene, it is not an allergy, and it is not dangerous. Cradle cap usually does not itch, but it may. If excessive scratching occurs, it can cause additional inflammation, mild infections or bleeding.
