What is seborrheic dermatitis (cradle cap)?
Also called cradle cap, seborrheic dermatitis in the infant is characterized by fine white scales. However, seborrheic dermatitis can also occur in the diaper area, face, neck, and trunk. Seborrheic dermatitis in this age group usually clears within the first year. What causes cradle cap? The exact cause of this skin condition is not known. What does cradle cap look like? The following are the most common signs and symptoms of cradle cap. However, each child may experience symptoms differently. Symptoms include dry or greasy scales on the scalp. How is cradle cap diagnosed? Cradle cap is usually diagnosed based on physical examination of your child. The rash involved with cradle cap is unique, and can usually be diagnosed simply on physical examination. SEBORRHEIC DERMATITIS SEBORRHEIC DERMATITIS Definition: A dermatitis characterized by a greasy scale covering a yellow-red marginated base. In patients with HIV infection, usually the scalp and central face are involved, although it may be present in areas of the chest, axillae or groin. Some 25-83% of patients with HIV have experienced this disease; it may wax and wane over the course of infection. The etiology of seborrheic dermatitis is unknown. S: Patient complains of new rash, sometimes itchy, or of "dry skin" that will not go away in spite of the application of topical moisturizers. O: PE: Pruritic or asymptomatic greasy scale rash covering a yellow-red marginated base. Typically found on the scalp, nasolabial folds, eyebrows, and ears. May be found under chest hair, in axillary area or groin. A: Rule out Reiter's syndrome, candidiasis, and psoriasis. P: LABS: Do a KOH preparation to rule out candida albicans and other superficial yeast infections.

What is seborrheic dermatitis?
Seborrheic dermatitis is a common scaling rash that occurs in infants, teenagers and adults. Dandruff is seborrheic dermatitis of the scalp. Seborrheic dermatitis may also occur on the cheeks, eyebrows, eyelid edges, ears, the skin near the nose, and such skin fold areas as the armpits and groin.
What causes seborrheic dermatitis?
The cause of seborrheic dermatitis is unknown, although recently researchers have suggested that an overgrowth of yeast is to blame. It is not related to diet and it is not contagious. Stress and physical illness may worsen the condition but do not cause it. This condition often disappears in infants by one year of age. It is common in teenagers and especially in adults. Seborrheic dermatitis may get better or worse without any apparent reason.

Treating a Chronic Skin Condition Q: Michelle, a family nurse practitioner, is troubled by a follow-up visit from a patient. An obese man, the patient is complaining of continued intertrigo candida below the overhang of his abdomen. At his last visit 2 weeks ago, Michelle prescribed nystatin powder to be applied at bedtime and in the morning to prevent further episodes, but the patient hasn't had long-lasting relief. What other options should Michelle consider prescribing for this patient? A. A. Michelle is dealing with one of the most common themes in dermatology: we rarely cure patients. Intertrigo candida is a great example of why we can't offer a simple cure for this and similar conditions. So why do we even need the specialty? My answer is that as with most specialties, dermatology is pattern recognition; you see something over and over again, and you become acquainted with it so that you can anticipate it. The advice I would give Michelle is to look at the pattern presented with this patient. She should ask her patient questions about the timing of his breakouts, the OTC remedies he's tried, and the soaps and lotions he uses. Michelle should also understand that intertrigo is a separate condition from candidiasis. When the skin occludes and rubs on itself, moisture collects causing maceration. Slowly the skin begins to break down, setting up the condition of intertrigo. The yeast

The name of an antibody found most frequently in the blood of patients with clinically-amyopathic dermatomyositis. This is the form of dermatomyositis that causes a dermatomyositis skin rash but no muscle weakness.
Acute cutaneous lupus erythematosus
The formal medical term for the butterfly facial rash experienced by patients who have the systemic form of lupus erythematosus.
Alopecia
A general term referring to hair loss of any cause. Lupus patients can experience several different patterns of hair loss.

Chemical building blocks that, when linked together, form proteins. Proteins are the building blocks of cells.
Amyopathic dermatomyositis
A form of dermatomyositis in which skin disease is present for prolonged periods of time (6 months or greater) but no muscle weakness develops. Patients having this form of dermatomyositis have a very small risk of developing interstitial lung disease and perhaps have an increased risk of developing internal cancers.

An abbreviation for antinuclear antibodies. These are the antibodies that react to an individual's own tissue who is suffering from autoimmune disease. The test for antinuclear antibodies is universally positive in patients with systemic lupus erythematosus. However, this test is also positive in a number of other conditions besides lupus.

Drug Watch: Treatment of Dandruff
There is growing, though largely indirect, evidence of a relationship between the yeast Pityrosporum ovale and moderate to severe dandruff and seborrheic dermatitis. A number of treatments have become available which treat these relatively common conditions by attacking the causative agent. These include lithium salts, which inhibit small colonies of P. ovale , and ketoconazole and other imidazoles which interfere with steroid synthesis in yeasts.
Bandolier sought evidence of effectiveness through a MEDLINE search using the terms DANDRUFF, DERMATITIS-SEBORRHEIC(exploded to all subheadings) and RANDOM*. This revealed four RCTs (three in English) since 1991; references before 1991 were not used.
Lithium succinate ointment
This is a British multicentre double-blind, randomised, placebo-controlled study of an ointment containing 8% lithium succinate (Efalith) compared with base ointment. The study [1] was designed to be cross-over, but is also analysed as a parallel design (158 patients) which is somewhat easier to understand. Patients were instructed to apply the ointment sparingly to affected areas twice daily for four weeks.
Using 100mm visual analogue scales (VAS), patients assessed redness, scaling, greasiness, itching and the overall impression of treatment. All were significantly better with lithium succinate, with redness, scaling and itching being most improved. There was evidence that the effects of treatment extended at least four weeks after stopping.

Acne Rosacea
Acne rosacea is a misleading term: rosacea and acne are two entirely different conditions, although they can and do appear together. Clogged skin pores and bacterial infections cause acne. Rosacea occurs when blood vessels move to just below the skin's surface, leading to blotchy red patches. These blotches fade and then return, becoming more permanent over time.
The blackheads, whiteheads and pimples associated with acne are not associated with rosacea. Red bumps called papules are. These papules are usually solid and hard. The papules range in size from small bumps that resemble the measles or chicken pox, all the way to larger, penny-shaped nodules. Left untreated, larger nodules can cause rhinophyma (see below).
Ocular Rosacea
About half of all sufferers have symptoms of ocular rosacea , where symptoms affect the eyes. The eyelids may become inflamed and develop small red bumps, or develop scales and crusts after a night's sleep. The eyes may become bloodshot with a "gritty" feeling, as if a piece of sand were in the eye. Sensitivity to bright light is common. Occasionally, eyelash loss occurs.

Seborrheic Dermatitis (Severe Dandruff) I. Definition: Seborrheic dermatitis is a chronic, recurring scaly rash involving the face, ears, eyebrows, scalp, and sometimes the chest. It occurs more frequently as people get older. There is no cure for this condition. II. Causes:
  • A yeast named Malassezia ovalis that grows on the skin causes seborrheic dermatitis. People prone to seborrheic dermatitis are usually genetically susceptible to this condition. Seborrheic dermatitis is usually worse in the colder months.
III. Treatment:
  • Treatment is a matter of control rather than cure. Seborrheic dermatitis usually responds to topical therapy. The treatment depends on its site as well as its severity. Sebulex Shampoo , which can be purchased without a prescription, should be used every other day to wash the scalp. The more often you shampoo the less likely you are to have this problem. Other good shampoos are Head and Shoulders and Selsun Blue Shampoos that contain conditioners can make one more prone to this condition. If you use a shampoo with conditioner alternate it with one that has no conditioner. Hair pomade can contribute to this problem. The use of Sebizon Lotion with your favorite pomade is an effective treatment for this problem.

DEVELOPMENT OF DEMENTIA COMMON IN PATIENTS WITH PARKINSON DISEASE
Archives of Neurology , one of the JAMA Archives journals. According to the article, information about which patients with PD will eventually develop dementia may be useful for the patient, caregiver, and physician in planning future treatment. Studies exist that document the prevalence of dementia in PD, but none have included adequate observation time, or modern criteria for defining dementia. Dag Aarsland, MD, PhD, of Central Hospital of Rogaland, Stavanger, Norway, and colleagues performed an 8-year prevalence study of dementia in patients with and without PD in a community-based sample.
