What Is Selective Mutism?
    Selective Mutism is an anxiety disorder that is characterised by the consistant failure to speak in situations where speech is expected.
    Official criteria are :
  • Consistent failure to speak in specific social situations, despite speaking normally in other situations The disturbance interferes with educational or occupational achievement or with social communication. The duration of the disturbance is at least one month (not limited to the first month of school) The failure to speak is not due to lack of knowledge of the spoken language required in the social situation. The disturbance is not better accounted for by a communication disorder (e.g.,stuttering) and does not occur along with Autism or related disorders.

    November 2002 I finally decided I need to ask other parents' advice on this one: At this point, it starts being a little odd, especially when people ask me surprised: She cannot talk yet? My husband suggests that we start sending her to her preschool full-time to force her interaction with children and adults outside of home, yet neither he nor myself are quite sure that it will do the trick, since she has a lot of social interaction as it is, and this will deprive her (and her mom) of some quality time together. Besides, if it has to do with her personality (nature as opposed to nurture), then this will not achieve a thing. Just for the record: Our older daughter is 6.5 and she has always been the most outgoing child I knew in my life, and she never stops talking. I'm sure someone on the network has been in a similar situation. Could you please tell me how you worked through it? Maria

    A Family and Genetic Study of Selective Mutism:
    Relationship to Social Anxiety Disorder and Serotonin Genes This SM research study is a collaboration between SMG~CAN (Dr Elisa Shipon-Blum) and The University of California, San Diego ( Dr. Murray Stein)
    Study Objectives:
    The objective of this study is to interview families in order to confirm that social anxiety disorder segregates within families of Selectively Mute children. A secondary objective is to determine the genetic make-up of parents, siblings, and the SM child in terms of an association with specific genes, such as the serotonin transporter (SLC6A4) genes and subtypes which has been linked to anxiety disorders. Findings from this study will improve our understanding of the hereditary nature of Selective Mutism, it's relationship to DSM-IV anxiety disorders, and it's possible neurobiological association.
    Study Design:
    50 families with SM children will participate in this study. Dr. Shipon-Blum will evaluate and either diagnose or confirm a previous diagnosis of SM to screen for the interview portion of the study. Upon acceptance into this study, if each SM family would TRY to provide us with another family (without SM) to serve as a control for the study.

Questionnaire HOME Abstract Selective Mutism (SM) is a relatively rare childhood disorder that appears to be closely related to social phobia. Previous attempts to study the disorder have been hampered by the lack of a standard severity measure such as those existing for other psychiatric disorders. The current investigation examines characteristics of the Selective Mutism Questionnaire (SMQ), a 36-item multiple-choice scale designed to assess severity of selective mutism. The questionnaire relies on parent report of child's speaking behaviors and interference associated with failure to speak. To date, 263 parents of children exhibiting symptoms of SM completed the SMQ via the Internet. As revealed by preliminary analyses, the data appear to be consistent with previously reported findings. The measure has acceptable internal consistency as evidenced by Cronbach's alpha coefficient (.74). In addition, subscale severity scores were associated with overall interference rating.     Selective mutism
    Contents of this page: Definition Selective mutism is a condition occurring in childhood in which a child who is fluent with language frequently fails to speak in certain situations where language is expected. It typically occurs in school or social settings. This pattern of mutism must be observed for at least one month. The first month of school is not included as excessive shyness is commonly observed during this period. Parents often think that the child is refusing to speak, but usually the child is truly unable to speak in particular settings. Causes, incidence, and risk factors Most experts believe that there are environmental, biological, interpersonal, and anxiety-related factors which cause selective mutism. Most children with this condition have some form of extreme social phobia. Some affected children have a family history of selective mutism, extreme shyness, or anxiety disorders that may increase their risk for similar problems. This condition is most common amongst young children (usually before the age of 5). Symptoms

    Signs and tests The essential element of selective mutism is that the child has the ability to both comprehend and speak, but fails to do so in certain settings or environments. This syndrome differs from mutism because the child sometimes speaks, depending on the situation. Children with mutism never speak.

    Without Samuel the term elective mutism would have had only a fleeting interest for me. What I knew about it came from a fascinating chapter on the subject in book " Reflexes, Learning and Behavior "
    And then came Samuel, five-year-old twin boy of one of my friends. For a whole year Samuel never opened his mouth in pre-school. If the teacher tried to find out if he had learned what she had tried to teach, there was no reaction. The parents did not particularly worry about this, because when he came home, Samuel would cheerfully tell his mother everything that went on in class, and showed her what he had learned. Obviously, Samuel was not stupid!
    Then came kindergarten. The twins were placed into different classrooms, because otherwise the boy twin would usually follow whatever his sister told him to do. Thus, when the two were separated, it became obvious that there was a huge difference in their behavior. The little girl took an active part in class, and played with others during recess. Samuel never talked in class and in the schoolyard just stood motionless on the sidelines.
    At this point the parents finally asked me to look at the children. As the parents watched, I did the
