Semantic Pragmatic Disorder Articles: Autism and Autistic-like Conditions: Subclasses among Disorders of Empathy , by Christopher Gillberg. In this academic paper, noted researcher and author Christopher Gillberg proposes that "Kanner autism", Asperger syndrome, high-functioning autism, and PDD-NOS, in addition to several other disorders, are all part of a broader group of "empathy disorders". Of particular importance to parents and clinicians is the wealth of data Gillberg presents in support of his argument for extensive neuropsychiatric and medical work-ups in all children with autistic-like conditions. This article was originally published in The Journal of Child Psychology and Psychiatry and Allied Disciplines, Vol. 33, No. 5, pp. 813-842, 1992. Definition of semantic pragmatic disorder / Signs that your child may be semantic pragmatic / Useful links Children who are identified as having Semantic Pragmatic Disorder (SPD) have a particular difficulty in the way they develop the ability to make sense of what words mean, how words and the behaviours around words (such as tone of voice non-verbal signals etc.) are used to convey ideas and feelings and how language is used as a social tool. There is a lot of controversy around the classification of this disorder. Some authorities are firmly of the opinion that it is part of the spectrum of disorders classified as Autistic - others are equally clear that there is a linguistic, language-based component to the disorder. The child with semantic pragmatic disorder will often rely on ritual and repetition to make sense of a confusing and apparently random environment. They are able to relate to people on their own terms and while there are similarities with elements of autistic behaviour, the child with SPD can demonstrate a willingness to communicate and an awareness of the difficulty that they are having, which can be very frustrating for them and those around them.

Autism
To many clinicians (myself included) autism is the far end of the ADD/ADHD continuum. Many of the same symptoms may occur, and additional symptoms are also possible. Again, individuals are different and there is no set of exact symptoms. Like ADD/ADHD, the diagnosis of autism is difficult to distinguish from ADD/ADHD with many differing symptoms common to both. The diagnostic "guidelines" used by psychiatrists and psychologists are mainly for insurance purposes, and can be wildly different between clinicians. There is a test developed by Psychologist Simon Baron-Cohen and his colleagues at Cambridge's Autism Research Centre. It's the Autism-Spectrum Quotient , or AQ, as a measure of the extent of autistic traits in adults. In the first major trial using the test, the average score in the control group was 16.4. Eighty percent of those diagnosed with autism or a related disorder scored 32 or higher. The test is not a means for making a diagnosis, however, and many who score above 32 and even meet the diagnostic criteria for mild autism or Asperger's report no difficulty functioning in their everyday lives. To use this test go to Autism Test Originally, the term autism meant that the child could not communicate to others. They were in a world of their own. But, this term wasn't enough, as there were individual variations here as well. Some could communicate, but not well.

Has anyone heard of this condition?
I have a 2 year 9 month old boy who has a vocabulary of five words, and as far as I can tell no comprehension without visual aids. For example if I ask if he wants a drink there is no response but if I show him a cup he doesn't nod but lets me know he wants it be running at me. He shrieks to get our attention and will shriek instead of using words. He can act out videos having only seen them once, but seemingly has no pretend play or imagination. No hearing problems, phobias or compulsions. Likes his own way or a shrieking fit will follow or a very angry temper tantrum which involves throwing anything close by. Good relations with brothers and sisters and parents, but not with children outside the family. Has been refused speech therapy until he sees an autism specialist which will be in the next four weeks. What little I have read on SPD could this be a possible diagnosis?

I couldn't find this specifically in the DSM IV (Diagnostic/Statistical
Manual). Under communication disorders are: Expressive Language Disorder,
Mixed Receptive-Expressive Language Disorder, Phonological Disorder,
Stuttering, and Communication Disorder NOS (not otherwise specified. I think
that Semantic Pragmatic Disorder must be speech-language pathology jargon
rather than psychiatric jargon.

Background: Asperger disorder is a form of pervasive developmental disorder characterized by severe persistent impairment in social interactions, repetitive behavior patterns, and restricted interests. Unlike an autistic disorder, no significant delay occurs in language development or cognitive development. Asperger disorder generally is evident in children older than 3 years and primarily occurs in boys. Children with this disorder often exhibit a limited capacity for empathy, a failure to develop friendships, and a limited number of intense and highly focused interests. While people with Asperger disorder may have certain communication problems, including poor nonverbal communication and pedantic speech, many individuals have good cognitive and verbal skills. Physical symptoms may include early childhood motor delays, clumsiness, fine motor difficulty, gait anomalies, and odd movements. Although normal language and cognitive development differentiate Asperger disorder from other developmental disorders, the severe social impairment associated with this condition overlaps disorders such as high-functioning autism (HFA). For clinical management purposes, Asperger disorder and HFA may be considered together. Impaired social skills are associated with several other conditions (eg, developmental learning disability of the right hemisphere, nonverbal learning disability, schizoid personality disorder, semantic-pragmatic processing disorder, social-emotional learning disabilities).
