Introduction The NNI Spine programme manages a variety of spine disorders.
The most common spine disorder is the degenerative condition of cervical and lumbar spondylosis, whereby the discs, bony protrusions and hypertrophied ligaments press on the nerve roots and spinal cord to cause pain, numbness, weakness and at times difficulty in walking. Treatment The majority of such condition can be treated with physical therapy and medications. However, those with spinal cord compression and neurological deficit, i.e. difficulty in walking, will require surgery.
Less common are spinal tumours which can be benign or malignant. These tumours are usually required to be removed so that they do not cause further paralysis.
Sometimes congenital disorders like chiari malformation with syringomyelia, where fluid circulating in the spinal cord is blocked, cause symptoms of numbness, weakness and loss of heat sensation. This condition may also improve with treatment. Neuro-Spine Programme Acute trauma with spinal injury is serious and often cause paralysis. The neuro-spine programme manages such patients in an acute spine injury unit in collaboration with the TTSH Spine Programme, whereby surgeons, therapists, nurses and rehabilitation physicians work together.

Spinal Disorder Center
The Spine Center at Orthopadic Hospital was established in 1988. It has since evolved into a comprehensive spine center for evaluation and treatment of complicated spine problems.
Due to the evolution in the management of spinal disorders in recent years, advanced rehabilitative techniques have eliminated the need for surgery in the majority of spine problems. When surgery is necessary, new techniques at Orthopaedic Hospital are available including microsurgery, as well as complex spinal reconstruction with state-of-the-art instrumentation for scoliosis and spinal deformity.

CONGENITAL spine disorders, Congenital spinal disorders are due to a developmentdefect in the mesenchymal anlage during the 4th to 6th week of gestation.

PAEDIATRIC SPINAL DISORDERS There are many variations. These have been divided and grouped into eight separate categories as shown below: IDIOPATHIC SCOLIOSIS This manifests as a lateral deviation and rotation of the spine with no identifiable cause, but it is usually related to a brain stem, hormonal or proprioception disorder. The adolescent form is most common but there is an Infantific form also a Juvenile form. Right thoracic curves, with apex T7 or T8 are the most common, followed by double major, left lumbar and right lumbar, in that order. Afflicted children are first noticed because of a trunk shift, shoulder or pelvic asymmetry, asymmetric umbilical reflex, spinal curvature, asymmetric rib hump and/or limb length inequality. Inclusion of the iliac crest on radiographs will reveal the degree of skeletal maturity. A lateral radiograph to check for spondylolisthesis is also recommended. An M.R.I. scan is needed if there is excessive Kyphosis, rapid progression, neurologic symptoms, onset before 11 years, of left thoracic/thoracolumbar curves. Treatment includes careful observation, some for bracing and if needed surgery.

The Department of Neurological Surgery participates in an internationally recognized integrated spine program. Working with the Departments of Physical Medicine and Rehabilitation, Orthopaedic Surgery, and Neurology and the Divisions of Neuroradiology and Neuropsychology we provide a multidisciplinary approach to managing spine disease. Such an approach is critical because spine disease will affect 8 out of 10 Americans at some point in their adult lives. Most of our patients' spine problems can be managed without surgical interventions. When surgery is required, however, our department delivers state-of-the-art technology and surgical expertise to optimally manage these problems. The Division of Spinal Surgery offers programs for the management of spinal deformity, spine oncology, degenerative disease, minimally invasive spine surgery, and rheumatologic disorders of the spine. These specific programs are directed by experts in the field and complement the management of routine disorders of the spine. Spine Deformity Northwestern's program for spinal deformity includes the management of scoliosis and sagittal plane disorders, such as kyphosis and hyperlordosis, and is considered a national leader in these areas in the field of neurological surgery. Pediatric spine disorders, such as idiopathic scoliosis and congenital scoliosis, are managed by faculty members at Children's Memorial Hospital. Adult scoliosis, such as that occurring as a consequence of degenerative disease or as a late progression of idiopathic scoliosis, is managed by faculty members at Northwestern Memorial Hospital. Our department is engaged in research on deformity of the spine, including the pathophysiology of scoliosis and mechanics of saggital plane disorders.
