The primary benefit to thoracoscopic spinal surgery over the traditional thoracotomy is in the decreased level of post-operative pain. This seems to be a nearly universal observation. By Laurence E. Mermelstein, M.D. Spinal deformity is the most frequent problem that will cause a child and his family to seek out spinal evaluation. The diagnosis of a spinal deformity can cause a great deal of distress within a family, mostly due to a lack of knowledge regarding the problem. In the vast majority of cases, these deformities can be managed non-operatively and only require careful observation. This article will highlight the latest medical information regarding the most common pediatric spinal deformity, Idiopathic Scoliosis. DEFNITION
Scoliosis is defined as a lateral curvature of the spine, most readily noticed when looking at the child from the back (or front). This is frequently manifested as a shoulder or pelvic asymmetry or an overall imbalance of the trunk to the left or right. ETIOLOGY
There are several known different etiologies of scoliosis, the most common of which is termed idiopathic, a technical term for unspecified or unknown cause. Other less common causes are congenital (secondary to vertebral anomalies that develop in utero) paralytic (secondary to muscle weakness) or neuromuscular in origin (e.g. Cerebral Palsy, Spina Bifida, Muscular Dystrophy). Various theories regarding the etiology of idiopathic scoliosis have been put forth, but none have been completely validated. Inheritance has a definite role in predisposing a child to developing scoliosis, but a discrete gene or mode of inheritance has not been established. Hormonal imbalance, abnormal tissue structure, subtle neurologic imbalances and/or abnormal reflexes have all been considered as etiologic factors, with no one reason being entirely responsible.

Cervical and thoracic spine disorders can affect more than one vertebral level of the spine. Degenerative disc disease, spinal stenosis, excessive kyphosis, vertebral fracture, and other disorders can cause pain, numbness, weakness, and tingling in the upper back, shoulders, and arms. In some cases, surgery is necessary. Sometimes these disorders involve the junction between the cervical and the upper thoracic spine. Characteristically, the cervical spine is quite flexible and the thoracic spine is rigid. Disorders involving these vertebral levels can present a specific challenge to the spine specialist. Now these difficult cases can be instrumented using a different device that is modular by design. This device allows the spine surgeon to stabilize the cervical and upper thoracic spine using anchors (screws and hooks) attached to a rod in a variety of positions.

SOUTHWEST GEORGIA REGIONAL SPINE CENTER DISC DISORDERS OF THE Intervertebral discs are the "cushions" of cartilage between the bony vertebral bodies in the cervical, thoracic, and lumbar spine. There existence allows the body to accept and dissipate load across multiple levels in the spine and still allow for the flexibility required for performing normal activities of daily living. As the body twists, bends, flexes and extends; the intervertebral discs are constantly changing their shape. The discs themselves are composed of a tough, outer fibrous layer (the annulus fibrosis) and a soft, almost gelatinous inner layer (the nucleus pulposis). As a part of the aging process, the discs begin to lose their high water content and their ability to dissipate force as efficiently (SO CALLED DEGENERATIVE DISC DISEASE). By the age of 35, approximately 30% of people will show evidence of disc degeneration at one or more levels. By the age of 60, greater than 90% of people will show evidence of disc degeneration at one or more levels on MRI. In some patients, this disc degeneration can be nearly asymptomatic; in others, disc degeneration can lead to intractable back pain. As the discs deteriorate, the spine can initially become less stable. Bony spurs can develop as a result of this instability and can cause pressure on nearby nerves leading to leg or arm pain. Narrowing of the neural canal by these bony spurs is known as degenerative spinal stenosis. The outer layer of the discs themselves can also tear. When this occurs, the inner, gelatinous layer can herniate out ( A "HERNIATED" OR "RUPTURED" DISC) and also cause pressure on an adjacent nerve. If the herniation occurs in the neck and causes pressure there, this can cause pain that radiates into the shoulder and arm; if it occurs in the back, the pain produced can radiate down into the hip and leg.
