Description
PCOS, also called Stein-Leventhal syndrome, is a group of symptoms caused by underlying hormonal and metabolic disturbances that affects about 6% of premenopausal women. PCOS symptoms appear as early as adolescence in the form of amenorrhea (missed periods), obesity, and hirsutism , the abnormal growth of body hair. A disturbance in normal hormonal signals prevents ovulation in women with PCOS. Throughout the cycle, estrogen levels remain steady, LH levels are high, and FSH and progesterone levels are low. Since eggs are rarely or never released from their follicles, multiple ovarian cysts develop over time.

Overview Symptoms Treatment Prevention Definition: Stein-Leventhal syndrome is an accumulation of many incompletely developed follicles in the ovaries. This condition is characterized by irregular menstrual cycles, scanty or absent menses , multiple small cysts on the ovaries (polycystic ovaries), mild hirsutism, and infertility . Many women who have this condition also have diabetes with insulin resistance.

Causes, incidence, and risk factors: Abnormal ovarian function sometimes causes incompletely developed follicles (ova) to accumulate in the ovaries. These ova fail to mature and ovulate, therefore causing them to accumulate as cysts in the ovary and contributing to infertility in the woman. Polycystic ovaries are two to five times as large as normal ovaries, and they have a white, thick, very tough outer covering. This condition is commonly called Stein-Leventhal syndrome, and usually develops shortly after puberty. A woman with polycystic ovaries will stop menstruating (she may not have ever started) or will menstruate erratically. She will gain weight, eventually becoming

Polycystic Ovary Syndrome Polycystic ovary syndrome (PCOS), also called Stein-Leventhal Syndrome, is the most common endocrinologic disorder in women of reproductive age. Approximately 5-10% of reproductive age women have PCOS. This syndrome can have many symptoms. However, the two key components defining this disorder must include chronic anovulation (inability to ovulate an egg) and clinical hyperandrogenism (elevated male type hormones). The various symptoms of PCOS can be irregular or absent menstrual cycles, infrequent or absent ovulation, excess facial and body hair, male pattern balding, acne of face/back/chest, and infertility. Other findings can include an elevated FSH to LH hormone ratio, elevated levels of male hormones, multiple small cysts of the ovaries and elevated cholesterol. Some women with PCOS also suffer from other subtle endocrine abnormalities. One is insulin resistance, which affects sugar and fat metabolism, and may increase the long-term risks of heart disease, diabetes and high cholesterol. Insulin resistance (IR), the precursor state to diabetes, is present in 35-40% of women with PCOS, even if they are not overweight. Insulin resistance is diagnosed by blood testing, either as fasting glucose to insulin ratio, or as a complete glucose tolerance test (GTT). Long term follow up of women with PCOS reveals that up to 40% develop impaired glucose processing or diabetes by age 40. The prevalence of diabetes in women with PCOS is seven times higher than for the non-PCOS population. Excessive insulin production is thought to promote excess male hormone production, though the actual mechanism explaining this observation is still unclear.

Polycystic Ovary Syndrome (PCOS), also called Stein-Leventhal Syndrome, is a disorder characterized by the absence of menstruation or irregular and abnormal menstruation, excessive amounts of body hair, excessive body weight, and subfertility. The syndrome is caused by abnormally elevated levels of testosterone and luteinizing hormone which disrupt the normal menstrual cycle. Immature cysts remain on the ovaries, giving it the characteristic "string of pearls" appearance. An inappropriate response to insulin is also suspected, but does not explain all cases. It is estimated that this condition affects 6 to 10% of premenopausal women of all races and ethnicities, beginning at puberty or in the early 20s.
Symptoms of PCOS
The most common symptom of PCOS is irregular menstruation which can take several forms:

This syndrome was first described in 1935 by Drs. Stein and Leventhal, and for many years PCOS was known as the Stein-Leventhal Syndrome. Women wth PCO have enlarged ovaries containing multiple small cysts which have led to the descriptive term, polycystic ovaries. PCOS is comprised of several clinical features, each of which may be present to a lesser or greater degree. Some women have been found to have polycytic ovaries without associated abnormalities of menstruation, hair growth, weight gain or infertility. These womem do not have Polycystic Ovary Syndrome. Thus, not all women with polycystic ovaries have PCOS, but all women with PCOS have polycystic ovaries. Women with PCOS have normal reproductive organs such as the uterus and fallopian tubes. Their ovaries each contain multiple small cysts around the periphery, each ovarian cyst generally measuring less than 8 mm diameter and easily seen by pelvic ultrasound. These cysts do not appear to grow and usually remain small. They do not require surgical removal and are not associated with an increased risk of ovarian cancer. The symptoms of PCOS
Prior to initiating fertility treatment, other factors which impact fertiity are usually evaluated. These factors include tubal patency, pelvic anatomic relationships, assessment of semen and sperm function, cervical mucous quality, presence of immunologic causes of infertility and uterine anatomical abnormalities. In women with PCOS, failure to ovulate is the usual reason for not achieving pregnancy.
