What research is being done? ... Organizations What is Stiff-Person Syndrome? Is there any treatment? The drug diazepam, which relaxes the muscles, provides improvement in most cases. Baclofen, phenytoin, clonidine, or tizanidine may provide additional benefit. In some patients, immunomodulatory treatments such as intravenous immunoglobulin may be beneficial. Physical and rehabilitation therapy may also be needed. What is the prognosis?

Stiff-Person Syndrome
Stiff-person syndrome is a rare progressive neurological disorder characterized by constant painful contractions and spasms of voluntary muscles, particularly the muscles of the back and upper legs. Symptoms may occur gradually, spreading from the back and legs to involve the arms and neck. Symptoms may worsen when the affected individual is anxious or exposed to sudden motion or noise. Affected muscles may become twisted and contracted, resulting in bone fractures in the most severe cases. Individuals with stiff-person syndrome may have difficulty making sudden movements and may have a stiff-legged, unsteady gait. Sleep usually suppresses frequency of contractions. Stiffness may increase and patients may develop a hunched posture (kyphosis) or a swayback (lordosis). Researchers theorize that stiff-person syndrome may be an autoimmune disorder. Other autoimmune disorders such as diabetes, pernicious anemia (a chronic, progressive blood disorder), and thyroiditis (inflammation of the thyroid gland) may occur more frequently in patients with stiff-person syndrome.

Stiff Man Syndrome is a neuromuscular condition in which a hyperactive startle reflex results in the contraction of the muscles causing violent spasms. These spasms are capable of slamming the victim into walls and furniture. The disorder's cause is unknown, although evidence is pointing increasingly to the fact that it is an autoimmune disorder. Although they appear completely normal, those afflicted with Stiff-Man Syndrome must avoid any situation where they might be exposed to sudden sounds or stimulation. Often, they remain isolated in their home and excluded from normal living. They are orphaned from society by this rare and ruthless disorder.     ABSTRACT Background Stiff-person syndrome is a disabling central nervous system disorder with no satisfactory treatment that is characterized by muscle rigidity, episodic muscle spasms, high titers of antibodies against glutamic acid decarboxylase (GAD65), and a frequent association with autoimmune disorders.     Patients with stiff-person syndrome, a disabling central nervous system disease with no effective therapy, are plagued by muscle rigidity and episodes of muscle spasm. Patients have high titers of antibodies against glutamic acid decarboxylase (anti-GAD65), suggesting an autoimmune pathogenesis. In this study, 16 patients were randomly assigned to receive either placebo or intravenous immune globulin for three months, followed by a washout period and then three months of therapy with the other agent. Eleven of the 14 patients who completed the study had a marked improvement in their condition with active therapy. Intravenous immune globulin is a safe and effective, though costly, therapy for patients with stiff-person syndrome and anti-GAD65 antibodies.

    The stiff-person syndrome, a rare and disabling disorder, is characterized by muscle rigidity and episodic spasms that involve axial and limb musculature. Continuous contraction of agonist and antagonist muscles caused by involuntary motor-unit firing at rest are the hallmark clinical and electrophysiologic signs of the disease. Except for global muscle stiffness, results of neurologic examination are usually normal. Results of conventional computed tomography and magnetic resonance imaging of the brain are also normal. The cause of the stiff-person syndrome is unknown; however, an autoimmune pathogenesis is suspected because of 1) the presence of antibodies against glutamic acid decarboxylase (GAD), the rate-limiting enzyme for the synthesis of the inhibitory neurotransmitter -aminobutyric acid (GABA); 2) the association of the disease with other autoimmune conditions; 3) the presence of various autoantibodies; and 4) a strong immunogenetic association. Anti-GAD antibodies, which are found in high titers in most patients, seem to be directed against conformational forms of GAD. New evidence suggests that these antibodies may be pathogenic because they interfere with the synthesis of GABA. In addition, a reduction in brain levels of GABA, which is prominent in the motor cortex, has been demonstrated with magnetic resonance spectroscopy in patients with the stiff-person syndrome.
