Co-occurring Mental and Substance-related Disorders Assessment This section covers co-occurring diagnostic and assessment issues that commonly arise for the patient with both a psychiatric and a substance abuse disorder. This section would apply to individual and facility providers, and for in-patient, outpatient and alternative levels of care settings.
  • All admissions or potential admissions for either a psychiatric diagnosis or a substance abuse diagnosis should be screened for a co-occurring disorder.

What are Substance-related Disorders?
Jeremy was only 14 when he first starting drinking heavily. At the beginning, this drinking was limited only to weekends. However, by the time he finished school and married he was drinking on a daily basis. Although his wife insisted that he drank too much, Jeremye never thought of himself as having a problem. That changed after he was involved in a serious car accident, where he wrote off two cars. The police who arrived at the scene of the accident insisted that his blood be taken for alcohol analysis. In view of the fact that his alcohol level far exceeded recommended levels, Jeremy was found negligent. Only at that point did he agree to go ahead with his wife's suggestion that he seek help. Ever since she could remember, Jane suffered from ongoing feelings of sadness and anxiety. The first time she remembered feeling happy was after a friend gave her an "upper". It was as if all her worries had washed away, and she could experience the joy of being alive. Unfortunately, she found that after coming down from her high, life seemed as bleak, if not bleaker than ever. She found herself using higher and higher doses, and experimenting with an increasing variety of street drugs. In order to pay for these, she resorted to some drug-trading and began to get involved in petty crime. As she said to herself at the time, it was worth anything to her to escape from the emptiness and worry that she felt when she was off of drugs.

Generic Criteria for Substance Dependence
  • The patient's maladaptive pattern of substance use leads to clinically important distress or impairment as shown in a single 12-month period by 3 or more of the following:

  • -Tolerance, shown by either of:
    - markedly increased intake of the substance is needed to achieve the same effect or
    -with continued use, the same amount of the substance has markedly less effect
    -Withdrawal, shown by either of:
    -the substance's characteristic withdrawal syndrome or
    -the substance (or one closely related) is used to avoid or relieve withdrawal symptoms
    -The amount or duration of use is often greater than intended.
    -The patient repeatedly tries without success to control or reduce substance use.
    -The patient spends much time using the substance, recovering from its effects or trying to obtain it.
    -The patient reduces or abandons important social, occupational or recreational activities because of substance use. -The patient continues to use the substance, despite knowing that it has probably caused physical or psychological problems.
Specify whether: With Physiological Dependence. There is evidence of tolerance or withdrawal (see above).
