ASAM Public Policy Principles for Outcome Evaluation in the Treatment of Substance-Related Disorders: a Joint AMBHA-ASAM Statement INTRODUCTION: The American Managed Behavioral Healthcare Association (AMBHA) and the American Society of Addiction Medicine (ASAM) share an interest in the treatment of addiction and of other substance-related disorders. Both organizations seek to promote treatment interventions that have high-quality outcomes, and encourage studies of the outcome of each step of the full continuum of services for substance-related disorders. Determining the most salient dimensions of treatment effectiveness is challenging. Relevant derivative questions include “what kind of treatment?” and “for what kind of condition?” as well as “what aspect of effectiveness?” and “quality as defined by whom?” AMBHA and ASAM have previously collaborated in the development of conjoint statements on Effective Treatment of Addictive Disorders, Parity in Benefit Coverage, Practice Guidelines, and A Guideline for Credentialing and Privileging of Clinical Professionals for Care of Substance-Related Disorders.

Patient Placement Criteria, Second Edition Revised American Society of Addiction Medicine (ASAM) Publishes Second Edition -Revised of Patient Placement Criteria (ASAM PPC-2R) The American Society of Addiction Medicine (ASAM) has just published the Second Edition - Revised of its Patient Placement Criteria (ASAM PPC-2R), the most widely used and comprehensive national guidelines for placement, continued stay and discharge of patients with alcohol and other drug problems. Responding to requests for criteria that better meet the needs of patients with co-occurring mental and substance-related disorders ("dual diagnosis"), for revised adolescent criteria and for clarification of the residential levels of care, the ASAM Patient Placement Criteria for the Treatment of Substance-Related Disorders, (Second Edition -– Revised ): (ASAM PPC-2R) was released in April, 2001. The ASAM PPC-2R provides two sets of guidelines, one for adults and one for adolescents, and five broad levels of care for each group. The levels of care are: Level 0.5, Early Intervention; Level I, Outpatient Treatment; Level II, Intensive Outpatient/Partial Hospitalization ;Level III, Residential/Inpatient Treatment; and Level IV, Medically-Managed Intensive Inpatient Treatment. Within these broad levels of service is a range of specific levels of care. For each level of care, a brief overview of the services available for particular severities of addiction and related problems is presented; as is a structured description of the settings, staff and services, and admission criteria for the following six dimensions: acute intoxication/withdrawal potential; biomedical conditions and complications; emotional, behavioral or cognitive conditions and complications; readiness to change; relapse, continued use or continued problem potential; and recovery environment.

Obsessive-Compulsive Foundation Related Disorders What other problems are sometimes confused with OCD? Some disorders that closely resemble OCD and may respond to some of the same treatments are Trichotillomania (compulsive hair pulling), body dysmorphic disorder (imagined ugliness), and habit disorders, such as nail biting or skin picking. While they share superficial similarities, impulse control problems, such as substance abuse, pathological gambling, or compulsive sexual activity, are probably not related to OCD in any substantial way. The most common conditions that resemble OCD are the tic disorders (Tourette's disorder and other motor and vocal tic disorders). Tics are involuntary motor behaviors (such as facial grimacing) or vocal behaviors (such as snorting) that often occur in response to a feeling of discomfort. More complex tics, like touching or tapping tics, may closely resemble compulsions. Tics and OCD occur together much more often when the OCD or tics begin during childhood. Depression and OCD often occur together in adults, and, less commonly, in children and adolescents. However, unless depression is also present, people with OCD are not generally sad or lacking in pleasure, and people who are depressed but do not have OCD rarely have the kinds of intrusive thoughts that are characteristic of OCD.

Buckley PF. Department of Psychiatry, Case Western Reserve University, and the Northcoast Behavioral Healthcare System, Cleveland, Ohio 44106, USA. Approximately half of the patients who suffer from schizophrenia are also substance abusers at some time during their illness. The motivational drive toward abusive consumption is compounded in individuals with schizophrenia who turn toward substances with reinforcing properties to alleviate aspects of psychosis. This review examines the prevalence, etiology, and clinical effects of substance abuse (e.g., alcohol, nicotine, cocaine) among individuals with schizophrenia. Clearly, substance abuse persists despite and in spite of treatment with typical antipsychotics. The efficacy of newer generation antipsychotics in the reduction of substance abuse among the schizophrenic population has yet to be established, but clozapine has been shown to reduce alcohol, smoking, and cocaine use. Hence, clozapine is a therapeutic option for dually diagnosed patients because of its superior efficacy relative to conventional neuroleptics and its capacity to control substance abuse.
