ABSTRACT
Neuropathic spinal arthropathy is a rare but well-reported sequela of tabes dorsalis. It is now more frequently seen as a complication of spinal cord injury - particularly after spinal fusion. With improved longevity of spinal cord injured patients, we are seeing this entity in increased frequency.
Clinical and radiographic aspects are described, and a case history is depicted. Classic treatment has been conservative. Based on the changing nature of its etiology and favorable reports from large centers we advocate anterior and posterior fusion and instrumentation of Charcot's Spine.

CASE HISTORY
The patient is a 55 year old white male quadriplegic since a diving accident at age 15. Thirty years later he began experiencing local thoracic back pain and difficulty with "sinking" into his wheelchair. Examination revealed a low thoracic palpable clunk and overlying decubitus ulcer. Figures 1 and 2 demonstrate the classic ball-and-socket giant pseudoarthrosis. A Classic Charcot Spine at T11-T12 A plastic surgeon was consulted and excision of the decubitus ulcer and infected bursa was performed with primary closure. Six weeks later, anterior T11 resection, debridement and iliac crest strut grafting were performed. Posterior spinal fusion with iliac crest bone grafting and CD instrumentation from T9 to L2 was completed at the same setting. (Figures 3, 4) Estimated blood loss was 1800 cc. Cultures were negative.

    The high-profile tour did not conceal that the author was ill. She spent part of her three months in the United States on intravenous infusions. At this time of her life, three years before her death, she weighed less than eighty pounds and ate almost nothing. Her hosts offered oysters, grapes, and champagnefoods she could keep downbut even these she ignored in favor of entertaining the assembled guests. Through a variety of hints, confidences, and declarations, she told intimates that she suffered from syphilis. Karen Blixen does not mention her illness in her lyrical memoir Out of Africa . Her book offers her impressions of Africa and the friends she made therenot insights into her private life. Denys Finch Hatton is a friend, not her lover, in her story. Karen Blixen went to Africa to marry her second cousin Bror Blixen and gain the title of Baroness. Her husband was generous with his affections, and his infidelities resulted in her syphilis in their first year of marriage. Bror Blixen recovered without medications, but Karen Blixen's more serious symptoms required that she take mercury and arsenicthe only medicines used to treat the disease at that time. Despite complications, she persevered with her life in Kenya. Years later she developed unusual abdominal pain, which she feared was caused by tabes dorsalis, late syphilis of the spine.

" Locomotor ataxia," although it expresses a very" characteristic feature of the disease,has this objection:it is a symptom which does not occur in the first (preataxic) stage of the disease; indeed a great number of years may elapse before ataxy comes on, and sometimes the patient, after suffering a very long time from the disease, may die from some intercurrent complication, having never been ataxic. The complications which may arise in this disease are inter-current affections due to septic conditions of the bladder, bedsores, pneumonia, vascular and heart affections. About 10% of the cases, at least, develop general paralysis of the insane. This is not surprising seeing that it is due to the same cause, and the etiology of the two diseases is such as to lead many neurologists to consider them one and the same disease affecting different parts of the nervous system. Tabes dorsalis occurs with much greater frequency in men than in women (see NEUROPATHOLOGY) . The avoidance of all stress of the nervous system, whether physical, emotional or intellectual, is .indicated, and a simple regular life, without stimulants or indulgence of the sexual passion, is the best means of delaying the progress of the disease. Great attention should be paid to micturition, so as to avoid

    The causitive organism of syphilis, Treponema pallidum, has a predilection for neural tissue and in neurosyphilis (tertiary), can cause tabes dorsalis, a wasting of the posterior roots of the spinal cord. Notice how the example below lacks staining myelin. The neurological deficits can lead to ataxia, Romberg's sign, hypotonia, areflexia, impotence, constipation, and hypotonic bladder. Patients may also complain of lancinating leg pain and, due to a loss of proprioception, are at increased risk for Charcot joints. Tertiary syphilis is also associated with Argyll-Robinson pupils, optic atrophy, and visceral crises.
