Die sekundäre Manifestation der Syphilis kann außerordentlich vielfältig sein. In dieser Phase treten die Treponemen von den Lymphbahnen und Lymphknoten ins Blut über und verteilen sich dadurch im ganzen Körper. Typisch ist eine allgemeine Schwellung der Lymphknoten und lokale oder generalisierte Hautveränderungen. Die Hautveränderungen sind symmetrisch, fleckig, papulär, papulosquamös und/ oder pustulär. Nach vier bis zwölf Wochen heilen die Hautläsionen spontan ab. In Hautfalten können die rötlichen oder grauweißen Papeln oder Pusteln geschwürig zerfallen. Sie sind dann hochinfektiös. Man nennt diese besondere Hautmanifestation "Conyloma latum". Oberflächliche Schleimhautdefekte sind rot oder grauweiß mit gerötetem Hof. Gelegentlich kommt es zu Haarausfall am Kopf mit der typischen Mottenfraßcharakteristik. Rapidly Progressive Tabes Dorsalis Associated with Selective IgA Deficiency Satoshi Nishimura, Hideaki Miura, Haruki Yamada, Tomiko Ryu and Yasusada Miura From the Department of Internal Medicine, Social Insurance Chuo General Hospital, Tokyo Tabes dorsalis is uncommon and progresses slowly from infection to clinical manifestation. We report a rare case of rapidly progressive tabes dorsalis associated with selective IgA deficiency (sIgAD). A 28-year-old man was hospitalized with lightning back pain, nausea, and bladder bowel dysfunction. Serum and cerebrospinal fluid (CSF) revealed high titers of Treponema pallidum antibody, and the serum IgA level was less than 5 mg/dl. Th1-dominant cytokine expression was observed, as is usually seen in neurosyphilis. He was treated with Ceftriaxone and CSF pleocytosis disappeared. We postulate sIgAD influenced the atypical rapid clinical course of tabes dorsalis in this patient.

Baldness. Conjunctivitis. Locomotor ataxy. Paraplegia. Characteristics. Thallium is a rare metal discovered by Crookes in the residuum left from the distillation of Selenium and named Thallium H. W ., xxxiii. 242). It has been experimented with by Lamy and Marme. Some symptoms of Thal. sul ., which is an energetic poison, are included (and distinguished) in the Schema. The symptoms observed on animals were from the sulphate, with which Lamy experimented ( H. W ., xxxiv. 82). Combermale, of Lille, used Thal . with success in the night-sweats of phthisis, but this treatment had the effect of causing profuse falling of the hair, so serious as to contraindicate its use. Huchard had patients become quite bald in several days. Hansen says Thal . relieves the violent pains of tabes dorsalis. Relations.

Sífilis Congênita Introdução Sífilis é uma doença venérea sistêmica.Ela era chamda de “A grande impostora” já que existem várias fases clínicas (três) e cada uma se assemelha a uma doença diferente. Seu agente etiológico é o Treponema pallidum e a transmição se dá por contato sexual ou po transmissão congênita. Todas as fases tem em comum a endarterite obliterante com infiltrado composto por plasmócitos. Essas lesões ocorrem pela ligação da espiroqueta às células endoteliais , mediada por moléculas de fibronectina.Ocorre participação da imunidade celular,no início para reduzir a lesão, e humoral sendo que anticorpos fazem reações cruzadas com células endoteliais causando a endarterite característica. Existem três fases distintas na sífilis: 1ª fase: •Lesões generalizadas pela espiroqueta •Aparece após três semanas do contato ,que corresponde ao período de latência). •Cancro (com ou sem tratamento ela some) • Dura de 2 a10 minutos 2ª fase: •Caracterizada por lesões cutâneas e mucosa e por linfadenopatia •Permanece por vários anos 3ª fase: •Lesões localizadas representadas por neurosífilis,aortite e gomas

DEGENERATIVE ARTHROSIS (Osteoathrosis) Osteoarthrosis is a degenerative condition of the joints. As there is no iflammatory process, the earlier terminology of osteoarthiritis is no longer used. Osteoarthrosis cen be broadly grouped as follows: 1. Primary osteoarthrosis when there is no previous pathology. 2. Secondary osteoarthrosis when it is secondary to some previous pathology. Primary osteoarthrosis is due to the wear and tear changes occuring in old age in which weight bearing joints like the hips and knees are more commonly affected. It is uncommon in non-weight bearing joints like the shoulder and elbow. Obesity is a predisposing factor. Osteroarthrosis is a progressive process affecting the articular cartilage of aging joints. It is characterised by focal degeneration of the articular cartilage. As the articular cartilage is cyclically loaded during movements of joints, it undergoes fatigue failure leading to framentation of the surface and fibrillation. In the later stage, the cartilage gets completely eroded, exosing the sclerosed (eburnated) bone and subchondral cysts are also formed. The bone undergoes reactive hypertrophy forming peripheral osteophytes. The synovial membrance undergoes hyperemia and reactive inflammatory thickening. As there is no destructive pathology, the joint does not get ankylosed.

Det finns ett antal stora sjukdomar och det finns också tusentals "små" sjukdomar, som bara förekommer i några enstaka fall, vilket gör det hela lite komplicerat. En av de första observationer som kan sägas ha ett vetenskapligt värde kom redan 1658. Det var Johan Jakob Wepfer som i Basel publicerade ett arbete där han beskrev att slaganfall, apoplexi, orsakas av en blödning från ett hjärnkärl. Det var ingen som hade funderat över det tidigare. Fortfarande på 1600-talet fanns i England och London en "medicinens gigant", Thomas Sydenham. Trots de små resurser som fanns på den tiden beskrev han bl.a. det som kallas efter honom själv Sydenhams chorea. Chorea betyder danssjuka och det är den danssjuka man kan få som komplikation till reumatisk feber om man är barn, förr i tiden såg man detta ofta, men det är helt borta idag. Charcot blev chefsläkare 1862 och han inredde ett laboratorium för mikroskopi, fotografering och "artistisk anatomi". Han hade ett litet rum för prover m m. Det lilla rummet kallade hans medarbetare för "La future de la neurologi", neurologins framtid. Under några ytterst intensiva och fruktbärande decennier kom han och hans medarbetare att beskriva många av de neurologiska sjukdomar vi känner till idag, avgränsa dem, beskriva symtomen. Många gånger visste man inte vad sjukdomen berodde på men det var ett omfattande arbete grundat på noggrann observation och skärpa.
