- See: Vascular Problems of the Upper Extremity - Discussion: - most often affects subclavian artery , vein, and lower trunk ( ) of brachial plexus ; - both the subclavian artery and the brachial plexus traverse between the anterior and middle scalene muscles ; - most symptoms arise from neural compression; - compression may occur at the junction of the lower trunk and 1st rib; - in some cases, thoracic outlet syndrome will be accentuated by recurrent anterior shoulder instability , and this may be the cause of the "dead arm syndrome;" - Exam: neuro exam: - compression of the inferior trunk ( ) is seen most often; - anterior division of inferior cord forms medial cord which gives rise to the median ulnar nerves ; - sensory changes in the ring and little finger; - intrinsic weakness; - can mimic symptoms of cubital tunnel syndrome ; - upper extremity edema: may indicate effort thrombosis of axillary vein; - extinction of pulses: - radial pulse obliteration is not by itself specific, but loss of pulse with reproduction of symptoms is a positive test; - decreases interscalene space by tensing anterior and middle scalenes; - arm of the affected side placed on the sitting patients thigh with forearm supinated; - turn head to the affected side; - extend neck and hold breath; - positive test is obliteration of the radial pulse; -

WHAT IS IT?
Thoracic outlet syndrome is a repetitive stress disorder that involves the shoulder and upper arm. WHAT CAUSES IT?
The thoracic outlet is a triangular area through which nerves and blood vessels pass from the neck to the arm. The area is bordered by the collar bone, the first rib, and the anterior and middle scalene muscles. Thoracic outlet syndrome occurs when the nerves and blood vessels between the neck and shoulder are compressed. Activities such as pulling your shoulders back and down (an exaggerated military stance), sleeping with your arms above your head, carrying items such as a stretcher, backpack, or suitcase or work that requires frequent overhead lifting may increase the risk of this syndrome. SYMPTOMS:
Symptoms are similar to carpal tunnel syndrome - numbness in the fingers and hand, tingling in the arm as if it is "asleep", and pain that begins in the base of the neck and radiates into the arm or hand.

THORACIC OUTLET SYNDROME
Introduction
Thoracic Outlet Syndrome (TOS) is a condition affecting the shoulder, arm, and hand. This condition is a very frustrating problem - both for the patient and for the physician. It is extremely difficult to prove that the diagnosis of Thoracic Outlet Syndrome is correct, because there is no test that has a high degree of accuracy in showing the problem. Usually, the diagnosis is made after all other causes of the symptoms have been ruled out - a frustrating and slow process sometimes!
Anatomy
The nerves and blood vessels that run into the arm and hand start at the side of the neck. They exit from the side of the spine through small openings between each vertebra called foramen . As they leave the spine the nerves are referred to as nerve roots . The individual spinal nerve roots then begin to join together to form the nerves that will run into the arm and hand. The nerves travel between two muscles in the neck (the scalene muscles), over the top of the rib cage, under the collar bone (clavicle), through the arm pit (axilla) and down the arm to the hand. The area where the nerves and vessels leave the neck between the two scalene muscles and over the first rib is know as the Thoracic Outlet.
Causes
There are probably several causes of TOS. The common underlying cause of the syndrome is compression of the nerves and arteries of the arm in the

CASE REPORT Role of Acupuncture in Thoracic Outlet Syndrome
Dr. M Tariq, Dr. Jawad Zahir and Dr. Salman Ahmad Saleem Abstract This report describes a case of Thoracic Outlet Syndrome (TOS) which was successfully treated with acupuncture after failed conservative management with a variety of interventions. Webmaster's Note: This is a brief summary of this case report, the full text of which is available in the paper and electronic versions of the Journal. A 21 year old woman had suffered with pain in the neck, scapular region and chest, with brachialgia and with paraesthesia of the left hand, for three and a half years. She had been treated without any lasting relief with a variety of modalities, including NSAIDs, opiods, physiotherapy, short wave diathermy, ultrasound and nerve block therapy. She was treated daily with electroacupuncture to points ST38, LI4, LI11, LI15, and TE14, for 10 days, with some improvement. She was then given three further week-long courses, at fortnightly intervals.

Home Richard Merbler Rolfing ... Useful Links Both are usually caused by repetitive movement and incorrect positioning of the head, neck, shoulder, arm and hand. Some good examples are:
  • Holding the phone to shoulder w/ head. Holding a violin to your shoulder with head.
  • The symptoms are:
    • Pain around clavicle (collar bone between shoulder and neck) Sometimes pain with movement of shoulder or head Headaches Sometimes pain radiates from neck or shoulder all the way down the arm and maybe fingers
    Usually, the connective tissue has shortened, thickened around a bundle of nerves (bracio-plexis) as a result of repetitive movement. The conventional treatment is surgery on the connective tissue that is pinching or entrapping a nerve. Usually, it is surgically cut. Rolfing can stretch the connective tissue and free the nerve that is pinched. The result is no more pain.

    DOUBLE CRUSH SYNDROME RELATED TO THE THORACIC OUTLET SYNDROME
    Jun Nishida , Katsuaki Ichinohe, Tadashi Shimamura and Masataka Abe Department of Orthopaedic Surgery, School of Medicine, Iwate Medical University, Japan ABSTRACT Cases diagnosed as having thoracic outlet syndrome were examined by neurological examination, including provocation tests, electromyography and radiological examinations, to detect other sites of entrapment neuropathy of the upper extremity. During the last four years, 555 upper extremities of 494 patients were diagnosed as having thoracic outlet syndrome. Forty-five patients (9.3%) were diagnosed as having other entrapment neuropathy in one extremity. Ten cases were complicated by cervical radiculopathy, 15 by carpal tunnel syndrome, 11 by cubital tunnel syndrome, five by radial tunnel syndrome, two by ulnar tunnel syndrome, two by both carpal tunnel and cubital tunnel syndrome, and one by both cubital and ulnar tunnel syndrome.
    Surgery was performed for 15 limbs, and the distal lesion was operated on the first in two­thirds of these patients.
