    SE-171 76 STOCKHOLM Yngre människor med perifera cirkulationsstörningar i händer och fötter är en patientgrupp som ofta förbryllar läkare. Tyvärr går dessa patienter ofta missförstådda genom sjukvården med ibland katastrofala följder såsom perifera gangrän och amputation som följd. En diagnos som alltid måste finnas med i den differentialdiagnostiska arsenalen hos dessa patienter är morbus Buerger (thrombangitis obliterans). Denna sjukdom har i Sverige varit relativt sällsynt men genom den relativt stora invandringen från länder kring östra Medelhavet har patienter med sjukdomen blivit alltmer frekvent. Med anledning av de mycket allvarliga konsekvenser som kan bli följden av att felbedöma dessa patienter ser jag det som angeläget att åter (1, 2) redogöra för det syndromkomplex som sjukdomen omfattar. Hur ställs diagnosen morbus Buerger?

    The affected person must stop smoking if the affected extremity is to be saved. Cold temperatures and other conditions that reduce circulation to the extremities should be avoided. Attempts to increase circulation may include warmth and gentle exercise
    Surgical sympathectomy (cutting the nerves to the area) may help control pain. Aspirin and vasodilators may also used. Amputation of the extremity may be necessary if infection or extensive tissue death occurs.
    Expectations (prognosis): Symptoms of thromboangiitis obliterans may disappear if the person stops tobacco use . For some, amputation is unavoidable.
    Complications:
    • Gangrene (tissue death) Loss of circulation beyond the affected extremity Amputation

    Background Panniculitis is inflammation of the subcutaneous fat. Fat is divided into lobules by connective tissue septae. These septae contain the blood supply supplying the lobule. A feeder arteriole supplies the center of the lobule while venules drain the septae. Disorders which disrupt the arterial supply lead to a lobular panniculitis while venous disorders lead to a septal panniculitis. The panniculitides are usually classifed by histopathologic findings of septal and lobular. However, in many of these conditions, there may be a mixture of patterns. In addition, there are numerous conditions which may cause a secondary panniculitis. Large vessel vasculitides such as polyarteritis nodosa are a common cause of such changes, usually a lobular pattern. Necrobiosis lipoidica and scleroderma may cause a secondary septal panniculitis. Once the predominant pattern is decided, a determination of the predominant type of inflammatory cells should follow. Finally, any special changes such as crystals or a unique form of necrosis, or vasculitis, should be determined.     PERIPHERAL BLOOD VESSEL DISEASES There are two major categories of blood vessel diseases: peripheral arterial diseases, which are disorders of the vessels carrying blood from the heart to all parts of the body, and peripheral venous diseases, which are disorders of the vessels carrying deoxygenated blood back to the heart.     DEFINITION

    Arterial disease may result from obstruction, which impedes the flow of blood; from disorders of the arterial muscles, causing them to either constrict or dilate; or from aneurysms, weakened vessel segments that fill with blood and balloon outward. As discussed earlier, the formation of atherosclerotic, or fatty, deposits along the inner arterial walls is the most common arterial disease. When the coronary arteries become seriously obstructed by these deposits, a heart attack or symptoms of coronary disease are an all too common result; when arteries supplying blood to the brain are blocked, a stroke may ensue. Similarly, arteries supplying other parts of the body also may become partially or fully obstructed, leading to a condition referred to as chronic occlusive arterial disease. This reduces the availability of blood to the tissues, causing pain and eventually damaging or destroying the tissues, and making them more prone to infections and gangrene.

Vascular Endothelial Growth Factor (VEGF) and Basic Fibroblast Growth Factor (bFGF), the main growth factors involved in the process of angiogenesis, appear to act by local upregulation of nitric oxide production and exhibit some synergism in their actions . Hypercholesterolemia impairs angiogenesis by inhibiting the actions of nitric oxide. The beneficial effect of laser myocardial revascularisation ( PMR or TMR) in relieving ischemia is also mediated through stimulation of angiogenesis . Promotion of angiogenesis with intraarterial injection of naked VEGF cDNA to relieve ischemia in atherosclerotic peripheral vascular disease was the first successful application of these research advances . Injection of VEGF cDNA directly into ischemic tissues has also been used successfully to reduce ischemia in both thromboangiitis obliterans and coronary artery disease . Intracoronary injection of purified bFGF has also been used to reduce ischemia in coronary disease . Both of these techniques have some potential disadvantages as direct injection into ischemic myocardium requires a limited thoracotomy while intracoronary injection means a most of injected material will not flow through the ischemic area. Other methods of delivery such as intrapericardial injection have had limited success prompting a search for the optimum way to deliver these potent therapeutic compounds. The recent identification of bone marrow progenitor cells which transform into endothelial cells and localise into the angiogenic network and augment growth factor stimulated angiogenesis provide further possibilities for this promising therapy

    Buerger's Disease: It's Not from Eating Hamburgers J anet Smith came to our clinic with a sore on her left foot that after several months had not healed. Before she had the sore, Janet remembered feeling pain in the instep of her foot when walking. Her pain had become steadily worse, and eventually became more constant, occurring at night while lying in bed as well as when walking. Pain in the leg or foot when exercising indicated that she had intermittent claudication, a symptom that is common in persons with peripheral arterial disease (PAD). The development of pain at rest and sores were indicative of critical limb ischemia (CLI) (see page 2), which indicated that her vascular disease was quite advanced.
