HealthScoutNews Reporter SUNDAY, June 24 (HealthScoutNews) Don't be too quick to blame a lousy lover or too much tequila if your last tango between the sheets was just plain forgettable. You might simply be suffering from a rare and Bill Clinton jokes aside very real case of sexual amnesia. Doctors say it's a version of TGA, short for transient global amnesia. It strikes during or right after sexual intercourse, usually wiping out memory for six hours or more. For better or for worse, that whole time period is lost forever. "It's an extremely scary event," says Dr. Lawrence B. Gardner, a hematologist at the Johns Hopkins Medical School in Baltimore who wrote about the baffling phenomenon in The Lancet , the British medical journal. Gardner and his colleague, Dr. Chi V. Dang, saw two patients, both men in their 70s, who were stricken after sexual intercourse with their wives. Both victims were otherwise healthy. Within 30 minutes of intercourse, however, both became so disoriented and confused that they couldn't even identify the president of the United States. One, a 75-year-old retired physician, babbled incoherently and wasn't back to normal for 15 hours. Even then, he couldn't recall anything that had happened in the six hours after having sex.

Methods: Three patients with TGA were given mini-mental state examination (MMSE), revised Wechsler memory scale (WMS-R) examination and MRI scan when they had been diagnosed as TGA. Using F labelled deoxyglucose as tracer, the patients were given positron emission tomography (PET) examination at the different periods after recovery. Results: No obvious abnormality was found in MMSE and MRI scan of the three patients. However, WMS-R examination and cerebral PET imaging displayed cognitive dysfunction in various degrees and low metabolism in local areas related with memory in 2 of 3 patients. Conclusions : In TGA patients, the cognitive function and cerebral metabolic levels are closely correlated with duration of symptoms. It is necessary to prevent the TGA attack in early time.

Memory loss
Definition: Memory loss is defined as a loss of memory resulting from brain damage (disease or trauma) or severe emotional trauma.
Alternative Names: Forgetfulness; Amnesia; Impaired memory; Loss of memory
Considerations: Whether amnesia comes on suddenly or slowly, and is permanent or temporary, depends on the cause.
Normal aging may result in trouble learning new material or requiring longer time to recall learned material (it does not lead to memory loss unless diseases are involved).

    Significance of the Sleep and Awake EEG Electroencephalogram (EEG) is one of the common neurology diagnostic tests ordered frequently, but quite a significant portion of EEGs are done only in the awake state. This essay explains the better value of sleep and awake EEG study. Other than epileptic syndrome, sleep and awake EEG studies are useful in other neurological syndromes. It is known that EEG abnormality is reported in a patient who has migraine headache. The rate of abnormality is varied and clinical significance is still under debate. Most of the awake EEG studies for migraine headaches are normal, but sometimes you may find 6 Hz positive spikes or 6 Hz phantom spikes during the sleep state, in which case addition of low dose of anticonvulsant helps the headache control. EEG abnormality seems more correlating to the migraine headache of the younger generation. If the EEG is done in the middle of the severe migraine attack, it occasionally demonstrates slow rhythmic activity pattern or even PLEDs. These findings are interpreted as manifestation of the transient subcortical gray matter dysfunction relating to the blood flow change during the migraine attack rather than actual structural abnormalities. Transient global amnesia sometimes needs to be differentiated from complex partial seizure attack. Those patients who carry the diagnosis of transient global amnesia attack, having more frequent recurrence with shorter duration, less than 30 minutes, require re-evaluation to rule out partial complex seizure disorder. In such situations, single awake state EEG may not find any abnormalities. These cases definitely require sleep and awake EEG study.

What exactly is amnesia?
A popular subject on the big screen and television (especially the daytime variety) is a form of memory loss known as amnesia . Stedman's Medical Dictionary defines amnesia as "a disturbance in the memory of information stored in long-term memory , in contrast to short-term memory , manifested by total or partial inability to recall past experiences." Amnesia is a condition in which someone can't recall stored memories, like their mom's maiden name or what happened last Christmas , but they may recall the knock-knock joke their little brother told them a few seconds ago. HISTORY OF PRESENT ILLNESS (by daughters, old medical records and patient): The patient is a 61-year-old, right handed female with a past medical history of hypertension, diabetes mellitus, increased cholesterol, hypothyroidism, nephrotic syndrome, and rheumatoid arthritis. Smokes three packs per day. Two days prior to admission to Lahey Clinic, the patient woke up and apparently seemed normal. She prepared the breakfast and suddenly told her daughters she was looking for the tools to repair computers. However, "she even doesn't know how to turn a computer on". Also, she started talking about events that didn’t happen and couldn’t remember recent events. She could not name her daughters when she saw them, only name them after they spoke. When asked direct questions she would say: "Of course, I know that…" and would try to change the subject, laugh or would make up some story. Some elements of the stories would be based in something she heard earlier or saw on TV. Also she would stay silent unless questioned by the family. "a big personality change- usually she talks a lot".

MOGADON® Roche Nitrazepam Hypnotic - Anticonvulsant Action And Clinical Pharmacology: Nitrazepam is a benzodiazepine with hypnotic and anticonvulsant properties.
In sleep laboratory studies, nitrazepam decreased sleep latency, increased total sleep time and decreased awake time. There is delay in the onset, and decrease in the duration of REM sleep. Nitrazepam is reported to significantly decrease stage 1, 3 and 4 sleep and to increase stage 2. Following discontinuation of the drug, REM sleep rebound has been reported in some studies.
Nitrazepam has been shown to raise the seizure threshold.
Nitrazepam has an intermediate half-life.
Rebound Insomnia: A transient syndrome whereby the symptoms that led to treatment with a benzodiazepine recur in an enhanced form may occur on withdrawal of hypnotic treatment.
Pharmacokinetics: Nitrazepam is rapidly absorbed from the gastrointestinal tract. Bioavailability after an oral dose averages about 80%. Peak blood concentrations after oral administration are observed in approximately 3 hours.
Following the administration of single oral doses of 5 or 10 mg nitrazepam to healthy volunteers, mean peak plasma concentrations ranged between 23 to 66 ng/mL and 55 to 107 ng/mL, respectively. In elderly patients suffering from various debilitating diseases, a mean peak plasma concentration of 22 ng/mL was observed after a single dose of 5 mg nitrazepam. Steady-state plasma concentrations following administration of 5 mg nitrazepam once daily were reached after approximately 4 days. Steady-state plasma concentrations of nitrazepam were approximately 40 ng/mL.

This story is for anyone who's ever had sex and later wished they could forget the whole experience. Rae Fry reports. Rae Fry: The man who forgot his sex. It's not a headline from the gutter press, it's a story from the Journal Neurology, Neurosurgery and Psychiatry. London neurologist Dr Russell Lane recently reported the case of a man who suffered amnesia while making love. His wife made him go to see Dr Lane because she was concerned by his behaviour. The patient was less concerned. He couldn't remember what had happened. Dr Lane told me he's never seen a case quite like this before. Dr Lane: Yes, this is a very curious case. This gentleman who was in his early 60's had suffered five quite stereotyped attacks, occurring exclusively with sexual intercourse. And these attacks comprised a change in behaviour in that the patient seemed a little confused, and in particular clearly amnesic for ongoing events. For example he would repeatedly ask his wife questions such as - what am I doing here, what are we doing, where are we, and so forth. And he would ask these questions over and over again even when given the answers. At the end of an event he returned entirely to normal, with the exception that he will never recall that particular event during the attack. In other words, during that episode new memory traces are not laid down and can never be recalled subsequently. It's just that if you like, the tape recorder is switched off temporarily.
