Non-gonococcal urethritis (NGU), also known as non-specific urethritis (NSU) is one of the most common sexually transmissible diseases. It can be caused by a number of different organisms, the most significant one being chlamydia. NGU not due to chlamydia may be due to exposure to normal vaginal or throat organisms.
What are the symptoms?
Men may experience a urethral discharge, pain when urinating, or urethral irritation. These symptoms may be more noticeable in the morning. Symptoms may develop between one and five weeks after the likely acquisition of infection. Some men are unaware of any signs or symptoms.

The urethra is the tube between the bladder and the end of the penis. Urethritis means inflammation of the urethra. What are the causes of urethritis? Urethritis is usually caused by a sexually transmitted infection. It is classed into two types.
  • Gonococcal urethritis if the cause is gonorrhoea. This is one type of sexually transmitted infection. A separate leaflet describes gonorrhoea in more detail.
  • Non-gonococcal urethritis (NGU) if it is caused by anything other than gonorrhoea. This is sometimes called non-specific urethritis (NSU). A bacteria called chlamydia is the common cause of NGU. This is one type of sexually transmitted infection. NGU can also be caused by a variety of other bacteria or viruses (bugs). NGU may rarely have a non-infective cause. For example, injury or surgery to the urethra can cause inflammation. A separate leaflet describes NGU in more detail.

Some experience a discharge accompanied by painful urination, and gonorrhea may be the first diagnosis. NSU does not respond to penicillin and therefore anyone who suspects that they may have it should be tested immediately. What are the symptoms of nonspecific urethritis? Urethritis is a condition that can occur in both men and women. The urethra is the tube that starts at the base of the bladder through which the urine flows out of the body. In the male the urethra passes through the prostate gland and then travels through the penis emptying at the tip of the penis. In women the urethra is very short only 2.5 to 5.5 centimeters. The urethra in both men and women is basically the same except for its length. The urethra is lined by epithelial (special skin cells) and has many glands that empty into it. It is in these glands that bacteria accumulate and can cause infection which leads to what we call urethritis.

The Causes
The Symptoms
Initially the symptoms may be tingling and burning within the urethra. These symptoms can progress to frank pain and discharge. The discharge associated with nonspecific urethritis is usually watery but it can also be pus-like (thick solid color, grey, green, yellow) or bloody. There may be very either little discharge or copious discharge. In men, the discharge may stain their underwear and in women, the discharge is often confused with vaginal discharge. Pain can range from slight to severe and it may occur only at urination or continuously.
The Diagnosis
Treatment
Treatment for chlamydia trachomatis is generally tetracycline or erythromycin 500 mg. every 6 hours for 10 to 14 days. For NSU alone Tetracycline 500 mg. every 6 hours for 10 days is usually more than adequate. For trichomonas metronidazole (Flagyl) either 2 grams as a single dose at bed time or 500 mg every 8 hours for 5 to 7 days may be used. When candida exists, oral antimonilia agents are used but treatment may require a Urologic evaluation as strictures, chemical irritations or possible tumors may exist. Not infrequently after treatment for gonorrhea a NSU may develop. A course of tetracycline is usually prescribed at this point. Generally, partners should always be treated for gonorrhea, chlamydia, trichomonas and candida. There is no specific curative treatment for herpes but Zovirax (acyclovar) can be used to limit and prevent subsequent attacks.

What is Urethritis?
Urethritis is inflammation of the urethra (the passage in the penis that urine and semen pass through). It can be caused by chlamydia or gonorrhea, or other bacteria. Frequently, however, the exact cause is never identified. There are two types of urethritis 1) those with a known cause for example chlamydia and/or gonorrhea, 2) Non-specific Urethritis. 3) Urethral Irritation. Urethritis can also be recurrent or persistent and on occasion may reoccur after treatment. Each of these subjects are covered below.
What are the Symptoms of Urethritis in General?
Often there are no symptoms at all; but when symptoms are present, they may include:
  • Discharge (milky or pus-like) from the penis Stinging or burning during urination Itching, tingling, burning or irritation inside the penis.
How is Urethritis Diagnosed?
The most important test is a culture of the urethra, a swab is gently inserted into the urethra and a culture is taken. The swab is then examined in the laboratory for signs of infection. Swabs should be taken 3 or 4 hours after you last passed urine. It usually takes several days for final chlamydia and gonorrhea culture results to become available.
Non Specific Urethritis (NSU)
When the cultures are negative gonorrhea or chlamydia, the urethritis is said to be a Non-Specific Urethritis, or NSU. The specific cause of NSU is generally not known, however antibiotic treatment is effective. Although NSU does not cause symptoms in women, female partners of men with NSU may need to be examined and even, on occasion, treated.

Causes, incidence, and risk factors: Urethritis may be caused by the same organisms that cause urinary tract infections (for example, E. coli or klebsiella) and by some sexually transmitted diseases (for example, chlamydia gonorrhea , and Ureaplasma urealyticum infections). Viral causes of urethritis include herpes simplex virus and cytomegalovirus. Urethritis may also develop because of a chemical irritation (such as spermacide in condoms or contraceptive jelly, cream, or foam). Trauma may also cause urethritis. Men between the ages of 20 and 35, those with multiple sexual partners, and those who engage in high-risk sexual behavior (such as lack of condom use, particularly during active anal intercourse) are at higher risk. Young women in their reproductive years are also at risk. In women, common bacterial causes include gonorrhea, chlamydia, mycoplasma hominis ureaplasma urealyticum , and mycoplasma genitalum . NSU can be caught during sex, including oral and anal sex. You CAN'T get it from hugging, kissing, toilet seats or sharing towels. However, it can reoccur in people who haven't been having sex for a while - we don't understand why this is (we call this recurrent NSU). What are the symptoms of NSU?

Nonspecific Urethritis , known as NSU , is an inflammation of the urethra. In men the symptoms include soreness after urinating, discharge and painful intercourse. In women the symptoms are less noticeable, and might be just a heavier than normal vaginal discharge, or there may be no symptoms at all. However, because it can be symptom-less in women, it is potentially dangerous as it can cause infertility through scarring of the fallopian tubes. If a man has NSU and his female partner is without symptoms, she must be treated as well because of the aforementioned problem of potential infertility. NSU is usually caused by chlamydia and treated quite easily in both sexes by taking antibiotics. Complementary therapies focus on boosting immunity. It's commonly caused by sex with a partner who already has a sexually transmitted infection. NSU doesn't usually cause serious health problems. It's easily treated with antibiotics. NSU is usually caused by vaginal, oral or anal sex. It's called 'non-specific' as there are a variety of infections that can cause it:
    • Chlamydia is the most common cause of NSU. Chlamydia is a very common bacterial sexually transmitted infection (STI) and is easily transmitted.
    • Other STIs can also cause NSU.
    • It can also be caused by other genital or urinary tract infections.
    • The urethra itself is very delicate and can be damaged through vigorous sex or masturbation - this can cause NSU.
