"I am a 58 year old man and have suffered what my dermatologist says could be hives. It first occurred about a year ago and I have no idea what causes its onset. Per reading various Internet sites, the medical field doesn't know either. The cause could be most anything."

Urticaria are raised, red, itchy areas of the skin which are commonly known as hives, wheals or welts. The quantity, color, and shapes of hives can vary. The color can vary from light pink to dark red, depending on the amount of fluid build up. The shape of the wheals are usually round, although bizarre patterns occasionally develop. They can last from a few hours to several weeks or months.

Hives are formed by blood plasma leaking out of small blood vessels in the skin. This is caused by the release of a chemical called histamine. Histamine is released from cells called "mast cells" which lie along the blood vessels in the skin. Allergic reactions, chemicals in foods, or medications can cause histamine release. Sometimes it's impossible to find out why hives are forming. Hives are very common 10-20 percent of the population will have at least one episode in their lifetime. Hives usually go away within a few days to a few weeks. Occasionally, a person will continue to have hives for many years. When hives form around the eyes, lips or genitals, the tissue may swell excessively. Although frightening, the swelling usually goes away in less than 24 hours.

    External survey "Questionnaire on mood" conducted by the "IMSP- Faculty of Medicine - University of Geneva" Urticaria Hives Urticaria is a skin symptom that accompanies many allergic disorders . It is a relatively common disorder caused by localized mast-cell degranulation, with resultant dermal venular hyperpermeability culminating in pruritic wheals. Mostly results from an antigen-induced ( pollens foods drugs insect venom ) release of histamine and other vasoactive amines via sensitization with specific IgE antibodies. Most individual lesions develop and fade within 24 hours. Allergic skin diseases such as urticaria and angioedema as well as atopic dermatitis are not clearly defined as true allergic diseases. Urticaria and angioedema are both due to excessive mast cell activity in the skin. Chronic urticaria and angioedema are seldom due to a definable IgE mechanism (as is the case with all allergies) but, when determined, is generally due to a

    What is Urticaria? Urticaria also called nettle-rash or hives or weals in a common language, simply means itching with rash. Medically, urticaria may be defined as skin eruption which is allergic in origin and is characterized by profound itching, red circular or irregularly shaped eruption on any part of the body which has a tendency to remain for a variable period, having a tendency to disappear and reappear. It tends to disappear without leaving behind any trace.
    Urticaria is an allergic skin disorder. Characteristically the skin eruptions are erythematous, raised above the skin level, with intense itching and usually worse by itching, with slight local warmth. Location and duration: Well, urticaria may appear on any part of the skin. Angio-oedema is a condition when deep tissues are affected. The typical lesions may last for one minute to half an hour. Some may last longer. Some patients may get the eruptions once in a while, some may have them many times during the day. It may be restricted to a couple of spots in some patients while some may have them real wide spread rashes appearing for days or even months together.
    There are acute, subacute, chronic and recurring variants as far as the frequency and duration are concerned.

In UK about 1% of the population will suffer from urticaria during their lifetime. Out of this appox. 25 % of the patients would be of chronic urticaria. Chronic urticaria presents in a variety of presentation. About 5 % are shown to be Reactive to the food allergens . 5% may have urticarial vasculitis Physical urticaria is seen in about 35%-40 %, while aprox. 50 % have so called Idiopathic Urticaria Physical Urticaria - in this most striking is dermographism. A useful procedure in confirming the diagnosis of dermographism is challenge test with a caliberated dermographometer, which measures the pressure being applied to the skin. Other common physical urticarias are cold urticaria, cholinergic urticaria and solar urticaria. Cold urticaria can be tested by appliing ice to the skin., cholinergic urticaria can be seen by exercising the patient or bathing in warm water and solar urticaria can be tested by solar simulator lamp. Patients with physical urticaria need no firther investigations. The patients shold be informed of the condition for which the average time cource os 2-3 years. and during this time the main therapeutic approach is to cover the patient with H1 antihistamine. Very little is known about the pathogenesis of the physical urticaria.

Hives also occur as a feature of anaphylactic reactions. The itchy, erythematous welts, typical of acute urticaria can occur in any distribution in the body. Infants may develop facial wheals on contact with food. Adults often have a single bout of hives when they encounter an unusual or seasonal food or take a drug. Hives following a strawberry feed in the summer or after a eating prawns at a sea-food restaurant are typical examples.

    Urticaria and Angioedema
    Introduction
    Urticaria, often called "hives", refers to a skin eruption consisting of raised demarcated intensely itchy papules or weals. While urticaria involves dermal swelling, angioedema is due to oedema in the deeper subcutaneous and submucosal tissues, especially involving the palms of the hands, soles of the feet, head, and neck. Angioedema of the throat may cause respiratory obstruction in severe cases. Urticaria/angioedema has been arbitrarily defined as ‘Acute’ if it lasts less than 6 weeks, with more prolonged cases of relapsing skin swellings being labelled as ‘Chronic’. Up to 50% of cases of urticaria are associated with angioedema, and up to 25% of the population will develop one of these problems during their lifetime.
    Pathophysiology
    Tissues involved in urticaria/angioedema reveal dermal or subcutaneous oedema in association with a perivascular infiltrate of T-lymphocytes, macrophages, eosinophils and mast cells which may be sparse and fleeting in acute cases or more extensive in chronic urticaria. The swelling and itch seen in urticaria/angioedema is due to mast cell degranulation. The triggers include a variety of factors (Table 1). However, in up to 50% of cases of acute and chronic urticaria/angioedema, no cause is identified. Some physical urticarias involve structural modification of the IgE molecule by various physical factors, and are IgE-mediated.

    Urticaria (Hives)
    Urticaria is the medical name for hives. These are welts; pink swellings that come up on any part of the skin. They itch and each individual hive lasts a few hours before fading away, leaving no trace. New hives appear as old areas fade. They can be pea sized or join to cover broad areas of the body. While the itch can be intense, the skin is usually not scabbed or broken. In some people the hives burn or sting.
    Hives are very common with 10-20 percent of the population having at least one episode in their lifetime. Hives can sometimes occur in deeper tissues of the eyes, mouth, hands or genitals. These areas mat develop a swelling that is frightening in appearance, but usually goes away in less than 24 hours. This swelling is called angioedema.
    In many cases, a single attack of hives is due to an infection or virus and these go away within a few days to a few weeks. Some people get repeated attacks that occur as an allergic reaction to a variety of things (foods, most commonly nuts, chocolate, fish, tomatoes, eggs, fresh berries and milk, insect stings, and medications). In this case, they usually break out within a few hours of the exposure. Usually, the patients figure out the cause by themselves, and they never bother coming to a doctor.
    Certain people can develop recurrent hives from sunlight, cold, pressure, vibration or exercise. These are called the physical urticarias. If hives develop from scratching or firmly rubbing the skin it is called dermatographism. It is the most common of the physical urticarias and it affects about 5 percent of the population. It doesn't always itch. This condition sometimes also occurs along with other forms of hives.
