Some Practical Ideas

What is Usher Syndrome ? It is a condition which attacks some congenitally deaf people (people who have been born deaf), combining hearing loss with progressive loss of sight. This loss of sight occurs through deterioration of the retina known as retinitis pigmentosa (RP). The visual problems are usually only detected when the deaf persons reach the late teens or early twenties.
What is the incidence ? Figures from the USA and Scandinavia suggest that: 3-6% of all congenitally deaf people have Usher's, and 25% of people with RP have an accompanying hearing loss.
How does RP affect vision ? Through poor dark adaptation - i.e.: night blindness Through the narrowing of field vision at a later stage.
Detection Routine eye tests carried out at school examine only distance vision and cannot detect either dark adaptation or poor peripheral vision. Because of this, there is often a delay in diagnosing Usher Syndrome until the vision problems become more obvious.

Learning
All children learn about their world through their eyes, but for children who are deaf or hard of hearing, eyesight becomes critical in providing information about the world around them. Communicating with family members and friends, and understanding how things work are primarily learned through the use of sight.
Eye Problems
Some children who are deaf or hard of hearing will have eye problems that can impair their use of vision. Certain vision problems are more common in individuals who are deaf or hard of hearing. It is therefore critical that children who are deaf or hard of hearing have eye exams early in life and on a regular basis to identify any eye problems. Often these vision problems are correctable, but some are not. In either situation, early identification will help identify resources to prepare and educate people about possible future medical and educational needs these children will have as they grow older.
What To Do
Because vision is critical to children who are deaf or hard of hearing, parents need to have their child receive a medical eye exam as soon as possible after hearing loss is diagnosed.

What is Usher-Syndrome
Usher-Syndrome is named after a British doctor, Charles Usher, who first described the symptoms at the beginning of this century. It is the combination of two illnesses: deafness or hearing deficiency and the progressive degeneration of the retina (so called Retinitis Pigmentosa ("RP")). RP is a hereditary disorder whose common feature is a gradual deterioration of the light sensitive cells of the retina (the "rods and cones"). The first effects of the deterioration of the rods will usually be noticed between the ages of 10 and 15 when vision in darkness is gradually lost, eventually leading to night blindness. Between the ages of 15 and 20, the deterioration of the cones usually commences affecting the ability to see colours and outlines. Some individuals will also have balance problems.

Definition Usher's syndrome is a form of Retinitis Pigmentosa (RP), with associated deafness. Usher's syndrome is the cause of approximately 10% of all hereditary deafness, with deafness existing at birth or developing soon afterward. The eye symptoms of RP appear later, usually by the age of 10, and start with night blindness. Visual acuity continues to decrease as the child grows older, and may result in complete blindness by midlife. People with RP may develop other treatable eye diseases, such as glaucoma and cataract Symptoms Hearing loss Night blindness Loss of peripheral vision The symptoms described above may not necessarily mean that you have usher's syndrome or retinitis pigmentosa. However, if you experience one or more of these symptoms, contact your eye doctor for a complete exam. Treatment Currently, there is no specific treatment for persons with Usher's syndrome or RP. Genetic studies of RP are a significant factor in finding a cure or prevention for this disease. The U-M is performing research on genetic factors of RP.

Usher's Syndrome
When retinitis pigmentosa (RP) is associated with hearing loss, it is called Usher's Syndrome. There are two types of Usher's Syndrome. Usher Type 1: Early damage to the cochlea of the ear results in deafness which starts from birth. The retinitis also starts early with a progression from night blindness to tunnel vision within the first 10 years of life. Usher Type 2: These patients are more fortunate. The loss of hearing is less severe and the retinitis pigmentosa becomes more evident during the child's late teens and twenties. Unlike most patients with Ushers Type 1, Type 2 patients are more likely to benefit from hearing aids and lip reading. Important Statistics Up to 6% of people born deaf have Usher's Up to 8 % of RP patients have Usher's Autosomal recessive inheritance means you must have two of the recessive genes to have the disease.
