Rev Mex Cardiol RESUMEN Se presenta el caso clínico de varón de 53 años de edad con diagnóstico de enfermedad de Von Recklinghausen y cardiopatía isquémica con manifestación clínica de angina inestable de reciente inicio. Fue estudiado de manera integral por la presencia de isquemia en las pruebas inductoras. Se realizó cateterismo cardiaco con coronariografía y se encontró arteria coronaria única. Se hace énfasis en la baja frecuencia de estas patologías; al parecer no existe ningún informe publicado en la literatura sobre algún caso similar. Basto Alvarez HV, Mier y Terán Suárez J, Vázquez Rodríguez G, Samaniego Arvizu G, García Pérez V Enfermedad de Von Recklinghausen asociada a tumor del estroma gastrointestinal Salud Tab RESUMEN La neurofibromatosis clásica descrita por Von Recklinghausen en 1882, se hereda con un rasgo autosómico dominante y un alto grado de penetrancia, su incidencia es 1 de cada 2,500 a 3,300 nacidos vivos, presente en todas las razas, y con predominio masculino. Neurofibromatosis tipo 1, presenta alteraciones genéticas en el cromosoma 17 en la región 17q12 17q22. Sus manifestaciones clínicas más conocidas son: lesiones cutáneas pigmentadas ( "manchas café con leche"), neurofibromas externos e internos y hamartomas pigmentados del iris. El presente caso clínico corresponde a una mujer de 44 años de edad, quién exhibe las típicas características clínicas externas, y una neoplasia gástrica benigna la que se manifestó por hematemesis. La neoplasia se clasificó como tumor del estroma gastrointestinal (TEGI), al demostrar positividad al CD 34 con estudios de inmunohistoquimica. Het is een ziekte die vaak erfelijk is en die de huid en het zenuwstelsel aandoet. Typisch zijn de lichtbruine vlekken (zogenaamde "cafe-au-lait" vlekken oftwel "koffie-met-melk" vlekken) die vaak al vlak na de geboorte te zien zijn. Ook hebben patiënten vaak sproeten in de oksels of liezen en soms een soort kleine bultjes op de huid. Niet zichtbaar aan de buitenkant zijn dezelfde soort bultjes die ook aan de zenuwbanen, bloedvaten of in organen kunnen zitten. Ook kunnen er afwijkingen in de ogen, vaak aan de botten en hersenen bestaan. De afwijkingen aan de hersenen kunnen ernstige problemen veroorzaken zoals leerproblemen, hyperactiviteit, spraakstoornissen, epilepsie. De precieze oorzaak is onbekend, behalve dat er iets fout gaat in de uitrijping van de zenuwvezels/zenuwbanen lang voor de geboorte. Ook is de genetische code bekend van deze ziekte, maar dat doet niets af aan het feit dat er geen echte behandeling voor is. 'Wij denken dat u met de meeste vragen het beste terecht kunt bij de neuroloog of klinisch geneticus (iemand die alles afweet van erfelijkheid). Er is vast ook een patiëntenvereniging, waarvan de neuroloog u het adres geven kan De Ziekte van Recklinghausen is mede omdat deze ziekte niet zo bekend is, een hele vervelende ziekte. Zelfs op een medium als internet waar zo veel is te vinden, kun je niet veel over deze ziekte vinden. Vaak weten huisartsen en artsen in het algemeen ook niet veel over deze ziekte. Er zijn in Nederland ook niet zo veel mensen die deze ziekte hebben. Zelf heb ik ook de ziekte van Recklinghausen, en ik wil graag op deze manier wat ik weet over de ziekte aan je vertellen. Friedrich Daniel von Recklinghausen (1833-1910), ein deutscher Pathologe, wies nach, dass die Hauttumoren dieser Erkrankung aus Nerven- und Bindegewebszellen bestehen. Diese Erkenntnis schrieb er 1882 in der Festschrift "Über die multiplen Fibrome der Haut und ihre Beziehung zu den multiplen Neuromen" nieder und widmete diese seinem Lehrer Rudolf Virchow anlässlich des 25jährigen Bestehens des Berliner Pathologischen Institutes. Eigentlich war es eine Fehldiagnose, wodurch die Neurofibromatose Typ 1 populär wurde. Sie war fälschlicherweise dem Briten Joseph Merrick zugeschrieben worden, der von 1862 bis 1890 lebte und dessen Leidensweg als Elephantenmensch vielfach künstlerisch aufgearbeitet worden ist (Film "The elephant man" von David Lynch). Ursachen Die Ursache der Neurofibromatose Typ 1 sind Mutationen des NF1-Gens auf dem langen Arm von Chromosom 17 (17q11). Die NF1 ist eine autosomal dominante Erkrankung.
