In September, 1999, I was diagnosed with Vulvodynia after suffering for 2 months with intense swelling, pinching, tingling, and ripping sensations, 24 hours a day, 7 days a week. Before I was finally diagnosed I saw - one family practitioner, my then - current gynecologist, and finally the gynecologist that diagnosed me. After hearing, "yes you look swollen but it probably is just your detergent", and, "well I don't see any signs of a yeast infection, but I am going to give you some suppository yeast medication anyway" and after trying every prescribed Herpes medicine (even though I tested negative for the disease), I decided to comb the Internet for help. Through an Internet referral from a STD site, I found the doctor who diagnosed me. After the initial shock of the diagnosis wore off I thought to myself, “ Okay, we live in the information age, I will go to the Internet”.  I typed the name of this disease in the search engines. I thought I would find doctors and medicines and whatever I needed to help me cope with this disease.  However, I learned there was little to no information available.  I remember the pain I was in, the flares I had almost everyday for hours with no relief.  I was taking a low dose of anti-depressants, (to help dull the nerves so that the pain would lessen,) nothing was helping the flares (severe swelling) and with what little energy I had left after battling the pain all day long, I would get on the Internet and try (in vain) to find information on possible treatments.  The process was exhausting!

Vulvodynia is a medical term that means 'painful vulva'. It feels like a wicked yeast infection - only worse. Burning, itching and pain are experienced in the skin of the vaginal entryway and sometimes the labia or clitoris. The term can cover a wide variety of vulvar pain syndromes including various infections and skin disorders. Some cases of vulvodynia may be due to compression or disease of the pudendal nerve, one of the main nerves that relays sensation to and from the genitals. The term is also used to refer to vestibulitis or Vulvar Vestibulitis Syndrome (VVS), which is an inflammation of the vestibule, or opening into the vagina and the tissues immediately around the vaginal opening.
This condition is sometimes also called 'vestibular adenitis'. Symptoms of vulvodynia or vestibulitis can range in severity from mild to severe. In mild cases, a burning or stinging sensation is noted during intercourse or when tampons are inserted. Upon touching the area with a cotton swab, pain is felt when the hymen and inner vaginal lips are touched. There also may be redness and swelling. In severe cases, the pain can be agonizing and much of the vulvar can be reddened, swollen and very inflamed. Often the edges of the inner vaginal lips are very sensitive and the pain so severe that it makes walking difficult.

    1. What are vulvodynia and Vulvar Vestibulitis Syndrome? Vulvodynia is a medical term that means "painful vulva". The term can cover a wide variety of vulvar pain syndromes, including various infections and skin disorders. Vulvar pain syndromes have been written about in medical books since at least the late 1800s. In 1889, Dr. A.J.C. Skene wrote a "Treatise on the Diseases of Women" wherein he described a disorder that was characterized by an "excessive sensitivity" of the vulva. He stated that itching was absent, but when "the examining finger comes in contact with the hyperesthetic part, the patient complains of pain which is sometimes so great as to cause her to cry out". He treated this condition by surgically removing the affected area. However, he noted that this provided only temporary relief. In 1928, Dr. H.A. Kelly wrote about a condition characterized by "exquisitely sensitive deep-red spots on the mucosa of the hymenal ring" as a frequent source of painful intercourse. However, this condition appears to have then been ignored in medical journals until the early 1980s, when Dr. Edward Friedrich began reporting on it.

    Clinical Professor of Obstetrics and Gynecology, University of Wisconsin Medical School, Madison, Wisconsin, and Director of Women Now Health Care, Milwaukee, Wisconsin. Overview and Prevalence Vulvodynia is defined as chronic vulvar discomfort with a duration of at least three months. This discomfort can be expressed as pain, burning, itching, dyspareunia (painful sexual intercourse), stinging, rawness, or "irritation" of a constant or intermittent nature. The prevalence rate is not well-established, but an article by Martha Goetsch, M.D., (American Journal of Obstetrics and Gynecology, 1991) reported the prevalence rate of Vulvar Vestibulitis Syndrome to be 15% in her general gynecologic private practice. Early results from a survey by the National Vulvodynia Association found the mean age of women with vulvodynia to be 43. Age ranges have been reported from 11-75 years.

Victoria also has a common, but relatively unknown disease, one that consumes her life. It is a disease with no cure - a disease that until recently had no name. It is a malady so personal, that Victoria won't discuss it with her closest friends or relatives, yet one afflicting 20 million or more American woman. Victoria has "Vulvodynia" - a constant burning and irritation in the mouth of her vagina. She cannot wear pantyhose or jeans. She is extremely uncomfortable sitting or even standing for long periods. Victoria describes it as "'like a particularly painful and irritating yeast infection that never goes away." She's been forced to live with the pain and discomfort for years, because doctors at first misdiagnosed her condition, a too typical occurance, and then could not find anything to relieve her symptoms.     What is vulvodynia? Vulvodynia is a puzzling syndrome in which women feel chronic pain in the vulva, which consists of the external genitals (including the clitoris, the pubic mound, and the labia). Women with the condition describe the pain as a burning, itching, rawness, or stinging, particularly during urination. This vulvar discomfort ranges from annoying to unbearable, and can be continuous or set off by touch. Some women with severe forms of the ailment hurt even while walking or sitting. Because the pain is so excruciating, they may be unable to have intercourse or even wear tight pants or stockings. Experts say that vulvodynia, which literally means "vulvar pain," is more common than people think.     What is it?
    This condition is a cause of vulval burning and soreness, but is distinct from other conditions such as vulval vestibulitis. Dysaesthetic (dis-as-thet-ic) comes from the medical term dysaesthesia - which means an altered nerve sensation and vulvodynia is a non-specific symptom which literally means burning of the vulval area. Nerve fibres are widely distributed throughout skin all over the body and detect a number of sensations such as touch, pressure, temperature and pain. In dysaesthetic vulvodynia (DV) nerve fibres of the vulva are irritated or damaged and fire abnormal nerve signals back to the spinal cord where you interpret them as pain. This can occur even when the painful area of skin involved is not touched. Another example of nerve-type or 'neuropathic' pain like DV is the pain some people experience following an attack of shingles. Once the rash of shingles has disappeared, the area of skin where the rash was can often be intensely painful and sore despite the skin appearing normal. This condition is called 'post-herpetic neuralgia'.

    Vulvar Vestibulitus Syndrome
    (VVS)

    Refers to an inflammation of the tissues surrounding the opening of the vagina. Symptoms may include: burning around the vaginal opening, pelvic and/or vaginal pain, painful intercourse, pain upon insertion of a tampon, and painful urination. Affected tissues are usually reddened. The cause is usually unknown; some theories include: allergic drug reactions, over treatment of chronic yeast infections, chemical reactions to soaps/detergents. VVS may also be caused by compression of pudendal nerve, which supplies the genital area.
    Levator Ani Syndrome
    (LAS)
    A condition in which the muscles of the pelvic floor, which create a sling-like support to the vagina, rectum and bladder, are in a state of chronic spasm and tightness. This usually occurs in conjunction with a condition such as VVS, as a result of vulvar pain. The muscle tightness creates a poor healing environment for the vaginal tissues, because circulation is compromised as in any painful muscle guarding condition. Treatment of LAS is an essential component of the treatment of many pelvic and vaginal pain syndromes.

    These descriptions may be characteristics of a lasting pain in the area around the opening of your vagina (vulva) called vulvodynia (vul-vo-DIN-ee-uh) or chronic vulvar pain. How many women have vulvodynia isn’t known, but experts believe the condition is greatly underrecognized. This may be partly due to the absence of visible signs of the condition and the reluctance of many women to talk about their symptoms.     The word vulvodynia means "painful vulva." Your vulva consists of the pad of fatty tissue at the base of your abdomen (mons pubis), the labia, the clitoris and the opening of your vagina.
    The main symptom of vulvodynia is pain in your genital area, which can be characterized by:
    • Burning
    • Irritation
    • Itching
    • Stinging
    • Rawness
    • Painful intercourse (dyspareunia)
