The symptoms are of recurrent episodes of midline abdominal pain with attacks lasting 1-72 hours and complete normality between episodes. The pain is of moderate to severe intensity and is felt in the midline of the abdomen, usually around the umbilicus, or poorly localised. The attacks of pain are usually accompanied by anorexia and nausea and about half of the patients will vomit with at least some attacks. Marked pallor is commonly noted during the attacks although some patients may appear flushed. The pain is severe enough to interfere with normal daily activities and many children describe their mood during the attack as one of intense misery. The attacks are self limiting and resolve spontaneously and patients are completely well and symptom free between attacks. The onset of attacks of abdominal pain may be at any time of day but occurs most frequently first thing in the morning on waking. Associated symptoms include photophobia (sensitivity to light), phonophobia (sensitivity to sound) and dizziness in many children.

Cyclic Vomiting Syndrome is characterized by recurrent, prolonged attacks of severe nausea, vomiting and prostration with no apparent cause. Vomiting occurs at frequent intervals (5 -10 times an hour at the peak) for hours to 10 days (1-4 most commonly). The episodes tend to be similar to each other in symptoms and duration and are self-limited. The person is typically well between episodes. The onset of Cyclic Vomiting Syndrome (CVS) occurs in infancy through childhood and adulthood but most commonly between age 3-7. Cyclic Vomiting Syndrome can persist for months to decades. The person may be prone to motion sickness, and there is a often a family history of migraine.

Extractions: My ten-year-old daughter has been suffering from this since she was an infant. She had unexplained vomitting lasting 6-10 hours followed by a coma-like sleep occuring at six-week intervals with no symptoms the following day. This pattern continued until age 5 when she was correctly diagnosed. She has been hospitalized twice since diagnosis for dehydration caused by vomitting that did not cease after 36 hours, contrary to her usual episodes.She is currently being treated with 1 tsp.periactin daily and it is working fine. I have been told that as she matures, she will develop more migraine headaches and less of the abdominal variety. Is there any information about this transition and/or the severity and frequency of the migraines after puberty? Will she experience auras (she hasn't so far) and will the vomitting continue?

This variant most typically occurs in children. They usually have a family history of migraine and go on to develop typical migraine later in their life. The attacks are characterized by periodic bouts of abdominal pain lasting for about two hours. Along with the abdominal pain they may have other symptoms such as nausea and vomiting, flushing or pallor. Tests fail to reveal a cause for the pain. Occasionally there may be EEG findings suggestive of epilepsy but this is rarely related to seizures. Medications that are useful for treating migraine work to control these attacks in most children. top of page
