    Extractions: MassageToday.com One of the tasks of the Agency for Health Care Policy and Research (AHCPR) is to "facilitate the development of practice guidelines and measurements of quality care" (please see "Chiropractic Cost Comparisons Presented at AHCPR Conference" in the March 1, 1991 issue). The U.S. Congress created the AHCPR within the Department of Health and Human Services in an effort to improve health care and reduce costs. To that end, the agency seeks to ascertain what types of treatment are most effective in terms of costs and results. Congress has allocated the AHCPR a very large annual budget, part of which will go towards the sponsoring of research and panels that will develop practice guidelines. Ultimately, this endeavor will dictate what types of treatment will be acceptable and reimbursable for specific ailments. Fortunately, within the chiropractic profession, a few individuals and a number of chiropractic organizations are in-step with Congress in the development of practice guidelines. That is why the Conference for the Establishment of Guidelines for Chiropractic Quality Assurance and Standards of Practice (the Chiropractic Quality Assurance Conference) has been formed and sponsored by almost every major chiropractic organization in North America (please see "Chiropractic Practice Guidelines" in the July 19, 1991 issue). People with chronic back disorders, or CHRONIC PAIN, have extremely heightened activity of the normal stress response. Pain activates the hypothalamo pituitary adrenal axis (HPA) which is responsible for preparing the system for stress. Chronic activation of this system, due to the chronic pain, could actually result in depression. I find it IRONIC that doctors are so unwilling to treat the pain, giving too little pain prescriptions, but wind up giving anti-depressants for the depression which could have been prevented in the first place.

These included (1) heavy physical work, (2) lifting and forceful movements, (3) bending and twisting (awkward postures), (4) whole-body vibration (WBV), and (5) static work postures. Many of the studies addressed multiple work-related factors. All articles that addressed a particular workplace factor contributed to the information used to draw conclusions about that risk factor, regardless of whether results were positive or negative. The review provided evidence for a positive relationship between back disorder and heavy physical work, although risk estimates were more moderate than for lifting/forceful movements, awkward postures, and WBV. This was perhaps due to subjective and imprecise characterization of exposures. Evidence for dose-response was equivocal for this risk factor. There is strong evidence The review provided evidence that work-related awkward postures are associated with low-back disorders. Results were consistent in showing positive associations, with several risk estimates above three. Exposure-response relationships were demonstrated. Many of the studies adjusted for potential covariates and a few examined the simultaneous effects of other work-related physical factors. Again, it appeared that lifting and awkward postures both contribute to risk of low-back disorder.

Back aches are the most common reason for doctor visits, after cold and flu symptoms. Fifty percent of all patients who suffer from an episode of low back pain will have another occurrence within one year. In the vast majority of cases back pain is caused by the irritation of a nerve root near the spine, not by problems with the muscles, ligament or bone. A nerve that travels from the spinal cord through the openings between the bones of the spine gets pinched or irritated, the surrounding muscles tense up and the patient experiences low back pain. Low back pain is widespread in our society, but the good news is that in most instances the pain ends within a few days. More serious cases of back pain are treated with anti-inflammatory medication, physical therapy and muscle relaxants. Surgery, a common treatment a generation ago, is now considered necessary for only a very small percentage of back pain patients.
