Extractions: Erb's Palsy Erb's palsy refers to the most common form of brachial plexus injury. "Brachial" refers to the arm, and "plexus" refers to a network of nerves. The brachial plexus is a network of nerves which transmits signals from the cervical spinal cord to the shoulder, arm and hand. Damage to the nerves of the brachial plexus can cause partial or total paralysis of muscles in the shoulder, arm, or hand. Most brachial plexus injuries occur at birth . Approximately one or two babies out of every 1,000 born are affected. Nearly 90 percent of brachial plexus injuries resolve during the first year of life without the need for treatment. The remaining 10 percent of children require exercise, therapy, and, in the most severe cases, surgery. Even with the best treatment currently available, the most severely injured children will have substantial, lifelong functional limitations with regard to use of the affected arm and/or hand. Anatomy of the brachial plexus The brachial plexus is comprised of five spinal nerve roots that exit the spinal cord in the neck. These roots extend through the axilla (arm pit) behind the clavicle (collar bone), and then branch into nerves that enable movement (motor branches) and sensation (sensory branches) of the shoulder, arm, and hand.

A) Three month old with complete flail arm. Image demonstrates absence of plexus elements in area of trunks. A hypointense round (r) abnormality proved to be a mass of disrupted scarred scalene muscle partially incorporating shards of nerve remnants. In this situation, nerve grafts can be used to bridge gaps. In addition, neuroplasty or removal of adhesions helps improve recovery. B) Six month old child with persistent upper trunk paresis. Imaging confirmed ball neuromas at C5 and C6 demonstrating complete disruption of upper trunk elements. Early demonstration of complete disruption with terminal nerve neuromas can allow for an earlier decision for restorative surgery and hence better outcomes. HOME IMAGE HOME NERVE COMPRESSION HOME

Extractions: The brachial plexus is a network of nerves which conducts signals from the spine to the arm and hand. These signals cause the arm and hand muscles to move. (Brachial means arm, and plexus refers to a network of nerves.) Injury to nerves of the brachial plexus can result in full to partial paralysis of one or both arms. The United Brachial Plexus Network (UBPN) is a registered non-profit 501(c)3 organization devoted to providing information, support and leadership for families and those concerned with brachial plexus injuries worldwide. West discovered an avid support group online, with parents throughout the world sharing information about the injury.

SHOULDER DYSTOCIA: DIAGNOSIS, MANAGEMENT AND RISK PREVENTION Some recognized risk factors for this brachial plexus palsy are fetal Macrosomia, abnormal labor, midpelvic operative delivery, maternal obesity and maternal http://www.attorneymedmal.com/shoulder.htm
Extractions: Nothing can reduce a grown man to tears faster than a shoulder dystocia! Shoulder entrapment during delivery is a true obstetric emergency that can result in significant material and infant trauma. The maternal and fetal consequences of shoulder dystocia can be devastating. Infant complication occurs in up to 20 percent of cases and includes traumatic brachial plexus injury, humeral fracture and clavicular fracture. The incidence of severe birth asphyxia and related complication is 143 per 1,000 births for delivery with SD as compared with 14 per 1,000 births for all deliveries. I. Diagnosis : usually made only after the head has been delivered. Certain signs then appear: Definite recoil of the head back against the perineum (turtle sign). Because of the friction with the vulva, the head seems incapable of movement The problem is usually recognized when traction from below and pressure from above fail to deliver the newborn Vaginal examination is made to rule out other cause of difficulty, such as:
