Extractions: TREATING INOPERABLE CIRCULATION DISORDERS In November 2001, Paul S. van Bemmelen, MD, PhD , clinical assistant professor of surgery, and fellow members of our vascular surgery division published an article in the Archives of Surgery describing the clinical study of a nonsurgical treatment of inoperable circulation disorders. This novel treatment is good news for patients with poor circulation who are not good surgical candidates. The first author of the report, Dr. van Bemmelen is a pioneer in the use of external intermittent compression devices for treating circulation disorders in patients for whom surgery is not possible. Good news for patients with poor circulation who are not good surgical candidates Dr. van Bemmelen is the inventor of the patented arterial assist device called ArtAssist , which is currently used throughout the world. He has the longest clinical experience with the use of this device for the treatment of rest-pain, necrosis (black discoloration) of parts of the foot, and intermittent claudication (walking difficulty). Typical reasons to use this type of treatment are: advanced age and poor medical condition, multiple previous bypass attempts, lack of veins that could be used to make a new bypass, and lack of distal outflow arteries in the foot to which to connect a bypass.

Even diabetic patients who are frequently affected by circulation disturbances of the legs are rarely troubled by occlusion of the upper extremity arteries. The reason for this sparing of the upper extremities from arterial problems is not entirely clear but it is a well documented observation. Traumatic disorders of the arteries of the upper extremities can lead to severe circulation disorders however. I have had occasion to treat two major league baseball pitchers with aneurysms of the upper extremity arteries(1) and a tennis player with dissection (a tear of the inner lining of the artery resulting in occlusion of the vessel) of the major artery in the arm (2). In spite of the relatively infrequent occurrence of arterial circulation disorders of the upper extremities, the diagnosis must be considered in anyone with upper extremity pain of a chronic nature.

When your balance is impaired, you may feel unsteady, woozy, disoriented, have blurred vision, or have a sensation of movement. It may seem that the room is spinning. You may not be able to walk without staggering, or you may not even be able to get up. Sometimes nausea, vomiting, diarrhea, faintness, changes in heart rate and blood pressure, fear, anxiety, or panic accompany the dizziness and balance problems. The organ of balance is located in the inner part of the ear close to the auditory nerve. Many times, but not always, the cause of dizziness is in this part of the ear. Consequently, individuals who are experiencing a balance disorder often have a hearing loss in one or both ears. Tinnitus, which is often described as "ringing in the ears" or "head noise," may also occur. Dizziness can be associated with conditions that occur at all levels of the auditory system. In 1990, 15% of fatal road accidents were alcohol related and 32% of all pedestrians killed in road accidents were under the influence of alcohol. Drinkers may also choke on their own vomit whilst unconscious due to alcohol, but this is very uncommon. After a period of alcohol consumption a mixture of factors, mainly withdrawal and dehydration (alcohol dehydrates the body) may cause a hang-over. Physical and psychological dependency can occur (commmonly known as addiction). Deaths from suicide, accident and cirrhosis of the liver are common among heavy drinkers. Foot problems are a leading cause of hospitalization for the eight million persons in the United States who have been identified as having diabetes mellitus. Common problems include infection, ulceration, or gangrene that may lead, in severe cases, to amputation of a toe, foot or leg. Most of these problems are preventable through proper care and regular visits to your podiatric surgeon. This physician can provide information on foot inspection and care, proper footwear, and early recognition and treatment of foot conditions. Causes Foot problems in persons with diabetes are usually the result of three primary factors: neuropathy, poor circulation, and decreased resistance to infection. Also, foot deformities and trauma play major roles in causing ulcerations and infections in the presence of neuropathy or poor circulation. Neuropathy (Nerve Damage) Your ability to detect sensations or vibration may be diminished. Neuropathy allows injuries to remain unnoticed and untreated for lengthy periods of time. It may cause burning or sharp pains in feet and interfere with your sleep. Ironically, painful neuropathy may occur in combination with a loss of sensation. Neuropathy can also affect the nerves that supply the muscles in your feet and legs. This motor neuropathy can cause muscle weakness or loss of tone in the thighs, legs, and feet, and the development of hammertoes, bunions, and other foot deformities.
