A number of cases of persistent and intense limb pain, frequently following some local injury, are associated with vasomotor disturbances and trophic changes, due to irritation/damage of the peripheral sympathetic nerves. This results in abnormal distribution of blood flow between the skin and the deep tissues of the limb, including the bone. The persistent vasoconstriction of the bone arterioles may be responsible for the bone rarefaction, sometimes shown by radiographs in severe cases. Although this sign was once used as a diagnostic criterion, its rarity produces a considerable number of false negatives. Microvascular Investigations Thermography of the upper or lower limbs is a highly sensitive diagnostic tool for the Complex Regional Pain Syndrome. Limbs with sympathetically mediated pain tend to exhibit an erratic vasomotor behaviour, which is sometimes demonstrated as marked thermal instability by a high-sensitivity Infrared Thermographic Camera. This abnormal behaviour is best elicited by means of a mild cold stress to the contralateral (non-affected) limb. The affected side, instead of a parallel decrease of the limb temperature as observed in normal subjects, exhibits a temperature increase. In other less severe cases, areas of abnormal sympathetic activity are elicited subsequent to direct cold stress applied to the affected side. Finger Nailfold Capillaroscopy is usually performed when Thermography is normal, since a number of Connective Tissue Disorders with marked arthralgia can mimic the clinical features of Complex Regional Pain Syndrome.

In the seventeenth century, Ambroise Pire presented the earliest description of RSD with severe burning pain following peripheral nerve injury. Pire, a surgeon, treated King Charles IX for smallpox by inducing bleeding with a lancet applied to the arm. After this treatment, the King suffered from persistent pain, muscle contracture, and inability to flex or extend his arm. In 1864, Mitchell coined the term causalgia, which means burning pain, to describe persistent symptoms following gunshot wounds to peripheral nerves during the American Civil War.

In this prospective pilot study, nine patients suffering from complex regional pain syndrome of the arm were treated with morphine 0.16 mg h1 (3.84 mg day1) applied continuously through an axillary brachial plexus catheter. In all of them an oral analgesic medication including the less potent opioid tramadol had not provided sufficient pain relief. During regional treatment, patients were kept in hospital and physiotherapy was carried out frequently in order to improve strength and function of the affected arm. Pain at rest and during movement as well as grip strength were assessed at first visit, during morphine infusion and at a long-term follow-up visit. All assessments improved significantly during plexus analgesia. There were no major opioid related side-effects. Results: This pilot study indicates that continuous axillary brachial plexus analgesia with low dose morphine might be beneficial in patients suffering from complex regional pain syndrome of the arm.

Extractions: Complex regional pain syndromes (CRPS) (formerly reflex sympathetic dystrophy and causalgia) are neuropathic pain conditions that are initiated by an extremity trauma or peripheral nerve lesion. Clinical definition and scientific understanding of CRPS are still evolving; however, both the clinical picture and therapeutic options are significantly influenced by a dysfunction of the sympathetic nervous system. Recent investigations suggest functional central abnormalities and a peripheral inflammatory component in the pathophysiology of CRPS. Interdisciplinary treatment includes physical, pharmacologic, and invasive inter- ventional therapy, as well as stimulation techniques.

Extractions: Protocol Number: 02-D-0169 Complex Regional Pain Syndrome Proteomics Study 02-D-0169 This study will try to learn more about complex regional pain syndrome, or CRPS (previously known as reflex sympathetic dystrophy, spreading neuralgia, and sympathalgia), by examining the release of small proteins in the blood of patients with this condition. Patients with CRPS usually have three types of symptoms: - Sensory abnormalities - increased sensitivity to pain or a painful reaction to a harmless stimulus - Perfusion abnormalities - alterations in blood flow, temperature abnormality, swelling, decrease or increased nail growth, and hair and skin changes - Motor abnormalities - weakness, guarding (Holding the limb in such a fashion that it minimizes accidental or intentional contact from possible sources of pain), and atrophy (wasting) The cause of CRPS is unknown, and there are no definitive diagnostic tests for the condition. Because early treatment improves the prognosis of CRPS, a test that enables early diagnosis would be important for optimal medical management. The findings of this study may contribute to the development of such a test and possibly new drug treatments. Normal healthy volunteers and patients of any age with complex regional pain syndrome who are in otherwise good general health may be eligible for this study.
