Extractions: Physiological Psychology - University of Stirling Paul Graham Morris Background: Though we naturally enough tend to think of the experience of pain in negative terms, it actually serves a highly important and complex purpose in warning us of aversive or damaging stimuli. Individuals with congenital insensitivity to pain often suffer serious injuries or even death which they would probably have avoided if their warning system were active. Pain is rather more complex than one might think: * Amputees often experience pain (and other sensations) in the missing limb. This is termed phantom limb pain - see Melzack * Pain generated by receptors in the heart is often perceived as coming from the shoulder and arm. This is termed referred pain. * In times of war, soldiers have often been noted as being apparently oblivious to pain despite having suffered serious injuries. * Acupuncture in one region of the body can reduce the sensation of pain in another. * Hypnosis and placebo pills have both been scientifically observed to reduce pain in certain individuals. (See placebo effects p93-94 Carlson)

You will not be able to leave the examination room until 45 min. have elapsed (7:15) so members of the class aren't disrupted. Extractions: Congenital insensitivity to pain with anhidrosis (CIPA; MIM 256800) is an autosomal-recessive disorder characterized by recurrent episodes of unexplained fever, anhidrosis (absence of sweating) and absence of reaction to noxious stimuli, self-mutilating behaviour and mental retardation . The genetic basis for CIPA is unknown. Nerve growth factor (NGF) induces neurite outgrowth and promotes survival of embryonic sensory and sympathetic neurons . Mice lacking the gene for TrkA, a receptor tyrosine kinase for NGF , share dramatic phenotypic features of CIPA, including loss of responses to painful stimuli, although anhidrosis is not apparent in these animals . We therefore considered the human TRKA

Extractions: Congenital insensitivity to pain with anhidrosis (CIPA) is a very rare genetic disorder of the peripheral nervous system characterized by recurrent episodes of unexplained fever, generalized anhidrosis, insensitivity to pain and temperature, and accompanied by self-mutilating behavior and mental retardation. We report on a 16 month-old boy with CIPA who exhibited these characteristic clinical features. A sural nerve biopsy revealed markedly reduced numbers of unmyelinated and small myelinated fibers, consistent with the characteristic features of CIPA.
