Extractions: (advertisement) Synonyms, Key Words, and Related Terms: Crow-Fukase syndrome, Takatsuki syndrome, PEP, polyneuropathy endocrinopathy plasma cell dyscrasia, polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, skin changes Background: POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, skin changes) syndrome is a rare multisystemic disease that occurs in the setting of a plasma cell dyscrasia. The pathophysiologic link between the constellation of symptoms and the underlying disease is not well understood, but the link may be related to changes in the levels of a cytokine or growth factor. POEMS syndrome first was described by Crow in 1956 and then by Fukase in 1968. The syndrome was termed Crow-Fukase (by which it is known in Japan) syndrome by Nakanishi in a study of 102 cases in Japan. In 1980, the acronym POEMS was coined by Bardwick et al based on the 5 main features of the disease, namely, polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes. No specific case definition for POEMS exists; however, most authors agree that patients with POEMS syndrome should have 3 or more of the 5 systems involved. The pathophysiology of the disease is not known; however, elevated levels of cytokines and growth factors have been implicated. Polyneuropathy: The polyneuropathy associated with POEMS syndrome is a bilateral symmetric disturbance. It involves both motor and sensory nerves, begins distally, and has a progressive proximal spread. Associated cranial or autonomic nerves are not involved. Both demyelination and axonal degeneration are noted.

In this case, the concentration of vascular endothelial growth factor (VEGF) was extremely high in the serum, and the levels of IL-1À, IL-6, TNF-¿, and thiamine, which were thought in past reports to be mediators of PH in Crow-Fukase syndrome, were normal. After prednisolone therapy, PH disappeared with a dramatic decrease in serum VEGF. Our results suggest that VEGF is closely correlated with PH in Crow-Fukase syndrome.

High-output heart failure was diagnosed by right heart catheterization and she was treated with diuretics. After 3 weeks, her symptoms disappeared but a high cardiac output state persisted. A diagnosis of Crow-Fukase syndrome was made based on the presence of polyneuropathy, organomegaly, endocrinopathy, M-protein, and skin changes. Her serum vascular endothelial growth factor (VEGF) level was markedly elevated after recovery from heart failure. We suspect that an elevated VEGF level and a high cardiac output state may play a role in the pathogenesis of heart failure in Crow-Fukase syndrome.
