Cushing’s syndrome is a condition in which there is an excess of cortisol (cortisone). This can be caused by drugs (e.g. prednisone, depo-medrol, dexamethasone, betamethasone) often prescribed for the treatment of many diseases; this syndrome is known as iatrogenic Cushing’s syndrome. It can also be caused by an excess of the body’s own cortisol, caused by a pituitary tumor or adrenal tumor. Iatrogenic Cushing’s syndrome is caused by administration of drugs that suppress the body’s own cortisol production. Clinical signs and complications are similar to the naturally occuring disease. The dog can become dependent upon the drugs with chronic use, and abrupt withdrawal can lead to signs related to lack of cortisol, such as lethargy, depression, vomiting, and diarrhea. For these reasons, chronic steroid use is not recommended except in cases of life-threatening immune-mediated disease or chemotherapy protocols. The lowest possible dose of steroids should be used for therapy. Pituitary dependent Cushing’s disease is caused by a functional pituitary tumor that stimulates the adrenal glands to produce excess cortisol. The tumor is usually microscopic and benign (a microadenoma). Sometimes the tumor can be large enough to cause pressure on the nerves to the eyes and the part of the brain called the hypothalamus, causing behavior changes, lack of appetite, and blindness, in addition to the other signs of Cushing’s syndrome. These tumors are known as macroadenomas. Pituitary-dependent Cushing’s disease makes up about 85% of spontaneous Cushing’s disease cases.

Cortisol is a powerful steroid hormone, and excess cortisol has detrimental effects on many cells throughout the body. There are many clinical signs that might be seen with Cushing’s syndrome. These include increased water consumption and urination, ravenous appetite, pot bellied appearance, thin skin, muscle wasting or weakness, ligament tears (commonly the cruciate ligaments of the knees), hair loss, panting, lethargy, recurrent urinary tract infections or neurological abnormalities. Many pets exhibit several of the listed clinical signs, but rarely does any one pet have all of the signs. Cats with Cushing’s syndrome are often diabetic and also have very thin skin that tears easily. How is Cushing’s syndrome diagnosed? How is Cushing’s Syndrome treated? Ketoconazole is an antifungal medication, which interferes with the production of cortisol. This medication is much more expensive than Mitotane and often less effective, but it is a viable alternative, especially in the short term if the pet requires surgery or if there is an adrenal tumor that is too large or invasive to be removed. L-Deprenyl (Anipryl) is another medication that might be recommended for some pets with pituitary dependent Cushing’s syndrome. This medication may help to alleviate some of the clinical signs by decreasing the amount of ACTH released from the pituitary gland. This medication hasn’t proven to be effective in all pets with pituitary disease and doesn’t suppress cortisol production like Lysodren does, but it has the benefit of being less toxic.

It is also becoming more prevalent in horses, and yet there is little written about identifying it, and caring for horses who suffer from it.
