Regulation of AVP release: Integration of AVP secretion and thirst maintains plasma osmolality tightly at 280-290 mOsm/kg. Plasma osmolality: Anterior hypothalamic osmoreceptors (anterior to the 3 rd Plasma volume: Baroreceptors (stimulated by hypervolemia, inhibited by hypovolemia) inhibit AVP release via cranial nerves IX, X. The atrial cardiopulmonary low-pressure baroreceptors are less sensitive than osmoreceptors, requiring a 5-10% decrease in blood volume before AVP is released. Severe hypovolemia, however, triggers the sino-aortic high-pressure baroreceptors to cause exponential increases in AVP, which may be high enough to exert a pressor effect. Interaction of osmo- and baroreceptors: A decrease in left atrial pressure (as in hypovolemia, hypotension) leads to a reduction of the osmotic threshold and increases the sensitivity for osmotic AVP release. Volume expansion dampens the sensitivity for osmotic AVP release.

It is a disease characterised by the passing excessive amount of urine due to the under production of Anti Diuretic Hormone (ADH), which is produced by the HYPOTHALAMUS and is stored and released by the PITUITARY. What is the Hypothalamus? It is a part of the brain, which control the functions of the pituitary gland. What is the pituitary? It is a small pea size gland situated in a hollow bony pouch, at the base of the brain, at the back of the bridge of the nose. It is the master gland of the endocrine system and controls the functions of the other endocrine glands. What is Anti Diuretic Hormone and what are the functions? This hormone controls and regulates the fluid balance of the body. It is produced by the HYPOTHALAMUS and is stored and released by the PITUITARY. When released into the blood circulation it travels to the kidneys. You have two kidneys which cleanse one cup volume of fluid (150mls) each minute. ADH makes the kidneys reabsorb the fluid passing through the kidneys. Without ADH you would pass plenty of urine day and night as the kidneys have to rely on other systems to try and reabsorb fluid but these are not as effective as ADH What symptoms does the patient have with DI?

Extractions: Main Diseases and Conditions Diabetes inspidus (DI) is a disorder that causes increased urination ( polyuria ) and increased thirst . It is caused by either a decrease in the hormone arginine vasopressin ( central DI ), or because the kidney does not respond correctly to the hormone ( nephrogenic DI ). Arginine vasopressin is a hormone that normally stimulates the collecting ducts of the kidney to reabsorb water, so if it is not able to work correctly, the kidney produces a very large volume of dilute urine. Arginine vasopressin is normally produced in the hypothalamus , and so there can be a decrease in the its production caused by any condition that disrupts the hypothalamus, including brain tumors and head trauma, and some inherited conditions (septo-optic dysplasia). Nephrogenic DI can either be inherited or it can be a side effect of some medications, including Lithium, and some chronic conditions, such as sickle cell anemia, pyelonephritis, and urinary tract obstructions.

Central or vasopressin (ADH)-sensitive diabetes insipidus, which is a hypothalamic-pituitary disorder, is referred to in this discussion as diabetes insipidus (DI) to distinguish it from nephrogenic diabetes insipidus (NDI), in which the kidney is ADH-resistant (see Ch. 229 ). Polyuria may result from DI (a deficiency of ADH), from NDI, or from compulsive or habitual (psychogenic) water drinking (physiologic suppression of ADHalso termed primary polydipsia or dipsogenic diabetes insipidus). Etiology and Pathophysiology DI may be complete or partial, permanent or temporary. All of the pathologic lesions associated with DI involve the supraoptic and paraventricular nuclei of the hypothalamus or a major portion of the pituitary stalk. Simple destruction of the posterior lobe of the pituitary leads to temporary, unsustained DI. The posterior lobe is the major site of ADH storage and release, but ADH is synthesized within the hypothalamus. Newly synthesized hormone can still be released into the circulation as long as the hypothalamic nuclei and part of the neurohypophyseal tract are intact. Only about 10% of neurosecretory neurons must remain intact to avoid central DI.

Diabetes insipidus is a disease that is caused by a decreased amount of ADH (the anti-diuretic hormone) which is also known as vasopressin. The two main types of diabetes insipidus are central and nephrogenic. Central diabetes insipidus occurs when there is a decreased production of ADH by the posterior pituitary. ADH is usually secreted by the posterior pituitary into the bloodstream where it acts upon the kidneys to maintain normal water balance. Patients with central diabetes insipidus may have the disease if they have pituitary tumors or infections because compression could occur or there could be side effects of surgical exploration. Patients having meningitis may have central diabetes insipidus. Nephrogenic diabetes insipidus has a normal production of ADH, or even an increased production, yet the kidneys are resistant to the hormone. This kind of diabetes may be hereditary or may be brought on by such medicines as lithium. Symptoms Symptoms can occur quite abruptly, often after some form of trauma. When this occurs there is severe thirst, polyuria and nocturia. There is often a large output of dilute urine. There is often high volume urination at night of this watery urine and sometimes dehydration occurs with bouts of dizziness.
