Medical informaticians recognize that many of the critical parts of a computerized medical record exist in the form of free-text files. Fully automated encoding of such free-text files, with scientific measures of error-rates, is still in its infancy. One of the persistent problems with fully automated encoding is the common appearance of unexpanded abbreviations and uncorrected misspellings in such text files. It is impractical to enforce editorial constraints on busy practitioners in clinical services. Traditionally, human coders compensate manually for the editorial inadequacies of the original document-writers. However, such editorial oversight is not possible in fully automated encoding systems.

We had a copy of the Ultrasound report sent to us; the bowel appears isoechoic, the head revealed a cephalic index of 68%, consistent with "Dolicocephaly" and the nucal fold was assessed at 6.7 millimeters, a ratio of 1.75 (1.75 and lower is considered abnormal.) We really dont know what any of that means. I'm getting an Amnio done Thursday morning to see whats wrong, if anything. What is Ecogenic Bowel? and how bad is a Nucal fold measuring 6.7 mm at 20 weeks 4 days? I'm driving myself crazy here and with the Amnio getting closer and closer my nerves are getting worse and I'm even more stressd out. We want so badly for Jayme to be healthy.
