But a full-length photograph reveals that Michael has a rare medical condition. Known as pseudoachondroplasia, the condition causes a form of dwarfism that strikes one in 50,000 people. Jacqueline Hecht, Ph.D., associate professor in the Medical School department of pediatrics , leads a research team that recently uncovered the genetic link to pseudoachondroplasia. The discovery was reported in the journal Nature Genetics. After years of analyzing DNA samples from families like Michael's who are affected by the condition, the researchers pinpointed the gene that causes pseudoachondroplasia on chromosome 19. Identification of the gene will allow scientists to make preliminary inroads into understanding and someday preventing the condition. "We also hope that eventually a treatment will be developed to help people with pseudoachondroplasia to grow and escape some of the physical ailments associated with the condition," Hecht said. "In most cases, the disease occurs because of a spontaneous mutation in the gene and not because infants inherit it from their parents," Hecht said. I have mixed feelings about this book because the protagonist is far more bitter than myself or the other little people I know. But I think the book does a good job of exploring the issues little people do deal with as well as issues related to genetic diagnoses.

    Extractions: The Genetics of Dwarfism Basic research is transforming our understanding of how the activity of specific genes shape skeletal growth and development, according to Clair Francomano, who heads the Medical Genetics Branch at the National Human Genome Research Institute. Recent findings have thrown new light especially on achondroplasia, the most frequent and familiar form of dwarfism. Achondroplasia is fairly common, occurring in one out of between 15,000 and 40,000 live births. It is an autosomal dominant disorder with complete penetrance; that is, all who possess one copy of the gene also possess the disorder's characteristic short limbs and normal-sized trunk. A child born with two such genes, one from each parent, dies within a year or two of birth. Early in 1994, the achondroplasia gene was localized to the short arm of chromosome 4, and scientists focussed their attention on a candidate gene already known to lie in this region: FGFR3, a gene for a fibroblast growth factor. Two research groups have found mutations in that gene in people with achondroplasia, but no similar mutations in their unaffected relatives. The achondroplasia gene turns out to have some remarkable features. In many genetic disorders, different affected families have different mutations in the disease genea condition that makes development of tests for the defective gene very difficult. But not achondroplasia. In all cases studied so far, the mutation occurs in a single nucleotide, resulting in the substitution of arginine for glycine in the protein product.

    Extractions: Psychosocial Dwarfism: A Closer Look Patty Allgeyer, RN Cardinal Glennon Children's Hospital Review of the Annotated Bibliographies section in the article "Growth and Failure to Grow" by Patti Rieser (PENS Reporter; Fall 1997) cited an informative article by Stanhope (1994) regarding the diagnosis of psychosocial dwarfism. The Endocrine Service at Cardinal Glennon Children's Hospital (CGCH) has followed a very interesting case of psychosocial dwarfism. CM, a 7-year-old white female presented to CGCH in June 1995 for evaluation of short stature. She was brought to the endocrinologist by both parents at the suggestion of the primary care physician. The parents describe CM as a picky eater and allergic to cow's milk. They were concerned about other children "babying" her. The parents stated they had been working with the school to help CM with her "special needs." Physical exam revealed a very cooperative, quiet, poised child with a high-pitched squeaky voice. CM had the appearance of a 3 year old, rather than a 7 year old. Height measured at 95.8 cm (-4.5 standard deviations below the mean). Weight was 14 kg (-3 standard deviations below the mean). Review of past measurements revealed a cessation of growth between the ages of 5 to 7 years. Possible differential diagnoses included hypopituitarism, psychosocial dwarfism, and GI disorder. Screening labs were negative with the exception of IGF-1=18ng/ml (low) and T4=4.9 (low normal). Bone age was 3 years 6 months with chronological age being 7.5 years. Subsequent growth hormone stimulation testing X 2 revealed low peak growth hormone levels of 6.8 and 4.0 respectively. MRI was interpreted as normal.

But nowadays, it is considered more appropriate to refer to us as little people. We recognize as little people all those born with one of some 100 medical diagnoses related to dwarfism. In Quebec, 3 500 persons are living with a medical diagnosis always recognized by specific congenital malformations generated by one form of dwarfism. It is important to know that anyone can give birth to a child with dwarfism; 8 cases out of 10 are simply due to some sort of chromosomic disorder. However, one thing is common to all : those concerned are left with a profound weakness of their bone structure, always affec-ting the growth process, therefore creating the small stature we all recognize. So, as a reminder, never forget that those living with dwarfism are, medically speaking from their birth and all through their life, persons bearing physical as well as functional limitations, also called handicaps. This definition of dwarfism is incomplete without a second level of limitations, being the never ending list of social and environmental situations in which all little people are helpless. These are those realities in which normal duties such as reaching for commodities or facing public counters become total nightmares. We are confronted with a double handicap: one is related to our physiological deficiencies coupled with architectural difficulties where everything is always too high.

    Extractions: Little People of America, Inc. Little People of America Inc. (LPA) is a genetic support group with a membership of over 5000, primarily individuals with dwarfism and their families. For 35 years it has been led by adults with various forms of short stature. The membership is diverse in background, profession and disability. However, there is a common feeling of self-acceptance, pride and community that has been compared to more traditional disability groups and the deaf community. The concept of a dwarf community is illustrated in part by the dozens of dwarf children from all over the world adopted by LPA members. However, perspective on the idea of community would be different for the 90% of people with short stature who are not involved in LPA, as membership in LPA denotes a self-selection or identification process. Some short statured people are involved in the larger disability community and not in LPA. Dwarfs and other persons of short stature vary as to whether they identify as having disabilities. This seems to be evolving. In the younger people who have been able to benefit from disability rights legislation there is more comfort and identity as people with disabilities. It seems that the dwarf community, and possibly other genetic support group communities, differ from the wider disability arena in their perspectives on the medical field. We tend to have positive, ongoing relationships with the geneticists that work with our families (short and average size parents). Some of the leading geneticists serve as a sort of primary care/specialist for dwarf children whom they see in their clinic, monitoring when and if intervention by other specialists is warranted (neurology, orthopedics, etc.).

"It needs to be reinforced that dwarfism is not a death sentence and life remains very much full of promise," Francis Kelly, chair of Short-Statured People of Australia (SSPA), told The Age . "We're short. So what?" Kelly added that most people with dwarfism, who grow no taller than 4 feet, 11 inches at maturity, "lead healthy, happy, and fulfilling lives." The doctors - - an obstetrician, a clinical geneticist, and a psychiatristconsidered the baby's mother "suicidal" when she demanded an abortion in January, The Age reported. The hospital's medical director, Glenn Bowes, told the newspaper that the woman had no previous history of mental illness. Abortions are allowed in the state of Victoria, where the hospital is located, at any stage of pregnancy if a woman's life or physical or mental health is endangered, according to a 1969 state Supreme Court decision.

There are many forms of dwarfism. The word pituitary is in reference to the pituitary gland in the body. This gland regulates certain chemicals (hormones) in the body. Therefore, pituitary dwarfism is decreased bodily growth due to hormonal problems. The end result is a proportionate little person, because the height as well as the growth of all other structures of the individual are decreased. Description Pituitary dwarfism is caused by problems arising in the pituitary gland. The pituitary gland is also called the hypophysis. The pituitary gland is divided into two halves: the anterior (front) and posterior (back) halves. The anterior half produces six hormones: growth hormone, adrenocorticotropin (corticotropin), thyroid stimulating homone (thyrotropin), prolactin, follicle stimulating hormone, and lutenizing hormone. The posterior pituitary gland only produces two hormones. It produces antidiuretic hormone (vasopressin) and oxytocin. Most forms of dwarfism are a result of decreased production of hormones from the anterior half of the pituitary gland. The most common form is due to decreases of growth hormone which will be discussed here. These decreases during childhood cause the individual's arms, legs, and other structures to develop normal proportions for their bodies, but at a decreased rate.
