Causes and Risks The underlying causes of prune belly syndrome are unknown. There is blockage to the flow of urine from the kidney to the outside of the body during fetal life. For example, dilatation of the urinary tract may develop as a result of urethral obstruction in the womb. The incidence is approximately 1 in 30,000 to 40,000 births. Boys are almost solely affected. Expectant mothers carrying affected infants may develop varying degrees of oligohydramnios (insufficient amniotic fluid ) that predisposes the infant to lung problems (see Potter syndrome ). The newborn infant has a wrinkled abdomen (it looks like a prune). This appearance results from swelling with fluid in the womb then loss of that fluid after birth leading to wrinkles of excess skin. The appearance is accentuated by the lack of adequate abdominal musculature. Genital abnormalities may be prominent as undescended testes . Approximately one third of the infants are either stillborn or die within the first few weeks of life from severe lung, kidney, or combined problems.

Urinary tract anomalies (mega-ureters, large bladder). Bilateral cryptorchidism (undescended testicles). The incidence of prune belly syndrome is 1 in 40,000 live births; 95% of cases occur in males. In the past, it was suggested that the abdominal wall was deformed by pressure from a distended bladder due to bladder outlet obstruction in utero. Currently it is thought that prune belly syndrome is a multisystem disease complex which derives from a primary defect in mesodermal development. The major prognostic factor is the degree of dilation of the urinary tract; 20% of patients are stillborn, 30% die of renal failure or urosepsis within the first two years of life, and the remaining 50% have varying degrees of urinary pathology. Renal abnormalities vary widely. There may be agenesis or hypoplasia, which is rare, or hydronephrosis. On IVP, 80% of patients have evident tortuous ureters with dilatation distally. Vesicoureteral reflux is common. The bladder is generally dilated, and the trabecula are thinned. A patent urachus may allow the bladder to decompress.

Some authors call for conservative management of the urinary tract in boys with prune belly syndrome, while others advocate an aggressive approach, operating on patients aged 10 days. No definitive timing for therapy has been substantiated. Pediatric urologists have observed that PBS can present with a spectrum of abnormalities, ranging from an infant that is stillborn and has severe urogenital and pulmonary problems to a child with few, if any, urological abnormalities who requires no therapy other than orchidopexies to correct the undescended testes. The decision to aggressively treat the urinary tract of these children should be based on the clinical presentation and not solely on radiographic appearance. Problem: Children with PBS can present with myriad renal, ureteral, and urethral abnormalities. Obstruction and/or upper urinary tract dilatation is not unusual in these children. Occasionally, the site of obstruction can vary from as high up as the pelviureteral junction to as far down as the prostatic membranous urethra.

Some have a utricle diverticulum from the urethra; a large, vertically oriented, thick-walled bladder; a urachal remnant from the dome of the bladder; and tortuous and dilated ureters. Varying amounts of hydronephrosis and varying degrees of renal dysplasia are seen. All have cryptorchidism. Amniotic fluid volume may be normal or decreased in neonates with prune belly syndrome. The presence of oligohydramnios may account for some of the accompanying findings of the extremities. Pathophysiology: The etiology of prune belly syndrome is not fully understood. The thinned abdominal wall has been attributed to hydronephrosis and the distended urinary system to interfering with normal descent of the testes.
