Clinical Research Team Essential tremor (ET) is considered the most common neurologic movement disorder. It affects an estimated 10 million people in the United States. ET is a chronic condition characterized by involuntary, rhythmic tremor of a body part, most typically the hands and arms. In most patients, ET is considered a slowly progressive disorder and, in some patients, may eventually involve the head, voice, tongue (with associated dysarthria), legs, and trunk. However, in many people, the disease may be relatively non-progressive and the tremor may be mild throughout life. All tremors usually disappear during sleep. Unlike the resting tremor associated with (during which muscles are not voluntarily activated), the symptoms of essential tremor are either absent or minimal during periods of rest. As the disease progresses, individuals with ET experience varying degrees of functional disability and resultant handicap based on the severity of the tremor.

ETs often called Benign Essential Tremor, because it usually doesnt result in marked or striking disability. Few ET patients become disabled; most are able to cope. ETs sometimes called Familial Tremor, or Hereditary Tremor, or Heredito-Familial Tremor because in approximately 50% of patients theres a family history. ETs sometimes called Senile or Senescent Tremor because it usually occurs in senior citizens. Senile or Senescent refers to the age at ET occurs, not to the patients mental capacity. Because of the misleading association of the terms senile or senescent tremor with senility or mental decline the term Senile or Senescent Tremor isnt used. Description PD Questionnaire ETs a chronic disorder characterized by involuntary rhythmical trembling or oscillation of a body partusually the hands. The trembling usually begins on both sides symmetrically unlike the trembling of PD which begins on one-side. ET may begin insidiously with the patient unaware of (or in denial of) of the trembling until its pointed-out. ET usually begins in the 2nd, 3rd, 4th or 5th fingerwhere its apt to go un-notice. This is unlike the trembling of PD which usually begins in the thumb and is more apt to be noticed. The trembling in ET may, for years, remain unchanged in amplitude, frequency and distribution. Or the trembling may slowly change in amplitude and distribution. Or the trembling may increase in amplitude and distribution. The changes are rarely rapid or striking.

The abstract is reprinted as a service to our readers Background: There is clinical variability in essential tremor (ET), but it is not clear whether this variability is because of the existence of distinct clinical subtypes of ET ( forms of ET that may differ in their etiology, rate of progression, or response to treatment). Objective: To examine in a group of ET cases the age of onset, anatomic distribution, and rate of progression of tremor, and to look for associations between these factors. Methods: Cases of ET were ascertained from a community and a tertiary referral clinic in Northern Manhattan, New York, NY. A total of 115 patients were examined. All subjects underwent an interview and videotaped tremor examination. Rate of progression was estimated based on the tremor severity and reported disease duration at the time of evaluation. Results: There were differences in the anatomic distribution of the tremor (arm tremor only versus head and arm tremor versus isolated head tremor). There was a large cluster of subjects with slower rates of progression, and a smaller number who had faster rates. There was an association between age of onset and rate of progression; cases with older age of onset (60+ years) progressed more rapidly. In addition, upper limb tremor progressed more slowly among those with associated head tremor.

A: Essential tremor is also known as "familial tremor," because it often occurs in several members of the family. About half of the cases are directly inherited, and the other half arise spontaneously. The head and hands are often affected, and even the voice may be involved (Katherine Hepburn is a good example of that). Although the tremor may start at any age, it usually begins in young adulthood and persists throughout life. Obviously this can be quite embarrassing, since it may look like the person is very nervous because of the shaking. It is not more common in women than men and does not necessarily get worse with age. One or two drinks of alcohol may eliminate the tremor briefly, but it will become worse after the alcohol has worn off. There are a few treatment options you could try. Propranolol is a heart/blood pressure medicine that will decrease the tremor in some people, especially if it is a fast fine tremor with a frequency of about 8 shakes per second. Mysoline, which your doctor prescribed, is probably the drug of choice. A dose of 50 mg can be taken at bedtime and increased gradually up to 250 mg if necessary.

These diseases are Parkinson's disease and essential tremor (familial tremor). In unusual circumstances, movement disorders may also be due to head injuries, brain infections such as encephalitis, or following strokes. Movement disorders take a wide variety of forms. One of the most common movement disorders is tremor. In Parkinson's disease, the tremor or shaking usually involves the arms and hands and is most prominent when the patient is at rest. In essential or familial tremor, the hands and arms are usually quiet at rest, but if the patient attempts to perform a task, such as picking up a glass of water or writing, then the tremor becomes very noticeable. Tremor can sometimes involve the head and neck and also sometimes involves the legs. Tremor interferes with many normal activities. Patients may be unable to write their name and, for instance, sign checks. They may be unable to drink from a cup or glass without spilling, and they may be unable to cut food with a knife and fork. In addition, patients may be unable to feed themselves, button clothing, comb their hair, or perform almost any movement that involves coordinated control of the hands, fingers and arms.

Extractions: "Geniospasm" was coined in 1894 for a hereditary trembling of the chin. 27 families from Europe and the USA with more than 350 affected members have been identified since. The peculiar tremor disorder is transmitted as an autosomal-dominant trait with approximately 80% penetrance and slight male preponderance. Its molecular basis is unknown as is a possible relationship to essential tremor. Ten patients (7 male, 3 female) from three families were studied in detail. The youngest subject was 4 months old, the oldest 68 years. All showed the typical intermittent, usually stress-induced quivering of the skin overlving the chin. This correlates with 8-30Hz discharges of groups of motor units in the mentalis muscle bilaterally. From clinical examination, there was no evidence for additional nervous system abnormality, although abnormal EEG, sleep disorders and involvement of other facial muscles have been described in rare cases of this essentially benign condition. In a single patient, local botulinum toxin injections succeeded in temporary abolishment of the symptom.

Hi Everyone, "Essential Tremor" was a new term to me until I met Rhonda Noble. (she gave permission to use her name) It manifests as a shakiness in the hands and typically shows up while drinking a glass of water or eating soup with a spoon. For some people it shows up in their handwriting. It is publicly embarrassing. For Rhonda, it was particularly noticeable when in public situations or when she felt pressure to perform. She took great pains to hide it from others by avoiding the activities that brought it about. Her doctors told her it was hereditary and that she would only get worse with time (she is 47 so, presumably, that gave her lots of time to suffer). Apparently, most of the women in her family have it. It's a sort of family curse. Many take medications for it with no lasting results (but several negative side effects). After some creative applications of EFT, however, the problem seems to have vanished. No medications. Nothing but tapping.

Extractions: Hi,My names is james and I am a student and i think i have essential tremor disorder. My hands and fingers shake, fairly rapidly and this started about four months ago. I am 18 yers old and at first thought that the shaking was due to my drinking which is excessive. At first I only noticed it when I went to take a drink from a water dispenser. However now it has progressed so that I notice it when I eat, drink etc. I used to work as a waiter and have now had to give that up as I felt it was being noticed by the customers and was becoming embarrassing. I have been to a doctor who also had the problem, which I thought was odd for a doctor, who diagnoised it as a Benign central tremor. He seemed to indicate thaT I should just live with it. however being a student I am finding it very difficult to just ignore it. It has now reached the stage where i have to have 4-6 cans of beer before I can go out anywhere. I also smoke, and although I would not say i was overweight I would say I was unift.
