Thalamotomy involves the precise placement of a lesion in the ventral intermediate nucleus of the thalamus using stereotactic neurosurgical techniques guided by microelectrode recordings. Thalamic stimulation is an alternative procedure whereby a stimulating electrode, implanted in the ventral intermediate nucleus of the thalamus using techniques similar to those for thalamotomy, provides chronic electrical stimulation to that region of the thalamus.

All 11 evaluable patients noted improvement in action tremor. Six of eight ET patients had complete tremor arrest, and the violent action tremor in all three patients with MS was improved. One patient developed transient arm weakness. Stereotactic radiosurgery for ET and MS-related tremor is safe and effective for patients who may be poor candidates for other procedures.

An essential tremor is a relatively benign condition affecting movement or voice quality, but with no other effects. It involves a rhythmic, moderately rapid tremor of voluntary muscles. Purposeful movements may make the tremors worse. There may be difficulty holding or using a small object (such as eating or writing utensils). Emotional stress may also increase the tremors. Over time, the tremors may affect the hands, arms, head, voice box (larynx), eyelids, or other muscles. An essential tremor rarely involves the legs or feet. The cause is unknown. It usually effects the upper extremities, head, and voice. The course of essential tremor is variable, but it usually increases gradually over time and becomes a potential for functional disability and social embarrassment. Nearly half of the patients with essential tremor presenting for treatment can not be controlled with currently available medical therapy, therefore many patients become surgical candidates in the long term. Stereotactic movement disorder surgery; namely thalamotomy and thalamic deep brain stimulation are proven to be very effective in controlling essential tremor. Patients having bilateral symptoms or head tremor may benefit from bilateral surgery. This paper reviews the common features and new surgical treatment alternatives for essential tremor. Essential tremor (ET) is a nerve disorder characterized by uncontrollable shaking, or "tremors", in different parts and on different sides of the body. Areas affected often include the hands, arms, head, larynx, or voice box (making the voice sound shaky), tongue, chin and other areas. Rarely, the lower body is affected. ET is not a life-threatening disorder, unless it prevents a patient from caring for him/herself. Most people are able to live normal lives with this condition although they may find everyday activities like eating, dressing or writing difficult, which leads them to withdraw socially. But it is only when the tremors become severe that they actually cause disability. Symptoms of Essential Tremor The term "tremor" is used to describe the uncontrollable shaking, but that's a symptom that can be caused by a multitude of different factors and diseases including Parkinson's disease, multiple sclerosis, fatigue after exercise, extreme emotional distress, brain tumors, some prescription drugs, metabolic abnormalities, and alcohol or drug withdrawal.
