The medicines have been donated by the Sasakawa Memorial Health Foundation given through WHO to almost all of the 55 endemic countries. The Pacific island countries of FSM and RMI became a part of the WHO sponsored multi-drug programs later than most countries and have not yet received the benefits of well focused public health programs. Thus new patients will continue to be diagnosed in Hawai`i for many years as people from these countries enter Hawai`i for reasons of employment, education, and relocation.

The limited tuberculoid form of the disease is characterized by one or a few hypoesthetic skin lesions and palpably enlarged nerves. The more extensive lepromatous form of the disease appears as multiple nodular skin lesions, sometimes with involvement of the nasal structures and eye. Current recommended regimens for treatment of Hansen's disease are based on combinations of dapsone, rifampin, and clofazimine. Recent advances in treatment have resulted in a significant decline in the worldwide prevalence of the disease. Hansen's disease is a major international health problem. Recent advances in treatment have significantly reduced the disease prevalence, but there are still an estimated 2.4 million cases worldwide.1 Although it is a rare disease in the United States, with 2,217 cases reported to the Centers for Disease Control and Prevention from 1984 to 1993,2 primary care physicians in endemic areas or those who treat immigrants may encounter cases of Hansen's disease. Ninety percent of US cases involve patients who have lived in foreign countries where the disease is endemic, primarily Asia, Africa, and Latin America. Hansen's disease is also endemic in Texas, Hawaii, Louisiana, and California.3 Contact with armadillos has been identified as a possible mode of transmission of the disease to humans.

When violently confronted by armed police, the lepers, many without fingers and toes, defended themselves with sticks, stones and farm implements. Hansens disease, arrested among the Montagnards during the Vietnam War with American medical assistance, has now reached epidemic levels, treated by the communists with only benign neglect. The demonstrations continued to grow and culminated in March of this year when tens of thousands spontaneously turned out in most all of the Central Highland provinces. These protests started when two Montagnard Protestant Pastors were arrested in Pleiku province after complaints over the burning of their house-churches (private residences where services are held). With their ancestral lands confiscated for coffee plantations, the Montagnard are no longer able to grow enough food for subsistence. This has led to wide-scale malnourishment and starvation among the Montagnards, while the government officials get rich from coffee sales. The communist's response to their hunger was, "Since you are Christians, let your God Jesus feed you from the sky." On February 8, Y Luyen Nie, the President of the Darlak Provincal Council, announced, "We will severely punish Christian believers in the Western Highlands until they will no longer be able to raise their heads." According to a recent report to the U.N. Commission on Human Rights in Geneva, Vietnamese authorities crucified three members of a church group in Phu Thien, Gialia province.
