Howard Hughes Medical Institute researchers have discovered how an inherited disorder triggers heart failure by disrupting the flow of calcium in heart muscle cells. The researchers say that the finding could lead to targeted treatment for dilated cardiomyopathy, a disorder that causes the heart to become enlarged to the point where it can no longer pump blood efficiently. Although the discovery was made during studies of an inherited form of heart failure, the researchers suggest that this molecular defect may help explain other forms of heart failure - an emerging epidemic that affects 4.7 million Americans and costs $17.8 billion annually. Promising results from these studies plus extensive pre-clinical testing and scientific discovery paved the way to begin worldwide, multi-center, randomized clinical trials. These trials are designed to confirm the safety and further evaluate the efficacy seen in earlier, non-randomized studies of the CorCap. The randomized trial in the United States began in June of 2000 and expanded to 18 centers in 2001. Nine additional centers will be added in 2002. Over 155 implantations of the CorCap have been performed to-date and over 190 patients have been enrolled in Acorn's safety and randomized clinical studies. Acorn has obtained CE Marking for the CorCap in Europe. In addition, Acorn has achieved ENISO 9000 and EN 4600 Quality System Certification. Acorn is in the process of setting up European distribution to facilitate commercialization of the CorCap. Much progress has been made in the last two decades in understanding and treating the heart failure syndrome, especially in the areas of symptom relief and prevention of disease progression. HHMI researchers have discovered a genetic mutation that damages heart muscle so that it dilates to the point where the heart can no longer pump blood. The discovery may have immediate significance for millions of people affected by dilated cardiomyopathy, says senior investigator Mark Keating of the Howard Hughes Medical Institute at the University of Utah. Although there is typically a poor prognosis for patients with dilated cardiomyopathy, Keating says that his team's research suggests that damage may be mitigated by relaxing cardiac muscle with beta blockers, drugs commonly used for hypertension. "If we can help the heart to relax a bit before the damage is really serious, we might be able to reduce the rate of cell death and slow down the course of the disease," Keating said. Keating's team published their research in the May 1, 1998, issue of Science.

Backup of blood in the veins, together with retention of fluid by the kidneys, causes the body's tissues to swell. The swelling (edema) most commonly affects the legs, but it can also occur in the lungs (causing breathing difficulty) and in other tissues and organs.

But a new study suggests that some heart problems can be prevented with diuretics that cost as little as $6 a month. In a study reported Tuesday in the Journal of the American Medical Association, researchers found that diuretics used to treat people with high blood pressure significantly reduced the risk of heart failure among older patients. Heart failure afflicts more than 2 million people each year in the United States, and results in approximately one million hospitalizations. CNN's Rhonda Rowland reports 2 min., 2 sec. VXtreme streaming video Researchers tracked 4,376 people age 60 or older who participated in the Systolic Hypertension in the Elderly Program (SHEP). Participants were randomly given the diuretic chlorthalidone, or a placebo. Heart failure, fatal and non-fatal, occurred in 55 of the 2,365 given the diuretic, or just 2.3 percent. Of the 2,371 receiving the placebo, however, 105, or 4.4 percent, suffered heart failure.

Extractions: Congestive heart failure is the term used to describe the inability of the heart to pump as much blood as the body needs. Its symptoms are widespread weakness, shortness of breath, swelling in the legs, abdominal discomfort or pain, and a loss of appetite. Despite its pronouncement-of-doom sound and the wide range of symptoms, congestive heart failure is generally a treatable condition although calling it a "condition" is not exactly accurate. Congestive heart failure itself is a symptom of some underlying condition which is depriving the heart of the strength to meet the body's metabolic demands. The most frequent cause of the condition in older patients is atherosclerosis the progressive narrowing of the heart's own arteries by cholesterol plaque buildups, which starves the heart itself for oxygen and nutrients. In younger patients, it is more likely to be from a faulty heart valve or from cardiomyopathy damage to the heart muscle from an infection or other cause. The task of the cardiologist is to find and treat that underlying cause. In the case of atherosclerosis, balloon angioplasty to reopen the blocked arteries or bypass surgery to route blood around the constriction will often relieve the symptoms of congestive failure. In valve defect cases, valvuloplasty replacement of a defective valve may be needed. Cardiomyopathy may be addressed with medication, though, in extreme cases, the diseased heart may be incapable of recovering, and heart transplantation may be indicated.

The study, covering years 1988 through 1995, found deaths from heart failure decreased 3 percent a year for black men and 2.2 percent a year for black women. For all elderly adults, heart-failure deaths fell 8 percent in the period. But the report also says that although death rates are down, the number of heart failure cases is rising. Researchers said possible reasons for the decline include improvements in treating risk factors, increased access to appropriate medical care and changes in death certificate coding. Kathy Harben of the CDC said doctor education is still an obstacle in treating heart failure.

An interruption in normal breathing patterns during sleep which is often seen in heart failure patients may contribute to heart failure rather than just being a result, according to findings of a Mayo Clinic collaborative study published this week in Circulation: Journal of the American Heart Association. "The goal of medicine is to not just treat symptoms of illness, but to find underlying causes," says Virend Somers, M.D. Ph.D., a Mayo Clinic cardiologist and one of the studys authors. "In this instance we are now finding that central sleep apnea, which has been previously understood as a symptom of heart failure, may contribute to the development of heart failure in people at risk." Heart failure is the most common cause of hospitalization in people over age 65, and people who reach age 40 have a 1-in-5 chance of eventually developing heart failure. Symptoms include fatigue, shortness of breath and swelling in the legs and abdomen. Sleep apnea is an interruption of airflow for 10 seconds or more during sleep, which can cause significantly lower oxygen levels in the bloodstream. Patients with severe apnea have 30 or more of these episodes per hour, while those with mild apnea stop breathing at least 15 times. In obstructive sleep apnea (OSA) the chest and abdomen move normally, but a blocked airway prevents the patient from breathing. Central sleep apnea (CSA) differs in that the patients airway is not obstructed, but the bodys breathing reflex is periodically interrupted so there is no chest and abdomen movement.
